This study aims to analyze the relationship between resilient safety culture and Safety Performance and to identify priority areas for improving safety culture dimensions at Siti Hajar Hospital, Sidoarjo, using the Importance-Performance Map Analysis (IPMA) approach. Resilient safety culture plays an important role in protecting healthcare workers from occupational risks amid the complexity of hospital services. This study employed a quantitative approach with an explanatory-causal design. The population consisted of all employees of Siti Hajar Hospital, with 150–200 respondents selected through proportional stratified random sampling. Data were collected using a structured questionnaire with a 5-point Likert scale measuring the importance and performance of safety culture dimensions. Data analysis was conducted using Structural Equation Modeling-Partial Least Squares (SEM-PLS) followed by IPMA analysis. The results showed that resilient safety culture has a positive and significant relationship with Safety Performance. IPMA results indicated that the Monitoring dimension is the main priority for improving Safety Compliance, while the Responding dimension is the priority for improving Safety Participation. Therefore, hospital management is encouraged to strengthen safety monitoring systems, improve incident response capacity, and maintain employee participation through continuous safety programs and appreciation initiatives.
