Abstract

Introduction. Patients with systemic lupus erythematosus (SLE) and rheumatoid arthritis (RA) are at increased risk of latent tuberculosis infection (LTBI) because of immune dysregulation and prolonged immunosuppressive therapy. However, data on the prevalence and factors associated with LTBI among patients with autoimmune diseases in Indonesia remain limited. This study aimed to identify factors associated with LTBI among adult outpatients with SLE and RA at Dr. Cipto Mangunkusumo National General Hospital (RSCM). Methods. This analytic cross-sectional study was conducted at the Allergy-Immunology and Rheumatology Outpatient Clinic of RSCM from November 2025 to July 2026. Data were obtained through structured interviews and medical record reviews. LTBI was assessed using the tuberculin skin test (TST) with 0.1 mL purified protein derivative administered intradermally, and induration was measured after 48–72 hours. Multivariable analysis was performed using binary logistic regression with autoimmune disease type included as a clinical covariate. Two sensitivity analyses were conducted using different disease activity dichotomization models. Results. A total of 193 patients were included, comprising 132 patients with SLE (68.4%) and 61 with RA (31.6%). The prevalence of LTBI was 10.9% (95% CI, 7.2–16.1%). In the bivariate analysis, three variables met the criterion for multivariable analysis (p Conclusions. The prevalence of LTBI among outpatients with SLE and RA at RSCM was 10.9%. Although no independent factor reached statistical significance, disease activity and a history of close contact with tuberculosis consistently demonstrated clinically relevant positive associations with LTBI, warranting further investigation in larger prospective studies.
