Abstract

Background: WHO in 2020, stated that maternal deaths during pregnancy, childbirth and postpartum were 295,000 worldwide. Africa, Sub-Saharan and South Asia account for about 86%. Meanwhile, the maternal mortality rate in Indonesia in 2020 is 230 per 100,000 live births. One of the causes of maternal death is bleeding and infection. Suturing perineal tears with anesthesia is one of the maternal care programs, which aims to reduce the pain experienced by mothers during the suturing process of birth canal wounds. Giving anesthesia can also cause damage to the immune system which results in a decrease in body resistance so that there will be a lengthening of wound healing 2-3 days than without anesthesia. Research Objective: Knowing the administration of local anesthesia on the healing duration of perineal wounds in puerperal mothers. Research Method: Quasi-experiment with post-test group design. The samples in this study were all postpartum mothers who experienced perineal injuries at PMB Nunung in November-December 2023 as many as 40 people. The sampling technique uses the total sampling technique. Abstract

The canteen is one of the strategic places in the spread of diseases carried by vectors and disease-carrying animals such as cockroaches. The spread of disease in the canteen is also influenced by environmental sanitation hygiene. A dirty and humid environment is a good breeding ground for cockroaches. The government has established a policy to reduce the vector population to minimize the risk of impact due to the disease it causes. Research to determine the relationship between sanitary hygiene and physical environmental conditions with cockroach density in the canteen of Sriwijaya University Indralaya campus was conducted. The study was an observational analytic study using a cross sectional study design. A total of 50 canteens on campus were used as samples and were total sampling. Data on sanitary hygiene standards were collected from observational sheets and measurements of physical environmental conditions were made using a hygro thermometer, while cockroach density was measured based on the sticky trap method. Data were analyzed univariately and bivariately using the Chi-Square test. The results showed that of the 50 canteens sampled, 6 canteens were classified as canteens with sanitary hygiene that did not meet the Permenkes requirements. This is indicated by the high cockroach density with a p-value of 0.000 <0.05. This means that sanitary hygiene significantly affects cockroach density. It was concluded that the most cockroaches were found in canteens with poor lighting conditions, low room temperature, and high room humidity. Sequential p-values of 0.000; 0.002; 0.002 for the 3 observed variables indicate a poor environment affects cockroach breeding, while a p-value <0.05 is interpreted as a significant relationship between physical environmental conditions and cockroach density.

Abstract

The immunization program is aimed to reach herd immunity, is requires high immunization coverage. The coverage of Universal Child Immunization (UCI) at the Village level in Trenggalek District for the period 2019–2021 still had not reached the target (100%), 91.08% in 2019, 92.36% in 2020, and 89.17% in 2021. This study aimed to identify the causes of the problem and suggest alternative solutions. This observational descriptive study was conducted at the Trenggalek District Health Office in January 2023. Informants were 17 people (4 officers of the health office who were responsible for the immunization program and 13 immunization coordinators from the Public Health Center/PHC which in 2019–2021 had not reached the UCI coverage at Village level). Identification of root causes was using a fishbone diagram and determining the priority of root causes to be intervened with using the Capability, Accessibility, Readiness, and Leverage (CARL) method which was done through brainstorming with all respondents. There were identified 11 root causes of not achieving the UCI coverage at the Village level. The dominant factor affecting the root problem was the officers (81.81%). The root problem that received the highest score with the CARL method was the immunization officers did not give communication, information, and education to parents optimally. This happened because the officers only focused on the number of targets to reach. In each PHC, there needs to be a team that gives communication, information, and education to assist immunization officers in providing education and information about immunization. The Immunization Officer in the District Health Office needs to monitor and evaluate the immunization services and give feedback to the PHC periodically.
