The global Sustainable Development Goals (SDGs) target for 2030 aims to reduce the Maternal Mortality Rate (MMR) to 70 per 100,000 live births and the Infant Mortality Rate (IMR) to 12 per 1,000 live births. However, the IMR in Paser Regency remains around 16 per 1,000 live births, indicating the need to improve the quality of maternal and neonatal healthcare services through the implementation of Continuity of Care (CoC). This study aimed to provide comprehensive and continuous midwifery care for Mrs. I from pregnancy, childbirth, postpartum, neonatal care, to family planning services. This research used a descriptive case study approach with SOAP documentation methods. Data were collected through interviews, observations, physical examinations, supporting examinations, documentation studies, and literature reviews conducted from February to May 2026. The results showed that during pregnancy, the mother experienced increased urinary frequency, especially at night, and was provided with health education and Kegel exercises. The labor process occurred normally with the implementation of Normal Delivery Care (APN) and complementary counterpressure therapy. During the postpartum period, the mother experienced breast engorgement characterized by inadequate breast milk production, hot, swollen, full breasts, and tenderness, which was managed through breast care interventions. Neonatal care findings were within normal limits, and the baby received vitamin K 1 mg, Hepatitis B0 immunization, and newborn screening examinations. In family planning services, the mother chose the 3-month injectable contraceptive method. Healthcare providers are expected to optimize comprehensive screening and integrate complementary care to improve maternal and neonatal health outcomes.
