Introduction: Social anxiety is a psychological problem commonly experienced by female adolescents. It is influenced by low self-confidence, social pressure, academic demands, peer relationships, and family parenting patterns. Non-pharmacological interventions such as Islamic Butterfly Hug are considered effective in reducing social anxiety. Objective: To determine the effect of Islamic Butterfly Hug therapy on social anxiety levels among female adolescents at Imam Syafi'i Islamic Boarding School, Brebes. Methods: This study employed a quasi-experimental pretest-posttest control group design. The sample consisted of 50 female adolescents selected through total sampling. The instrument used was the Social Anxiety Scale for Adolescents (SAS-A). Data were analyzed using the paired sample t-test. Results: Before the intervention, the experimental group showed 16.0% very high, 28.0% high, and 56.0% moderate social anxiety. After therapy, 80.0% had low anxiety and 20.0% had very low anxiety. In the control group, anxiety levels showed minimal change. Statistical analysis revealed a significant reduction in social anxiety in the experimental group (p = 0.000; p < 0.05), while no significant change was found in the control group (p = 0.742; p > 0.05). Conclusion: Islamic Butterfly Hug therapy is effective in reducing social anxiety among female adolescents in Islamic boarding schools and may serve as a safe, simple, and spiritually appropriate non-pharmacological intervention.
