Abstract

Hypokalemia is a condition in which the blood potassium levels are below the normal range, which is typically between 3.5 and 5.0 mEq/L. Potassium is an essential electrolyte that plays a critical role in many physiological processes in the body, including nerve and muscle function, heart rhythm, and acid-base balance. This literature review aimed to describe the clinical and current management aspects of hypokalemia in pediatric patients. The pathophysiology of hypokalemia involves a disturbance in the balance of potassium in the body, which can occur due to several mechanisms, including reduced intake of potassium-rich foods, increased loss of potassium, and redistribution of potassium. In children, clinical symptoms of hypokalemia may include weakness and fatigue, abdominal discomfort, cardiac symptoms, respiratory symptoms, neurological symptoms, polyuria, and renal symptoms. There are several diseases and conditions that can cause hypokalemia in children, including renal tubular acidosis, prolonged diarrhea, cystic fibrosis, hyperaldosteronism, malnutrition, medication, type 1 Bartter syndrome, type 2 Bartter syndrome, and Liddle syndrome. Some options for hypokalemia treatment are potassium supplements, a potassium-rich diet, intravenous potassium, and treating underlying conditions. In conclusion, hypokalemia is an overview of the underlying disease and requires immediate management. Understanding the pathophysiology of hypokalemia will increase the accuracy of diagnosis and accelerate hypokalemia intervention in children, as well as prevent complications due to hypokalemia.

Abstract

Background.Prolonged parturition or labor that lasts longer than usual can increase the risk of complications and the potential for maternal death. The birth ball exercise, also known as the birth ball exercise or the pregnancy ball exercise, can help harness the influence of gravity to induce labor progress. This study aimed to assess the effect of birth ball exercises on the progress of labor during the first active phase at the PONED UPTD DTP Pedes Health Center, Karawang Regency, Indonesia. Methods.This study is an experimental study, where as many as 30 research subjects participated in this study. Analysis of the progress of labor was carried out using SPSS software in a univariate and bivariate manner. Results. The results of the study showed that the birth ball exercise had an effect on the progress of labor with a p-value = 0.002 (p <0.05). Conclusion, there is an influence of birth ball exercises on the progress of labor at PONED UPTD DTP Pedes Health Center, Karawang Regency, Indonesia.

Abstract

Background. Cucumber is one of the natural ingredients that has been widely used traditionally to maintain facial skin health. Cucumber is rich in antioxidants which have the potential to suppress the inflammatory process that arises due to hormonal imbalances in melasma sufferers due to the use of injectable contraception. This study aimed to determine the effect of using a cucumber mask on the incidence of melasma in acceptors injection contraception users at the independent midwife practice in Bungursari, Purwakarta, Indonesia. Methods. This study was an experimental study in which a total of 30 research subjects participated in this study. Data analysis was carried out using SPSS 25 software in a bivariate manner to determine the effect of using cucumber masks on melasma. Results. After using the cucumber mask on the first day, there was an improvement in melasma in the intervention group compared to before using the cucumber mask, p=0.02 (p<0.05). Likewise, after using the cucumber mask on the second to the seventh day, there was an improvement in melasma in the intervention group compared to before using the cucumber mask, p <0.05. Conclusion. There is an effect of using a cucumber mask on melasma in acceptors of injection contraception users at the independent midwife practice in Bungursari, Purwakarta, Indonesia.

Abstract

Background: Breast cancer is the leading cause of death in women caused by cancer. The primary treatment for breast cancer is surgery, where several recommended surgical methods are breast-conserving surgery or mastectomy. Postoperative mastectomy reconstruction has provided an opportunity to overcome the emotional and aesthetic burden of malignancy. Methods: This research is a descriptive observational study with a retrospective approach. The sample was 45 breast cancer patients who had undergone a modified radical mastectomy operation at Dr. Kariadi General Hospital for January 2019 - April 2023. The data were processed using SPSS for Windows software, and a descriptive analysis was carried out. Results: Most of the patients who underwent post-MRM breast reconstruction were in the age group >40 years (71.1%), had stage 2 mammary carcinoma (44.4%) and stage 3 (40%), featured invasive ductal carcinoma (84 .4%), breast on the right side (64.4%). Some suffer from hypertension (13.3%) or diabetes mellitus (22.2%). The most used type of reconstruction was STSG (82.2%). A small proportion of patients experienced dehiscence (17.8%). In the Karnofsky score assessment, the highest score was 70 (64.4%), followed by 60 (17.8%), 80 (11.1%), 50 (4.4%), and 40 (2.2%). Conclusion: The patients undergoing MRM are dominated by the age group > 40 years, not suffering from hypertension or diabetes mellitus, stage 2, histopathology of invasive ductal carcinoma, right-sided breast. STSG dominates the choice of reconstruction with postoperative complications in the form of dehiscence in a small proportion of patients. The Karnofsky score is dominated by a score of 70.

Abstract

Background: Oocytes are gamete cells that play an important role in the daily practice of in vitro fertilization (IVF) laboratories. Oocyte morphology is important to study and assess in order to increase the success of IVF pregnancy. Methods: A computerized database search process in Pub med was performed to obtain data. Then, the most updated and relevant articles were selected. Results: Normal oocyte morphology should have normal-appearing cytoplasm, single polar body, appropriate zona pellucida (ZP) thickness, and appropriate perivitelline space (PVS). Abnormal oocyte morphology can be in the form of immature oocytes, large in size, ovoid or amorphous in shape, having fragment/double/multiple polar body I (PBI) and granular cytoplasm or smooth endoplasmic reticulum (SER). Conclusion: Abnormal oocyte morphology in the IVF laboratory can be seen by its maturity level, size, shape, condition of PBI, and cytoplasmic granularity. A better selection of oocytes can increase the performance of the IVF laboratory.

Abstract

Background: Paraffin injection into the penis is one way that people see it as increasing penis size. But many do not know that paraffin injection actually causes side effects in the form of penile paraffinoma. This study aimed to describe the management of paraffinoma using bilateral techniques. Case presentation: A 40-year-old married man came to the emergency room at Samuel J. Moeda Naval Hospital, Kupang, with complaints of pain and swelling of the penis for 6 hours. The pain felt to get worse when the penis was erected. The genital organs found swelling of the entire penile skin accompanied by tenderness and induration. The bilateral scrotal flap technique was because the scrotal skin had many blood vessels, good flexibility, little adipose tissue, thin skin, and could feel erogenous sensations almost the same as the physiological characteristics of the penile skin. Conclusion: Bilateral scrotal flap technique on the penile shaft is a surgical technique that is quite effective in eradicating paraffin from penile tissue.

Abstract

Background: Osteofibrous dysplasia is a rare benign bone tumor characterized by abnormal differentiation of fibrous tissue in bone and commonly involves the maxilla, mandible, and tibia and affects children less than 10 years old. Osteofibrous dysplasia affecting the metacarpal is a very rare case. This study presents a case report to present biological reconstruction in the management of metacarpal osteofibrous dysplasia. Case presentation: A 48-year-old female presented with swelling and pain in her right hand over 1 year. On examination, palpable mass with immobile and hard consistency on the first metacarpal and distal neurovascular within normal limit. Radiographic examination (X-ray and MRI) supports osteofibrous dysplasia. We performed complete resection of the first metacarpal and used a fibular graft to maintain length, followed by arthroplasty of the first metacarpophalangeal joint and arthrodesis of the first carpometacarpal joint. Pathological results showed a Chinese letter appearance with osteoblastic rimming that confirmed osteofibrous dysplasia. After 6 months of follow-up, there is no recurrence, and the functional outcome is excellent (MSTS score 93,33%). Conclusion: Osteofibrous dysplasia on the metacarpal and affected adult is a very rare case. Treatment with complete resection of the metacarpal, using a fibular graft to maintain bone length, and arthroplasty MCP joint will give good results (MSTS score 93,33%). Follow-up for recurrence is necessary.

Abstract

Background: Gastric perforation will cause acute peritonitis. Proper diagnosis and management, especially in ICU, are needed to prevent complications and even death from this event. This study aimed to describe the intensive management of diffuse peritonitis caused by gastric perforation. Case presentation: A 61 years old male patient presented with pain in the whole abdomen for 6 days before admission. The pain began at the epigastrium and suddenly spread to the whole abdomen. The pain was felt continuously and increased by movement. Routine blood examination revealed anemia, lekousitosis, and radiology examination found homogeneous confluence was found in the abdominal, dilated small intestine, psoas line was not visible, in the left lateral decubitus position, an air-fluid level and free air were found. The patient underwent diagnostic laparoscopic surgery + peritoneal lavage on emergency surgery for 2 hours, obtained a resolution of gastric perforation, and the patient was treated postoperatively in the ICU of Dr. M. Djamil General Hospital. Conclusion: Cases of spontaneous resolution of gastric perforation are not only in terms of surgical management but in terms of postoperative care. It also plays an important role in the success and good results in cases of gastric perforation.

Abstract

Background: One disease that is still a health problem in the world, including Indonesia, is tuberculosis (TB) which is caused by germs Mycobacterium tuberculosis. The success of TB treatment requires patient compliance in taking antituberculosis drugs (OAT), family support such as supervision, and motivating TB sufferers are factors that play a role in patient compliance. This study aims to determine the factors associated with medication adherence in pediatric patients with pulmonary tuberculosis in Mesuji Regency, Lampung, Indonesia. Methods: This study was an observational study with a cross-sectional approach, where as many as 52 respondents were included in this study. Data collection through interviews using a structured questionnaire. The subject of the study consisted of tuberculosis patients who were reported to have attended treatment for at least two months in Mesuji Regency. Data analysis was carried out using SPSS using univariate and bivariate methods. Results: The results showed that 76.9% of respondents adhered to the treatment of pulmonary tuberculosis. Patient medication adherence to tuberculosis was influenced by the presence of drug side effects (p-value = 0.007) and the supervisor's support for taking medication (p-value = 0.021). Conclusion: Compliance with taking medication in tuberculosis patients is influenced by the presence of medication supervisors and the uncomfortable side effects of antituberculosis drugs.

Abstract

Background: One of the causes of malnutrition that often occurs is gastrointestinal disorders. The existence of anatomical and physiological abnormalities in the gastrointestinal tract is the main cause. This study aimed to assess the relationship between gastrointestinal disorders and the incidence of malnutrition in children with cerebral palsy in the WKCP community Yogyakarta, Indonesia. Methods: A cross-sectional analytic observational study involving 20 cerebral palsy subjects who met the inclusion and exclusion criteria. The data collection technique is by consecutive sampling. The type of data used in the research uses a questionnaire gastrointestinal symptoms rating scale (GSRS). Data status anthropometry has also been collected. Data analysis using SPSS with chi-square. Results: The mean age of the subjects was 8.9 ± 4.1 years. The male gender (60%) is greater than the female. The comparison of good nutritional status and malnutrition is not much different, namely 55%:45%, respectively. The percentage of children experiencing severe gastrointestinal symptoms is 25%. The most common gastrointestinal disorders are constipation (65%) and upper dysmotility disorders (60%). In the results of this study, there was no relationship between gastrointestinal disorders and the incidence of malnutrition (p=0.069). Conclusion: In this study, there was no significant relationship between gastrointestinal disorders and malnutrition in children with cerebral palsy. However, gastrointestinal disorders increase the morbidity and mortality of children with cerebral palsy, so it must be an important concern.
