Stunting remains one of the health problems among children under five that requires attention, including in the working area of Siatas Barita Public Health Center. Based on data from Siatas Barita Public Health Center, Stunting cases in Siraja Hutagalung Village were still found every year, with 17 children under five in 2023, 18 in 2024, and 17 in 2025. This condition indicates that Stunting remains a health problem that needs attention, particularly in relation to environmental hygiene and sanitation. An unhealthy environment and poor hygiene practices may increase the risk of infections that interfere with nutrient absorption and child growth. This study aimed to determine the relationship between environmental hygiene and sanitation and the incidence of Stunting among children under five in Siraja Hutagalung Village, North Tapanuli Regency. This study used a quantitative method with an analytical observational design and a cross-sectional approach. The study population consisted of 109 children under five, with a sample of 75 children. Data were collected through questionnaires and observations and analyzed using the Chi-Square and Fisher’s Exact tests. The results showed that 14 children (18.7%) were stunted. Significant associations were found between handwashing with soap practices (p=0.032; PR=3.167), cleanliness of eating utensils (p=0.021; PR=3.021), maternal nail cleanliness (p=0.019; PR=3.268), latrine ownership and condition (p=0.002; PR=4.444), household waste management (p=0.001; PR=5.816), and wastewater drainage conditions (p=0.000; PR=13.351) and Stunting. However, there was no significant association between clean water sources and Stunting (p=0.356). In conclusion, environmental hygiene and sanitation were associated with Stunting among children under five. The community is encouraged to improve clean and healthy living practices and environmental sanitation as efforts to prevent Stunting.
