Abstract

In increasing social health services there are a lot of effort that have been done by the government such as providing facilities of health services such as pharmacy. As an effort to make phamacist can give a profesional pharmacy practice based on the decision of Health Minister Republic of Indonesia No.27/Menkes/SK/IX/2004 about the Standard of Pharmacy Practice in pharmacy community the goal is to find out the figure of Standards of Pharmacy Practice implementation in pharmac community at Yogyakarta City. The results of the research generally showed that the standards of pharmacy practice in Yogyakarta has not been implemented well. Total score for the standards pharmacy practice based on pharmacists respondent 21% (good), 79% (enough); employees respondent 21% (good), 75% (enough), 4% (bad) and patients respondent 38% (good), 42% (enough), 20% (bad). The inhibiting factors towards the implementation standards of pharmacy practice such as education programme, seminar or another thing that rarely held that make the opportunity for self development very limit, the socialization are rare and control function from institution that have authority are weak so monitoring and evaluation should do periodicaly. Keywords : Standards of Pharmacy Practice, Pharmacy, Pharmacist, Employees, Patients

Abstract

Nowadays, the rational drug usage is the main orientation of pharmaceutical service. One of parameter that can be used to measure the rationality of drug usage is by using valuation toward drug usage description, so this research’s done to see the drug usage description toward outpatient in Panti Nugroho Hospital Sleman. This research is descriptive non experimental. The data was taken prospectively with systematic random sampling method. The data taken from the prescription that given for outpatient, patients direct observation that is started from consultation with the doctor in polyclinic until the patients got the drugs, and key drugs (drug of choice) in RSPN Sleman that is analyzed by using WHO drugs usage indicator 1993. The result of this research showed that the average number of drug per encounter is 2,44 R/, the percentage of drugs prescribed by generic name is 27,92%, the percentage of encounters with an antibiotic prescribed is 15,27%, the percentage of encounters with an injection prescribed is 0,33%, and the percentage of drugs prescribed from hospital formulary is 85,05%. However, based on the patients service indicator research result, the result is that the average consultation time is 8 minutes 24 seconds, the average dispensing time is 9 minutes and 56 seconds, the percentage of drugs actually dispensed is 95,89%, the percentage of drugs adequately labeled is 99,66%, and the percentage of patient’s knowledge of correct dosage is 90,00%. From the research result using health facility indicator, it can be seen that in RSPN, there is hospital’s formularium (1), and the percentage of key drugs availability is 100,00%. Keywords : drugs usage description, WHO indicator 1993, Panti Nugroho Hospital Sleman.

Abstract

Identification of problems caused by the use of drugs, followed by corrective actions, treatment, and prevention are essential to ensure safety. Implementation of medication error reporting systems as well as providing recommendations by pharmacists are strategies to improve medication safety practices in the health care system. A quasi experimental study was conducted at two surgical wards in Yogyakarta Bethesda Hospital. Clinical pharmacist did the identification, corrective actions, as well as reporting of the medication errors (MEs) incidents in both wards. A total of 283 drug problems at Ward I and 39 drug problems at Ward II were identified by clinical pharmacists. All of ME incidents identified at Ward II and 13,5 of those at Ward I were reported through pharmacy’s reporting system. Nineteen DRPs at Ward I and 13 DRPs at Ward II were resolved through communication and delivering recommendations to physicians. A total of 26,3% of written recommendations were accepted compared with 84,6% of verbal recommendations (p = 0,02). All of the DRPs resolved by verbal communication, whereas only 33,3% of those resolved by written communication. Keywords: medication safety practices, medication errors, Drug Related Problems, reporting

Abstract

GMC is an organization that provided health assurance for academicians in UGM. As a health assurance, GMC should increase their quality. Quality Function Deployment (QFD) is a part of Total Quality Management (TQM) tool that can use for measure TQM by House of Quality (HOQ) design. The aim of this study is to analyze the implementation of HOQ. This study located in GMC Health Center of Yogyakarta. The research was conducted using 2 samples. The samples consist of 2, 100 external customers and 10 internal customers, amounting to 10. This research followed by interviews with top managers to build house of Quality. And then build house of quality, finally analyze the atribute that must be repaired by management of GMC. The results showed that there were three aspects that should be prioritized by GMC. The first aspect is pharmaceutical care that included 4 attributes. The highest gap of this aspect completeness of drug supplies 6.21 and the availability of a pharmacist that was consumer value answered by the management GMC with the high level of difficulty. From the relative weight score 106.61 percent can be showed that the availability of pharmacist has the highest value on the customer excpectations for this aspects.The other aspects that must be improved are the emphaty and responsiveness and time. Emphaty dimension has the the highest value of technical descriptors 538.397 that can be done by the GMC for complete customer expectations in all aspects. Abstract

Jamkesmas is social assistance health services program to poor society. This program is aimed to make cross subsidy health services for all poor society. This research is aimed at identifying the characteristics of patients, verification of Jamkesmas, average discrepancy of therapy cost, and average discrepancy of medicine cost. This research uses non‐experimental descriptive design, which is using proportional random sampling method that collected retrospectively. The result shows that appendicitis patient that need inclution criteria is 37 cases, of acutegastroenteritis (GEA) is 33 cases, of hypertension is 31 cases, of urinary tract infections is 32 cases , and of acute respiratory infection is 38 cases. The total amount of claims of Jamkesmas inpatient in 2008 is Rp 6.152.995.376, of the payable claims is Rp 6.053.265.909, and of the non‐payable claims is Rp 119.729.467. The average discrepancy cost of Jamkesmas verification and direct medical cost on receipt of appendicitis therapy is 56.028 ± 299.686, of acutegastroenteritis (GEA) is Rp ‐ 12.266 ± 204.275,, of hypertension is Rp  118.697 ± 329.572, of urinary tract infections is Rp 155.582 ± 696.428, and of acute respiratory infection is Rp 86.417 ± 156.541. The average discrepancy of medicine cost of Jamkesmas verification and medicine cost on receipt of appendicitis therapy is Rp ‐424.818 ± 444.459, of acutegastroenteritis (GEA) is Rp ‐ 31.110 ± 96.157, of hypertension is Rp ‐67.001 ± 114.541, of urinary tract infections is Rp 41.635 ± 353.945, and of acute respiratory infection is Rp 32.955 ± 132.815. Key words : Cost Analysis, Inpatient, Jamkesmas, RSUD Panembahan Senopati Bantul

Abstract

Type 2 Diabetes Mellitus is a chronic disease which can’t be healed totally. Therefore type 2 DM will affect the Health Related Quality of Life (HRQOL). The purpose of this study was to determinate the QOL of type 2 DM patients based on patient’s characteristics, drug groups, and therapy pattern (single versus combination therapy). This study was conducted in RSUP Dr. Sardjito Yogyakarta. This study is an analytical research with cross sectional design. Data collection was done by interview and review of patient’s medical records. Research’s subjects are ambulatory patient with type 2 DM who are threated with oral antidiabetic(s) and willing to fill the questionnaire. Total 227 total patients were included in this study. Quality of life was measured by Diabetes Quality of Life Clinical Trial Questionnaire (DQLCTQ). The analysis showed that gender, age, duration of DM, educational level, marital status and occupation affect the QOL. Patient taking metformin had 67.9 score, 67.7 for patient with acarbose therapy and patient taking sulfonylurea 67.6 score. Those differences were not significant (ANOVA test, p = 0.298). Patients taking single antidiabetic agent had 70.6, whereas those taking combination therapy had 67.6 score. Independent sample t‐test showed that those score have significant difference (p = 0.002). Linear regression analysis with enter method showed that QOL’s difference between single and combination therapy groups did affected by patient’s characteristics (p=0.049). Keywords : type 2 DM, ADO therapy, quality of life, DQLCTQ

Abstract

Local Drug anf Food Agency in Manado (Balai Besar POM) is a Strategic Public Service Unit of National Drug and Food Agency (Badan POM) which expected capable to work effectively and efficiently to detects, prevents, and controls products, and also to protects consumer’s safety, security and health. Therefore, its necessary to know the current performance of quality control sector staff in Balai Besar POM Manado in order to increasing it. To detect staff performance of quality control sector in Balai Besar POM Manado which suitable to its importance level, and to detect the rank of performance variable which has biggest value on performance forming at Balai Besar POM Manado, its needs an approach using Importance‐Performance analysis of cartesius diagram to find out which organizational elements that most need improvement. This research is carried out on 34 quality control sector staff in Balai Besar POM Manado using descriptive research design and data collecting is obtained from primary and secondary data. Primary data is questionnares which distributed to quality control sector staff in Balai Besar POM Manado. Secondary data is comes from annual report of Balai Besar POM Manado. The results of this research based on Importance‐Performance analysis of cartesius diagram shows that ability to complete job and productivity variables has to get top priority from Balai Besar POM Manado to increase both variables, while ability to make decision and cooperation relationship with other people variables are also needs to increase, but these variables aren’t top priority. The order of performance variable rank based on its importance level is knowledge and understanding job, working attitude, productivity, ability to complete job, cooperation relationship with other people, and ability to make decision variables. Key Words : Balai Besar POM Manado, Performance, Importance‐Performance Analysis

Abstract

Malaria is still become one of a serious health problem in Indonesia especially in Kalimantan and other island except Java and Bali. South Kalimantan is one of the endemic province of malaria. Malaria problem is not only about the healthcare, but also concern about the economic problem. economic problem can occured because of cost of healthcare, especially when patient have to hospitalized, and the lost of productivity during the illness. The purpose of this research is to know any component and direct medical cost that patient have to pay when patient had a health care during opname. This research is also to observe therapy guidelines in RSUD Ulin Banjarmasin South Kalimantan. This research used a descriptive non experimental method. Sample wass collected retrospectively from medical record of patients who hospitalised in 2006‐2009 and finance departement to know how much the cost that patient have to spend. Cost of therapy in malaria patient without complication for VIP, Utama, I, II, and III was Rp 6.010.736,00; Rp 3.345.877,00; Rp 1.531.057,00; Rp 2.384.985,00; dan Rp 1.919.354,00 and in malaria patient with complication for Utama and III was Rp 3.993.575,00 dan Rp 3.160.617,00. Average cost of therapy in malaria patient with complication was higher than malaria patient without complication. The component of therapy cost was drug therapy cost and nondrug therapy cost, including hospitalising cost, medical action cost, supporting cost, and administration cost. The evaluation of malaria therapy in complication and non complication patient shows that not all of antimalaria and antimalaria dosage which were used inRSUD Ulin Banjarmasin was according to DEPKES RI guidelines in 2003. Keywords: malaria, direct medical cost, therapy evaluation

Abstract

Drug procurement system in H Boejasin Hospital, Tanah Laut regency showed over stock that have been existed since 2006. This problem may cause poor availability and inefficiency of drug procurement. The aim of this research is to evaluate drug procurement system concerning drug availability and efficiency at Hadji Boejasin Hospital. This was a qualitative descriptive study, which support used secondary data and observation of drug storage warehouse as well as depot of hospital pharmacy. Subject of the study consisted of two groups: procurement group and user group. Result of the study showed that the process of drug procurement using local expenditure and revenue fund of Tanah Laut regency at Hadji Boejasin Hospital from 2006 ‐ 2008 had been based on the President Decree No. 80/2003 through selective tender and negotiated purchase. Result of the evaluation showed that: 1) period of drug procurement was 1‐3 month; 2) frequency of drug procurement was 1 2 time/year; 3) procurement procedure was based on some standardized phases. The procurement system caused highly damaged/expired drugs (0,48‐1,90%), highly non prescribed drugs (5,98%‐ 11.11%), drugs were out of stock (1‐10 month), and low annual turn over ratio (TOR). TOR value in 2006 was 1.28; in 2007 was 0.40, in 2008 was 0.36, which meant that drug circulation in warehouse was relatively low. Based on these study, it can be concluded that level of drug availability and efficiency at Hadji Boejasin Hospital using procurement system of local expenditure and revenue fund of Tanah laut regency in 2006–2008 was not good enough. Keywords : drug procurement system, drug availability

Abstract

Today the competition in the industry of herbal medicine / jamu tends to increase. PT Air Mancur as a manufacturer of herbal medicine should be able to compete by offering quality herbal medicine and can satisfy consumer desire. In the face of such competition, PT Air Mancur in addition must pay attention to product quality also needs to have promotion mix strategy (promotional mix) is implemented correctly. This study chose Madurasa as a subject studied because Madurasa is a product that produces great profitability for PT Air Mancur. The purpose of this study to determine the effect of product quality and promotion mix of Madurasa purchasing decisions by consumers in Surakarta. This study uses a questionnaire to determine the effect of product quality and promotion mix of the purchase decision. Large sample used in this study based on the formula Aaker and Day with 95 % confidence level is 400 respondents. Engineering samples of the determination of non probability design capture through purposive sampling procedure. Data were analyzed with regression method to determine the effect of product quality and promotion mix of Madurasa purchasing decisions by consumer in Surakarta. The result showed that the variable product quality, advertising, and personal selling them influence on purchase decisions by consumers Madurasa in Surakarta. Meanwhile, sales promotion and public relation not impact on purchasing decisions. Ads have the most significant influence on Madurasa purchase decisions by consumers in Surakarta. Keywords : quality of product, promotion mix, purchasing decisions, Madurasa.
