Leaf spot disease is a major constraint in oil palm nurseries because it can reduce seedling quality and impair plant growth. This study aimed to identify fungi associated with leaf spot disease and to determine the incidence and severity of the disease in selected oil palm nurseries in North Aceh and Bireuen Regencies, Indonesia. A survey was conducted at three nursery sites: a farmers’ nursery in Sawang District, PT Syaukath Sejahtera in Gandapura District, and CV Buket Indah in Juli District. Seedlings aged 4, 7, and 11 months were examined. Fungal identification was conducted through isolation on Potato Dextrose Agar (PDA), macroscopic and microscopic morphological characterization, and pathogenicity testing based on Koch’s postulates. Four fungal genera associated with leaf spot symptoms were identified: Curvularia, Pestalotiopsis, Phoma, and Nigrospora. Disease incidence ranged from 82 to 100%, while disease severity ranged from 25.00 to 45.50%, generally falling within the moderate severity category. The highest disease incidence and severity were observed in the farmers’ nursery in Sawang and at CV Buket Indah. These nurseries had conditions that were more conducive to disease development, particularly high plant density, humidity, sanitation, and water management. The findings highlight the importance of accurate etiological identification and improved nursery management for integrated management of leaf spot disease.
