Abstract

Pulmonary hypertension is a common complication of congenital heart disease due to systemic – pulmonary circulation shunt which if left uncorrected leads to increased pulmonary artery pressure, vascular remodeling and further increase of pulmonary vascular resistance. Percutaneous closure of the defect interrupts this shunt thus reducing right heart and pulmonary circulation load and pulmonary artery pressure. In this paper we present two cases of percutaneous secundum atrial septal defect closure complicated by pulmonary hypertension along with echocardiographic evaluation of cardiopulmonary hemodynamic changes before and shortly after device closure. Forty years old and thirty three years old females presented to our clinics with classical symptoms of atrial septal defects, assessment revealed TVG of 37 mmHg and 30 mmHg,shortly after the procedure patient was re-evaluated and revealed TVG of 39 mmHg and 23 mmHg respectively. From these cases we conclude that changes in pulmonary artery pressure is not constantly found after device closure. However both patients display improvements in functional capabilities.

Abstract

Myocardial infarction in young female is rare condition, but carries significant morbidity and mortality. The recognition management for MI in young female is challenging due to its unspecific clinical presentation and lack of guidance in the current guidelines. We reported a 29-years old woman presenting with infarction type chest pain, showing ST- elevation in the inferior leads with total AVblock as complication. The risk factors in this patient were smoking, family history, cardiomyopathy, and preeclampsia history. The cause for myocardial infarction in this patient was atherosclerotic plaque rupture, but we did not manage to performed coronary intervention due to her low ventricular function and unavailability data for myocardial viability at the time of hospitalization.

Abstract

Background: Cardiovascular disease, especially hypertension, is the main cause of death in both developed and developing countries. Hypertensioncan lead to atherosclerosis, which is the most common cause of ischemic and coronary heart disease. Some countries have used Computed Tomography Coronary Angiography (CTCA), which is a noninvasive test, to observe the anatomy of coronary artery. Nevertheless, invasive angiography isstill the gold standardfor coronary artery visualization. Atherosclerotic plaques cancause left main coronary artery stenosis. Purpose: The purpose of this research is to analyze the correlation between blood pressure and severity of the atherosclerosis in the left main coronary artery. With CTCA examination, the presence of plaque, severity of stenosis and type of plaques were assessed. Methods: The study design was analytic observational with cross sectional design. Data was drawn from medical records. The sample included a total of 29 patients which were suspected of having coronary heart disease and examined with CTCA who meet inclusion and exclusion criteria in Dr. Kariadi Hospital, Semarang, Indonesia. Result: The correlation between systolic blood pressure and left main coronary artery stenosis was obtained by Spearman’s rho r = 0.0429, p-value of with of 0.020 (2-tailed). The correlation between diastolic blood pressure and left main coronary artery stenosis was obtained by Spearman’s rho r = 0.354 with p-value of 0.060(2 -tailed). Some types of plaques were found in left main coronary artery, and systolic blood pressure are significantly different analysed usingAnova (analysis of variance) which probability was significant with p value 0.041. Conclusion: There was a significant correlation between systolic blood pressure and left main coronary artery stenosis. Therewas no correlation between diastolic blood pressure and left main coronary artery stenosis. Furthermore, there was a significant difference between systolic blood pressure and the type of plaques in the left main coronary artery.

Abstract

Background: An isolated persistent left SVC with concomittant agenesis of right SVC in adult patients is a very rare abnormality. Physician should consider it particularly in patients, in which venous acces will be performed. Our rare case deals with the importance of detailed echocardiographic examination with screening of coronary sinus dilatation before the electrophysiology study.Case: A 65-year-old woman came to outpatient clinic for a chief complaint of palpitations. Her ECG showed paroxysmal SVT with WPW syndrome. She underwent echocardiography examination before electrophysiology study and it was found that she had a dilated coronary sinus. Therefore we performed cardiac CT. It was found that she had a persistent left superior vena cava (SVC) and an absence of a right SVC with no other congenital anomaly.Conclusion: A comprehensive echocardiography examination to look for a dilation of coronary sinus is a first suggestion to screen this anomaly, eventually followed by echocardiography with agitated saline injection and/or computed tomography can help physician to anticipate the anomaly before the invasive procedure involving the thoracic vein.Keywords: echocardiography, cardiac CT angiography, vascular malformation, superiorvena cava
