Abstract

Effect of infusion is the occurrence of phlebitis, which is characterized by swelling, redness, and pain along the veins. Today, the administration of intravenous therapy (antibiotic administration) is the primary treatment modality. However, it is suggested that the occurrence of phlebitis due to infused antibiotics is more and more increasing. The aimed to identify correlation between the administration infusion of betalactam antibiotics and the occurrence phlebitis at the ward, health care board Jombang. This study used cross-sectional design. The population was all patients receiving betalactam antibiotics infusion treated at ward, health care board Jombang. Samples were recruited using consecutive sampling, with sample size of 40 respondents. The mechanism of data collection, each respondent was observed for three days, on the first day of infusion was insertion and over the next two days were observed. All collected data were analyzed with Chi square correlation test with significance level of alpha <= 0.05. The administration of betalactam antibiotic infusion has no correlation with phlebitis. The result of statistic test showed that level of alpha was 0.000, so that correlation was not confirmed. The reason is not due to the patient's condition can phlebitis neither age nor illness is not severe, procedures using aseptic technique and the insertion with immobilization technique. Although the correlation is not found, the administration of betalactam antibiotic infusion should not overlook all factors that may lead to phlebitis. To obtain a more valid result, further studies are needed with larger sample and using random sampling to have a more generalized result.

Abstract

Diabetes mellitus is a disorder in the metabolism of carbohydrates, lipids and proteins, which is characterized by hyperglycemia. Its prevalence in the world is increasing, especially type 2 diabetes, causes an increased incidence of morbidity and mortality throughout the world due to complications of hyperglycemia. Hyperglycemia stimulates the release of superoxide (O2-) at the level of mitochondria which is the trigger early onset of oxidative stress in patients with diabetes mellitus type 2. Therefore one of the therapeutic modalities that can be developed in type 2 diabetes mellitus is a substance as potent antioxidant. Geraniin of rambutan peel extract is known to have the ability as a potent antioxidant and radical scavenging, so it is possible to be a prime candidate antidiabetes in the ype 2 diabetes melitus. We searched Cochrane Library data bases, Google Scholar, Medline and PubMed, from 2006 to 2016, to assess the effect of rambutan peel extract for type 2 diabetes melitus. We obtained 17 articles in accordance with the inclusion criteria. In the peel of rambutan (Nephelium lappaceum), geraniin is the main phenolic compounds that have the potential ability as an antioxidant and antihyperglycemic through inhibition of alfa-glucosidase and alfa-amylase, preventing the polyol (aldol reductase inhibition) and prevents the formation of advanced glicosilation products (AGEs). There is a very strong correlation between the ability as an antioxidant with the ability to inhibit alfa-glucosidase and alfa-amylase, so geraniin of rambutan peel extract (Nephelium lappaceum) expressed as the ideal candidate for the management of hyperglycemia in individuals with diabetes.

Abstract

Surgery is one of procedur to cure of the disease with method cut and slice of the body. Almost surgery use general aenesthesia before the procedure began. After general anesthesia procedure, the acumulation of mucus secretion in respiratory tract have occured. The accamulatin of mucus secretion it it cause by intubation endotracheal tube procedure or effect of anesthetic agens itself. The mucus accumulation can lead improving of bacterial colonization, especially Staphylococcus aureus which was a normal flora and might become pathogenic when amount more than usual. Staphylococcus aureus were increasingly more and colonized in respiratory tract, especially in pharix, would getted into the lungs and caused pneumonia nosocomial. One of nurse efforts to protect the patient from pneumonia nosocomial is minimized the colonization of Staphylococcus aureus by implementing effective cough and deep breath. The procedure can do after surgery in post operative phase to help patient removed the excessive mucus secretions, it's mean amount of bacteria that contained in the mucus can minimized. This research is quasy experiment study. The design of the study is posttest only experimental design with non-equivalent control group design. The samples in this study were 20 respondents postoperative patients with general aenesthesia divided into 2 groups (control group and the experimental group). The intervention (effective cough and deep breath procedure) is given to the experimental group. Data were analyzed by independent t-test. The results showed that t value = 4.405 (p value = 0.000 < 0.05), that mean the procedure of effective coughing and deep breathing have effected to reduce the colonization of Staphylococcus aureus in secret of patients post surgery with general aenesthesia in Soebandi Jember Hospital and could have minimized risk of pneumonia nosocomial.
