Abstract

Asthma is a chronic inflammatory disease of the airways characterized by recurrent wheezing, shortness of breath, chest tightness and cough especially at night and or early morning. Airflow resistance in asthma were caused by changes in the airways include bronchoconstriction, airway edema, hyperresponsiveness and airway remodeling. The inflammatory response in asthma patients varies among individuals, whether it can be immediate or late (slow-type) response. Different types of cells are known to play role in this process, especially mast cells, eosinophils, T lymphocytes, macrophages, neutrophils and epithelial cells. Degranulation of mast cells in the airways release inflammatory mediators and various metabolites that directly lead to smooth muscle hypersponsive resulting in airway obstruction. While eosinophil cells, mast cells and lymphocytes associated with slow-type responses, will release various mediators including leukotriene, prostaglandins and a number of proinflammatory cytokines. Type 2 immune response in the lower respiratory tract is a central immunologic process in asthma. This type 2 immune response is mediated by Th2 cells of CD4 + and IgE. The CD4 + Th2 cell is characterized by the large amount of transacting T-cell-specific transcription factor GATA-3 and the secretion of type 2 cytokines (IL-4, IL-5, IL-9 and IL-13). Excessive type 2 cytokines in the lower airway will trigger IgE-mediated hypersensitivity, epithelial cell activation, inflammatory cell inflation mediation into the airways, and cause remodeling responses in the epithelium and subepithelial matrices. This inflammatory cascade of type 2 cytokines is the pathological basis of the main symptoms of asthma.

Abstract

Background: Pleural effusion is a frequent complication of advanced stage are obtained in patients with malignancy. Shortness of breath at rest and decreased exercise tolerance are major functional limitations for patients with cancer who develop extensive pleural effusion. Pleural fluid drainage followed by a pleurodesis can reduce the symptoms of shortness of breath and prevent pleural fluid reaccumulation. Unfortunately, pleurodesis attempts fail in 10 to 40% of patients with recurrent pleural fluid and shortness of breath. Low pleural fluid pH can be used as a marker for increased metabolic activity of tumors in intrapleura, and is associated with increasing tumor, and can predict the failure of pleurodesis. The purpose of this study was to prove association between low pleural fluid pH value with failure pleurodesis in patients pleural effusion malignancy. Method: This is an observational study with cross sectional design that is the analysis conducted at Hospital Dr Soetomo in patients pleural effusion malignancy were subjected to pleurodesis for 6 months. Total 11 patients who meet the inclusion and exclusion criteria. Result: The mean value obtained pleural fluid pH 7,3±0,2, based on logistic regression analysis obtained low pleural fluid pH is not significant association with failure of pleurodesis, as shown from the significant value generated wald test is 0,116 where the value is above 0,05. Conclusion: Low pleural fluid pH is not significant association with failure of pleurodesis.

Abstract

Background: Mixed cellularity classical hodgkin lymphoma (MCCHL) is the secound subtype of classical hodgkin lymphoma (cHL) which often happens. MCCHL is aggressive but has a relatively high recovery rate. The diagnosis of cHL is sometimes difficult. Spontaneous regression can occur in cHL but is very rare, temporary or permanent. CHL including diseases with a fairly high cure rate, about 80% of patients recover with first-line chemotherapy. Case: Male age 26 years, 9 months cough, shortness of breath, chest pain 8 months, 6 months fever disappear with enlargement of right supraclavicular lymph nodes appearing at 11 days before admission. Chest X-ray shows the presence of mediastinal mass supported by contrast thoracic CT scan. FNAB has been done three times with no meaningful results. In one of the chest radiographs and CT scan of the thoracic with contrast evaluation showed a reduction in tumor size. Open thoracotomy biopsy is performed with Hodgkin’s lymphoma results. On immunohistochemical examination obtained MCCHL. Chemotherapy with ABVD regimen was administered for three cycles with partial remission and was continued with 6 cycles with stable disease outcomes. Conclusion: Spontaneous temporary regression in cases with mediastinal tumor suspicion may occur in cHL and may cause difficulties in diagnosing. Open biopsy is required as a gold standard and has to be supported by immunohistochemical test. First-line chemotherapy response in cHL is good.

Abstract

Outdoor air pollution contributed harmful impact to public health. There are several respiratory disorders related to outdoor air pollution such as acute respiratory infection, lung cancer, asthma, chronic obstructive lung disease (COPD) and lung function disorder. Respirator is a personnel protective device which has role in the primary intervention step. Currently exist many types of respirators in industrial setting that have specific function to certain hazard exposure in work process. It is difficult to choose one type of respirator that can be implemented in population setting to protect against all air pollutant content. Therefore, it is relevant choosing one respirator type which has the ability to effectively filtrate one of air pollutant content that is the particulate matter. One respirator type, N95 mask has superiority in term of cost and technical use aspects for protecting particulate matter pollutant. Respirator usage effectivity in population setting is an important subject to find out more.

Abstract

Background: Complicated drug-resistant Tuberculosis (TB) management becomes a distinctive challenge for health care provider in rural area. Case: We reported 6 drug-resistant TB cases that were found in Sikka Regency, East Nusa Tenggara, based on rapid molecular test. Each case had unique problem which needs specific management, such as difference between the conventional and rapid drug susceptibility test, appearance of second line injection drug resistant (Pre-XDR TB), Human Immunodeficiency Virus (HIV) co-infection, emergence of serious acute psychosis side effect, and the drop out management. Discussion: We will discuss the management of each individual’s problem that arises during the antitubercular treatment monitoring and the requiring regimen modification under the national guideline. Conclusion: Drug-resistant TB management is a very complex matter. However, with sustainable effort, commitment, and collaboration between referral center and health care provider in rural area, guidelin be appropriate management could be achieved. Despite its possibility, prevention of resistant TB should have been done as early as possible, one of them is completion of regular TB treatment.
