<![CDATA[Elderly health services require longitudinal monitoring, yet paper-based records are slow to retrieve and make changes across visits difficult to review. This study designs and develops a web-based elderly health monitoring information system for a community health centre in Medan, Indonesia. The study uses a Research and Development approach. Functional requirements were elicited through semi-structured interviews, observation, and document review, then translated into system functions, data structures, and operational classification rules. Development followed the Waterfall model using Laravel, MySQL, and Chart.js, with Use Case and Class Diagrams for system modelling. The system records five examination parameters: blood pressure, heart rate, body temperature, body weight, and blood glucose. Four parameters are classified automatically using explicit rule-based thresholds derived from clinical references, whereas body weight is displayed as a longitudinal trend without status classification. Black-box testing of 11 scenarios showed that all tested functions behaved according to specification, including boundary-value classification. The system integrates elderly records, condition labels, filtering, and trend visualisation for healthcare officers. It is positioned as an administrative monitoring and documentation aid rather than a clinical decision-support system. Usability, user acceptance, security, performance, and before-after service evaluation remain necessary before claims regarding operational effectiveness can be made.]]>
