The Free Health Check-Up (CKG) Program is a government initiative aimed at promoting early disease detection and preventive health services. However, community participation in the program remains limited in Bolaang Mongondow Regency. This study aimed to describe community perceptions of the CKG Program at Maelang Public Health Center based on the six dimensions of the Health Belief Model (HBM). A descriptive quantitative study with a cross-sectional approach was conducted in Lolanan Village, Bolaang Mongondow Regency, from May to July 2026. The sample consisted of 54 community members who had participated in the CKG Program, selected using total sampling. Data were collected through interviews using a Likert-scale questionnaire and analyzed using univariate analysis, including frequency distributions and percentages. The findings showed that perceived benefits (70.4%), perceived severity (59.3%), perceived susceptibility (57.4%), and self-efficacy (51.9%) were predominantly categorized as positive. In contrast, perceived barriers were predominantly categorized as negative (53.7%), while cues to action were mainly categorized as uncertain (48.1%). These findings indicate that although participants generally recognized the benefits of the CKG Program, barriers and insufficient motivational cues remained concerns. Strengthening health promotion, providing accessible and understandable information, and reducing barriers to health service access are recommended to support greater community participation in the program.
