The National Formulary Restrictions (FORNAS) regulate medication use within Indonesia's National Health Insurance (JKN) program, including indications, quantities, treatment duration, and specific clinical requirements. Non-compliance with FORNAS restrictions may result in medication claims being denied by BPJS Health and may affect hospital financing efficiency. This study aimed to analyze prescription compliance with FORNAS restrictions among JKN patients at the Cardiology Outpatient Clinic of Hospital X in Semarang from July to December 2022. This retrospective observational study used outpatient prescription data from the Cardiology Clinic. Samples were selected using purposive sampling according to the inclusion criteria. Data were analyzed descriptively by calculating the proportion of prescriptions compliant with FORNAS restrictions. The compliance rate was 82.97%, while 17.03% of prescriptions did not meet the restrictions. Non-compliance was mainly identified in antihyperlipidemic, antiplatelet, and antihypertensive drugs due to inappropriate indications, treatment sequencing, duration, or unmet clinical requirements. These findings indicate that most prescriptions complied with the restrictions, although implementation gaps remain in clinical and administrative verification. Prescription compliance with FORNAS restrictions at the Cardiology Outpatient Clinic of Hospital X in Semarang was not yet optimal; therefore, periodic evaluation and stronger implementation of FORNAS are needed in JKN patient care.
