This study examines the factors shaping patients’ hospital choices in Jakarta. It contributes to healthcare choice research by clarifying how financial access, service attributes, institutional characteristics, and operational efficiency jointly influence hospital selection in a major urban healthcare market. Data were collected from 430 patients through an online questionnaire and analyzed using PLS-SEM with SmartPLS. The findings show that insurance options, service quality, hospital facilities, hospital type, and waiting time significantly influence hospital choice, whereas treatment cost does not. Insurance accessibility emerges as the most influential consideration, followed by service quality. The findings indicate that patients evaluate hospitals beyond direct treatment expenses and place greater emphasis on accessibility, service experience, institutional characteristics, and operational convenience. Hospital managers should therefore strengthen insurance accessibility, service quality, facility provision, and waiting-time management to improve patient preference and healthcare access. Â
