Abstract

Dengue infection is a major global health threat, often manifesting as thrombocytopenia. While Carica papaya is traditionally used to enhance platelet counts, synthesized clinical evidence regarding its efficacy remains limited. This study evaluated the effectiveness of Carica papaya leaf extract (CPLE) on platelet count recovery in adult dengue patients. Following PRISMA 2020 guidelines, a systematic literature search was conducted across two databases (PubMed and Google Scholar) to identify randomized controlled trials (RCTs) published between 2012 and 2024. Risk of bias was assessed using Cochrane RoB 2, and data were analyzed in R Studio using a random-effects model. Four RCTs involving 279 adult participants were included. The meta-analysis demonstrated that CPLE significantly increased platelet counts compared to controls, with a pooled mean difference of 14.34 × 10³/µL (95% CI: 5.90 to 22.77; p = 0.0124 for overall effect). Statistical heterogeneity was low-to-moderate (I2 = 27.0%), indicating a generally consistent therapeutic direction. In conclusion, CPLE serves as a viable adjunctive therapy for accelerating platelet recovery in adult dengue patients. However, given the sensitivity of the pooled estimates to specific highly weighted trials, future large-scale, rigorously blinded RCTs are needed to solidify these findings and confidently support their integration into clinical management protocols.Keywords: Carica papaya; dengue fever; thrombocytopenia; platelet recovery; meta-analysis; adjucntive therapy

Abstract

Stature is a fundamental anthropometric indicator used in nutritional assessment and pharmacokinetic calculations; however, direct measurement is often challenging in intensive care settings or among individuals with skeletal deformities. Estimation based on ulna length offers a practical alternative. Although numerous formulas have been proposed, none have been specifically developed or tested for the Madurese ethnic group. This study aimed to estimate stature from ulna length among early‑adult Madurese individuals. An analytical observational design with a cross‑sectional approach was employed. A total of 34 students aged 18–29 years from the Faculties of Medicine, Nursing, and Pharmacy at Universitas Muhammadiyah Malang were selected using purposive sampling. Stature was measured using a microtoise, whereas ulna length was assessed using a sliding caliper. Data were analyzed using univariate statistics, independent t‑tests, and linear regression. The findings demonstrated significant differences between actual stature and estimated values generated by existing formulas, with the Pureepatpong equation yielding estimates closest to actual measurements compared with the Thummar formula. Ulna length showed a significant association with stature. In conclusion, ulna‑based measurement is a valid method for estimating stature in early‑adult Madurese individuals. The Pureepatpong formula exhibited superior accuracy and is therefore recommended for application in this population.Keywords: stature estimation; ulna length; early adulthood; Madurese ethnicity; anthropometry

Abstract

Patient safety culture encompasses shared values, beliefs, and norms regarding the importance of safety in healthcare settings. Ineffective communication during handoffs and information exchange can lead to medical errors, complications, and even an increased risk of mortality. This study aimed to analyze the effectiveness of I-PASS training on the quality of handoffs and information exchange among nurses at Hospital X, West Jakarta. This study employed a quantitative approach with a pre-experimental design and was conducted from February to March 2026. The sample consisted of 56 nurses who met the inclusion criteria: working in inpatient wards, ICU, and emergency departments; willing to participate; levels PK 2 and 3; and actively involved in handoffs and information exchange processes. The data collection tool was a questionnaire. The results showed that the majority were aged 26–35 years (66.1%), female (75%), PK level 2 (85.7%), and D3 Nursing (60.7%). The results of the I-PASS training for nurses after training showed that the majority (80.4%) had good results, and the quality of handoffs and information exchange after training was good (57.1%). There was a relationship between age (p-value = 0.032) on the quality of handoffs and information exchange. There was a significant difference in the quality of handoffs and information exchange before and after the intervention (p-value = 0.000). The results of the multivariate analysis showed that age (p-value = 0.019) and I-PASS training (p-value = 0.029) significantly influenced the quality of handoffs and information exchange (p < 0.05). The coefficient of deter mination (R²) was 0.112, indicating that these two factors contributed 11.2% to explaining the quality of handoffs and information exchange. Hospitals are recommended to establish I-PASS training as a structured and sustaiable nursing development program by integrating I-PASS-based learning mechanisms into standard operating procedures (SPOs), ensuring consistent implementation among all nursesKeywords: I-PASS training; quality of handoffs and information exchange; nurses

Abstract

ABSTRACTBackground: Hospitals operate in an increasingly competitive environment that demands continuous improvement of service quality, patient safety, efficiency, and organizational performance. Strategic management, comprising strategy formulation, implementation, and evaluation, is a key determinant of hospital sustainability. However, the implementation stage remains the most challenging, as many well-formulated strategies fail to be translated into measurable performance. Purpose: To review scientific evidence regarding the implementation of hospital strategic management and its role in improving service quality and organizational performance. Method: A literature review using a systematic approach based on PRISMA guidelines. Literature searches were conducted through PubMed, ScienceDirect, Google Scholar, and Garuda databases for articles published between 2020 and 2026. Eight articles met the inclusion criteria and were included in the analysis. Results: The success of hospital strategic management implementation was determined by the interaction of strategic planning and environmental analysis (SWOT and PESTEL), performance measurement systems (the Balanced Scorecard and key performance indicators), strategic leadership and organizational culture, adequate organizational structure and resource allocation, and the use of hospital management information systems. These factors collectively contributed to improved service quality, patient safety, operational efficiency, and cost control. Conclusion: Hospital strategic management implementation is a multidimensional process influenced by the alignment of leadership, planning, performance measurement, organizational systems, and information technology. Hospitals should strengthen strategic leadership, structured strategic planning with measurable indicators, and supportive organizational systems to improve service quality and organizational performance.Keywords: hospital; lmplementation; organizational performance; service quality; strategic management

Abstract

Diabetes mellitus is a major non‑communicable disease that poses a serious global health threat across socioeconomic and national boundaries. Individuals with diabetes are at high risk of developing chronic complications, including diabetic neuropathy, which may progress to diabetic foot ulcers. Approximately 90% of patients with diabetic foot ulcers face the risk of lower‑extremity amputation. Inadequate foot‑care behavior among people with diabetes further increases the likelihood of severe complications, including recurrent foot ulcers and amputation. This study aimed to analyze the relationship between foot‑care behavior among patients with diabetes mellitus and the occurrence of recurrent diabetic foot ulcers. A quantitative cross‑sectional design was employed using accidental sampling. The sample consisted of 51 patients with recurrent diabetic foot ulcers. Descriptive analysis and Spearman’s rho correlation test were used to examine the association between foot‑care behavior and ulcer recurrence. The findings demonstrated a significant relationship between foot‑care behavior and recurrent diabetic foot ulcers, with a p‑value of (0.022) and a correlation coefficient of (–0.320). Most patients with recurrent ulcers were male (60.8%), nearly half were aged ≥65 years, and 47.1% had lived with diabetes for 1–5 years. A majority experienced two episodes of recurrent ulcers (78.4%) and exhibited poor foot‑care behavior (72.5%). Poor foot‑care behavior is significantly associated with recurrent diabetic foot ulcers. The demographic profile of affected patients indicates predominance among older males with a moderate duration of diabetes. Strengthening foot‑care practices is essential to reduce recurrence and prevent severe complications.Keywords: diabetes mellitus; diabetic foot ulcer; complications; foot care

Abstract

Unintended pregnancy and inappropriate contraceptive use remain significant public health challenges among women of reproductive age. Limited access to accurate contraceptive information and inadequate counseling may hinder informed decision‑making. With the rapid advancement of digital health technologies, e‑module–based contraceptive education has emerged as a promising strategy to strengthen women’s knowledge and support informed contraceptive choices. This systematic review aimed to examine the effectiveness of e‑module–based contraceptive education in supporting informed contraceptive decision‑making among women of reproductive age. A systematic review was conducted following PRISMA guidelines. Searches were performed in PubMed, ScienceDirect, ProQuest, and Google Scholar. Eligible studies included original quantitative or mixed‑method research involving women of reproductive age, using e‑module or digital contraceptive education interventions, and reporting outcomes related to decision‑making, knowledge, attitudes, or contraceptive choice. Data were synthesized narratively. Across the included studies, e‑module–based contraceptive education consistently demonstrated positive effects on multiple dimensions of informed decision‑making. Most interventions significantly improved participants’ contraceptive knowledge, including understanding of method effectiveness, side effects, and correct use. Several studies reported enhanced attitudes toward contraceptive planning, greater confidence in evaluating available options, and increased readiness to engage in shared decision‑making with healthcare providers. Digital modules also contributed to reductions in decisional conflict, particularly by clarifying personal values and aligning contraceptive choices with individual preferences and reproductive goals. In some studies, women expressed higher satisfaction with digital learning formats due to their flexibility, privacy, and ease of access. Overall, digital contraceptive education showed comparable or superior effectiveness to conventional counseling, especially in improving comprehension and supporting autonomous decision‑making. In conclusion, E‑module–based contraceptive education effectively strengthens informed contraceptive decision‑making among women of reproductive age. Integrating digital modules into nursing practice and family planning services may enhance patient‑centered care. Further high‑quality studies are needed to assess long‑term outcomes.Keywords: contraception; decision making; digital health; reproductive health; women

Abstract

Asphyxia is a condition in which a newborn fails to breathe spontaneously and regularly. According to the Ministry of Health in 2017, neonatal mortality caused by asphyxia reached 27.8%. One of the causes of neonatal asphyxia is anemia gravidarum. This process begins with decreased hemoglobin (Hb) levels, impaired oxygen transport, and eventually leads to fetal hypoxia. This literature review was conducted in March 2026. Literature searches were performed using Google Scholar, PubMed, ScienceDirect, and Garuda, with a publication range from 2016 to 2026. Article selection was carried out following PRISMA guidelines. Five studies showed a significant association between neonatal asphyxia and anemia gravidarum, while one study showed no significant association. Therefore, healthcare professionals are expected to provide education regarding nutritional intake for pregnant women as a preventive measure against anemia and to emphasize the importance of antenatal care.Keywords: anemia gravidarum; asphyxia; indonesia

Abstract

Malnutrition is a condition in which an individual has nutrient intake below adequate levels, leading to impaired growth and development. Efforts to prevent malnutrition in toddlers can be implemented through supplementary feeding based on local food ingredients, including purple sweet potato. This study aimed to determine the effect of purple sweet potato–based supplementary feeding on the nutritional status of underweight toddlers in the Gunungsitoli City area. A quasi-experimental design with pretest–posttest and a control group was employed. The study population consisted of all malnourished toddlers, and a total sample of 20 children was selected using purposive sampling, comprising 10 in the intervention group and 10 in the control group. Before receiving purple sweet potato–based supplementary feeding, most toddlers in the intervention group were categorized as underweight (70%) and of normal height (70%), whereas in the control group, the majority were underweight (60%) and stunted (50%). After the intervention, most toddlers in the intervention group achieved normal weight (90%) and normal height (70%), with a significance value of 0.002 (p<0.05). In contrast, the control group remained predominantly underweight (80%) and stunted (50%), with a significance value of 0.08 (p>0.05). These findings indicate that purple sweet potato–based supplementary feeding positively affects the nutritional status of underweight toddlers, as reflected in improvements in both weight and height.Keywords: purple sweet potato; supplementary feeding; toddler undernutrition status

Abstract

The Elderly Integrated Health Service Post functions as an early detection program for health problems among older adults. Attendance of elderly individuals at this service is influenced by several factors, one of which is the role of health cadres. Cadres act as community mobilizers, provide health education, and monitor elderly health conditions. This study aims to determine the relationship between the role of health cadres and the attendance of elderly individuals in the Elderly Integrated Health Service Post at the working area of Raya Health Center, Teladan Subdistrict, Pematangsiantar City. A cross-sectional design was employed. The population consisted of 375 elderly individuals aged 60–80 years. A sample of 94 respondents was selected using systematic random sampling. Data were collected through questionnaires and analyzed using Spearman’s Rho test. The hypothesis test yielded a p-value of 0.000, indicating a significant relationship between the role of health cadres and the activeness of elderly individuals in attending the Elderly Integrated Health Service Post in Teladan Subdistrict, Pematangsiantar City. In conclusion, the findings demonstrate that the role of health cadres significantly influences elderly participation in the Elderly Integrated Health Service Post.
