Jurnal Plastik Rekonstruksi is an open access, peer reviewed, international journal focusing on global plastic and reconstructive surgery. JPR publishes on all areas of plastic and reconstructive surgery, including basic science/experimental studies pertinent to the field and also clinical articles on such topics as: breast reconstruction, head and neck surgery, pediatric and craniofacial surgery, hand and microsurgery, wound healing, and cosmetic and aesthetic surgery. Abstract

Backgrounds: Fibrous dysplasia is an uncommon, benign disorder characterized by a tumor-like proliferation of fibro-osseous tissue. It can present as an autosomal dominant disorder affecting the mandible and maxilla bones in children in their teenage years. The patient manifests with unilateral firm swelling of cheek. It can be managed by excision and resulting contour defect that can be managed by reconstructing using vascularized dermal fat. Resorption is the main problem due to an insuf!cient blood supply, and vascularized dermal fat have been described to minimize this problem.Patients and Methods: In this study we report the results after insertion of vascularized dermal fat into zygomatic-maxilla region. The method consists of implantation of vascularized dermis with attached subcutaneous fat from the antero-lateral thigh (ALT) into the right zygomatic-maxilla region, anastomosing the vascular with temporal superficial vessel. During a one-year follow up, the patient showed good results. Graft atrophy was observed. Despite the more extensive surgery and some minor complications the safety of this method with good functional and cosmetic results makes vascularized dermal fat an excellent alternative to reconstruct facial contour defect.Results: Compared with free-fat grafts, resorption rates for vascularized adipose tissue transfers are very low. Satisfactory cosmetic results were also achieved.Summary: The advantages of this method is the minimized resorption and an excellent functional and cosmetic result. Therefor, we imply that vascularized dermal fat is an excellent method to reconstruct facial contour defect.

Abstract

Background: Venous stasis due to vein thrombosis is a complication after microsurgical procedure which if left untreated, may ultimately lead to flap failure. Medicinal leech has been useful as a conservative treatment to overcome venous stasis in cases where it is not possible to perform salvaging procedure.Patients and Methods: A 28-year-old female who suffered from a defect on the right heel following traffic accident. The patient was initially treated by performing sural flap to cover the heel defect, but was unsuccessful. Next reconstruction was done with free radial forearm flap, however in the early postoperative period vein compromise was seen which was caused by vein thrombosis. Even though heparinization therapy was immediately begun, the result was still unsatisfactory, so medicinal leech therapy was performed to prevent loss of entire flap tissue.Results: Leeches turned out to be bulging due to sucking process on the congested venous. The sucking result on this patient was satisfactory.Summary: This study shows that free tissue transfers with surgically unsalvageable venous obstruction can be helped by leech therapy protocol.

Abstract

Background: Nipple hypertrophy causes psychosocial embarrassment and discomfort to the affected individuals. The only mean to correct it is by performing surgical procedure aiming to reduce the diameter as well as the vertical height of the nipple while preserving the lactiferous ducts. Patients and Methods: Two cases of nipple hypertrophy were described in this case series : the first one was a male patient presented with an overly prominent nipple of the left breast. Second case was a female patient with bilateral nipple hypertrophy. Circular design was performed at the tip of the nipple to match the size of the normal nipple. Three wedged excisions was made, and also three radial pillars with its pedicle divided into two parts: the lower part was de-epithelialized. This technique was able to reduce the diameter and also the vertical height of nipples without compromising their vascular supply. Result: No complications such as nipple necrosis or sensory loss were found in both patients during follow-up period. The normal symmetry of the nipple contour was restored. Conclusion: We described a simple technique for both male and female patients with nipple hypertrophy, which provided good aesthetic result and patient satisfaction.

Abstract

Background: Being a developing country with a GNI (Gross National Income) per capita of US$3,830, the cost for managing patients with facial fractures was often unaffordable. The use of a free 3D-rendering software presumably would decrease cost. With OsiriX, a free and open source 3D-rendering software, complex images manipulations and real-time 3D image visualization is easily accessible anytime and anywhere from the surgeon’s personal computer, making diagnosis and preoperative planning more convenient for the surgeons. Patients and methods: From December 2009 to January 2010, 14 patients with facial fracture were admitted to our unit. Seven of them underwent surgery. Preoperatively, diagnosis & surgery plan was developed using OsiriX. Results: By using OsiriX, more accurate diagnosis was made at a lower cost. Patients could save up to 48.1% from total diagnostic cost (19.2% from monthly income). OsiriX provided easier access to multidimensional navigation and visualization tools in personal computers, making easier planning possible for surgical steps and materials needed. Summary: OsiriX being free and open source is bene!cial from !nancial point of view, which is an important point for patients in a developing country. It is user friendly and ef!cient in developing treatment plan for facial fracture patients.

Abstract

Management of condylar fracture draws controversy for over three decades. Previously, closed reduction with intermaxillary !xation (IMF) had been predominant for many surgeons, considering shorter length of procedure, ability to achieve preinjury occlusion with acceptable adjustment, and low cost of procedure. Introduction of osteosynthesis material rigid internal !xation (RIF) and adaptive miniplate !xation marked technology development in condylar fracture management and became popular in term of the applicability and outcome results. Over period time, several studies proposed vary classi!cation system and selection patient criteria for surgical treatment on the basis of age, location of fracture, degree of communition, direction of proximal fragment displacement, location of condylar head, concomitant medical illness or associated trauma, and patient’s preferences to achieve optimum goal. This review focuses on the shifting of condylar fracture management at Cipto Mangunkusumo Hospital from 2004-2012.

Abstract

Backgrounds: : Inthe past few years, clinicians worldwide have been using honey for wound treatment. But until now, there was no such standard on method of honey application on wound. In our center, honey was applied on wound by once a day application and the result was observationally satisfactory. What if application of honey were done once every two days? Would the result become more satisfactory? This study aims to search honey application method, which gives the best result on wound treatment. Methods: This is single-blinded non-randomize clinical trial, which was conduscted in Cipto Mangunkusumo Hospital Jakarta from July until September 2012. 14 Patients with acute partial thickness wound resulted from STSG harvesting were involved in this study. Patients were devided into 2 groups: control (once every two days application of honey) and the rate of wound healing were evaluated. Rate of wound healing will be assessed as number of percentage of reduced non-epithelialized areas on the seventh day of application. Results: The mean percentage of non-epithelialized area reduction on treatment gruop was 86,76% and 97,97% on control group. There was significant difference on percentage of reduced area between control and treatment group (p<0,00). Conclusion: There was statistically signi!cant difference between once a day and once every two days application of honey. However, changing of honey dressing once a day is still a preferable method in wound treatment.

Abstract

Background: Aesthetic surgery is a component of Plastic Surgery which deals with a “normal” person with neither disease nor disfigurement. They seek surgery to enhance appearance, which is expected to boost confidence level and performance. Because aesthetic patient come with no physical deformity, their expectation for perfection is high. As plastic surgeons, we must be aware of the pitfalls in our works, how much can we deliver the expectations of patients as suited to our knowledge and our capability in mastering surgical technique including the implementation of new techniques combined with technology. Our awareness throughout each surgery, and the obsession for perfection must measure up to what the patient desire. Patients and Method: A review of 3 augmentation rhinoplasty patient cases who come to my private practice between 2006-2012 with concern of postoperative results is presented, prior previous surgeries were done either by author or other surgeons. Result: All patients received surgical counseling and scheduled correctional surgeries, nearly all patient accepted the final results and were satisfied. Summary: Pitfalls are common in aesthetic surgeries. To prevent this, we must be aware all aspects involved starting from the first consultation with patient, carefully assess the anatomy and anomaly of each case, master the surgical techniques used, and careful when implementing new techniques. Finally, have the wisdom to say ‘no’ to patients who are not good candidates for aesthetic surgery.

Abstract

Background: Various reconstructive techniques for partial traumatic ear amputations have been reported. The choice of technique is based on the missing ear components and the availability of tissue for defect coverage. The goal is to obtain an aesthetically acceptable ear. The authors report a two-stage skin flap pocket technique for the reconstruction of traumatic ear amputations. Patient and Method: Three cases of partial traumatic ear amputation due to bite is reported. After sufficient debridement, skin flap pocket was created on the retroauriculomastoid area. Conchal cartilage graft was used to replace cartilage defect in one case. The other 2 cases utilized their amputated cartilage graft, deepithelialized and then resutured to the remaining cartilage. All three cases underwent second stage surgeries for flap division, three weeks after the pocketing. Result: In 2 months follow-up, all of 3 cases showed no signs of infection. Normal dimension of the ears were achieved and aesthetic appearances were perceived as acceptable to the patients and other viewers. One case was able to be followed one year post surgery with acceptable aesthetics, and no sign of cartilage resorption. Summary: The technique used in these cases provided acceptable results in reconstructing the size and shape of the partially amputated ears. Cartilage grafts implanted in the pockets retained their shape with no infection or resorption.

Abstract

Background: Posterior meningoencephalocele is a complicated case that requires adequate method for closure of the defect. Congenital meningoencephalocele is a rare incidence estimated at 1:3000 to 10000 live births; with occipital encephaloceles are the most common. The method for closure should prevent leakage of cerebrospinal fluid (CSF) and coverage for the calvarial bone. Meningoencephalocele are treated by excising the non-functional brain tissue and closed the defect using thick connective tissue graft, alloplastic material and local flaps. The problems with closure are continuous leakage of cerebrospinal fluid and infection, especially when alloplastic material is used. In this case report, a pericranial flap is used to close the dura. Patient and Method: A neonate with occipital meningoencephalocele was consulted to the Plastic Surgery Department with wound dehiscence and recurrent herniation of brain tissue after undergoing first surgery by the neurosurgery team. On the second operation, a premilene mesh was placed which was later infected and causing wound dehiscence. We then close the defect using pericranial hinged flap and primary closure of the skin and subcutaneous tissue. Result: After the closure using pericranial hinged flap, there were no signs of infection and no CSF leakage. In 4 month follow up, the defect has completely healed. Summary: Closure of calvarial bone defect with pericranial-hinged flap provides a tight closure of the intracranial space, without increased risk of infection. Pericranial hinged flap should be considered as a method of choice for closure of intracranial defect, preventing leakage of cerebrospinal fluid and reducing risk of infection.

Abstract

Background: The temporalis muscle !ap (TMF) is a very versatile and valuable axial flap, which could be used in various reconstructive procedures in and around the maxillofacial region. The surgical anatomy, vascular pattern and technique of elevation of the flap are described, associated with our experience in different reconstructive situations. Patient and Method: There were two patients, one case of TMJ ankylosis and one case of facial paralysis. The TMF was used as an interpositional arthroplasty for TMJ ankylosis, as a dynamic facial reanimation for facial paralysis. Result: In the first patient, he was able to open his mouth 4 cm in 2 weeks following the surgery. There was no pain or other complication complained. In second patient, in two weeks follow up after the surgery, we found the edema was decrease gradually. The contraction on the right nasolabial sulcus was slightly seen. Summary: These report described the reliability, versatility and reproducibility of temporalis muscle flap. The rich vascularized tissue and its proximity to the reconstruction site make this flap reliable. TMF should be taken into consideration before deciding on more extensive reconstructive procedures.
