This study aims to analyze the efficiency of public health expenditure across districts and municipalities in Lombok Island and Sumbawa Island, West Nusa Tenggara Province, during the 2015–2024 period. The study employed a quantitative explanatory approach using panel data covering 10 districts/cities. The analytical methods used were output-oriented Data Envelopment Analysis (DEA) with a Variable Return to Scale (VRS) assumption to measure technical efficiency and Tobit regression analysis to identify factors affecting health expenditure efficiency. The results indicate that most districts/cities achieved relatively high efficiency levels with average scores above 0.90. Bima City and North Lombok were identified as the most efficient regions, while Bima Regency showed the lowest relative efficiency. The ratio of health workers had a positive and significant effect on health expenditure efficiency, while population density showed different effects across regions. The findings imply that improving the distribution of health workers, optimizing budget governance, and enhancing the quality of public health services are essential to increase the efficiency of regional public expenditure.
