<![CDATA[Information systems play an important role in service management at the puskesmas. This study aims to design an outpatient medical record information system in Setono Public Health Center, Ponorogo, using the research and development (R & D) approach. The study population was all registration officers in the medical record section (3 people), while the research object was the flow of medical services. System design using PHP scripts, MySQ database and Note Pad ++ as Text Editor. It is known that the Setono Public Health Center, Ponorogo requires a medical record information system that is used to process outpatient registration data, provide information quickly and accurately, and does not require large storage capacity. Keywords: public health center, medical record, information system. ABSTRAK Sistem informasi berperan penting dalam manajemen pelayanan di puskesmas. Penelitian ini bertujuan merancang sistem informasi rekam medik pasien rawat jalan di Puskesmas Setono, Ponorogo, menggunakan pendekatan research and development (R&D). Populasi penelitian adalah seluruh petugas pendaftaran di bagian rekam medik (3 orang), sedangkan objek penelitian adalah alur pelayanan medis. Perancangan sistem menggunakan skrip PHP, MySQ database dan Note Pad ++ sebagai Text Editor. Diketahui bahwa Puskesmas Setono, Ponorogo membutuhkan sistem informasi rekam medis yang digunakan untuk mengolah data pendaftaran pasien rawat jalan, memberikan informasi dengan cepat dan tepat, serta tidak membutuhkan kapasitas penyimpanan yang besar.
