The basic purpose of bedrest for the patient supposed to the patient lies on the back in the bed for 24 hours to minimize all the patient’s organ system. The unsafety behavior, careless, lack of attention or motivation from the nurse to the patient can cause adverse event, such as pressure ulcer for the patient. Patient safety culture model for the nurse is better to decrease the number of accidents, the failed and also to optimally the service to the patient. The purpose of this study focused to examine the influence of patient safety culture model toward pressure ulcer risk prevention. This field research is mixed methods research model sequential explanatory. It used total sampling technique of 102 nurses in the five wards and quota sampling to select 3 informants from the patient culture control team in a Government Hospital. Multiple linear regression analyses were performed to identify qualitative data. Semi-structured guide in-depth interview used to collect the qualitative data. The summary of multiple linear regression analysis of all independent variables indicated that perceptions of patient safety were the most influential indicator to pressure ulcer risk prevention. The in-depth interview also indicated nurses’ perceptions of patient safety as an important factor that influences incident reporting, resources, and patient involvement as important factors that influence patient safety culture application on pressure ulcer risk prevention. Abstract

Chronic renal failure is a characteristic, progressive, and irreversible disorder that causes the kidney's ability to remove waste products and maintain fluid and electrolyte balance. The frequent impact is accumulation of metabolic waste, including uremia, excess fluid load, acidosis, hyperkalemia, and hyperuricemia. The aim of the study was to determine and examine more deeply the effect of hemodialysis therapy on changes in elderly uric acid levels in patients with chronic renal failure, a perception that has been focused on gout for several years as a sign of vascular problems in patients with kidney failure. This type of study was Quasy Experimental study with Pre and post test without control approach. The sample in the study were 15 respondents with purposive sampling technique. The results of the study was Wilcoxon test obtained p value of 0.003 (<0.005) there was a significant effect after hemodialysis, which was reduce in uric acid levels, in the elderly who got hemodialysis therapy in RSUD Kotabaru in 2018. The conclusion of this study is that there is an effect of hemodialysis therapy on changes in uric acid levels in men aged 45-59 years with chronic renal failure at Kotabaru Hospital. It was suggested to the hospital to continue to improve hemodialysis therapy services and to pay attention to uric acid levels to determine changes in uric acid levels so as to support the success of hemodialysis therapy.

Abstract

Organizational culture in which there is a relationship between humans can cause why a strategy can be implemented in a hospital. Leadership as one aspect of human relations determines a strategic plan that is technically imperfect if implemented properly, it will get better results compared to a perfect strategic plan but only on paper. This study uses ethnographic studies to explore the implementation of the strategic plan of nursing services in the perspective of the leadership of the head of the room based on organizational culture at one hospital in Kalimantan. Data retrieval uses indepth interviews, observations, and documentation. The focus of this research is divided into subfocus procedures, personnel, strategies or methods, staff structuring, direction, leadership style, values, assumptions, artifacts, and rituals. The results of this study indicate that the organizational culture is less conducive due to members who are just waiting for instructions from the leadership resulting in the leadership of the head of the room in administrator style. Whereas the implementation of the strategic plan depends on how the human resources in the hospital carry it out, especially how the leadership.
