Abstract. Cardioembolic stroke is a subtype of ischemic stroke with a poor prognosis, particularly when accompanied by atrial fibrillation (AF). Atrial fibrillation is the most common cardiac arrhytmia worldwide and accounts for approximately 50% of cardioembolic stroke cases. Atrial fibrillation increases the risk of large embolus formation, which may lead to more severe impairment of gross motor function. This study aimed to analyze the differences in gross motor function outcomes between cardioembolic stroke patients with and without AF. This study employed a prospective cohort design involving 66 cardioembolic stroke patients at RSAU Salamun Bandung, consisting of 16 patients with AF and 50 patients without AF. Data were collected from primary sources using the modified Rankin Scale (mRS). Data analysis was performed using univariate analysis and the Chi-square test. The results showed that the presence of AF was significantly associated with poorer gross motor function outcomes in cardioembolic stroke patients compared with those without AF. This condition is associated with larger embolus size occluding the middle cerebral artery, leading to extensive cortical infarction and more severe neurological impairment.
