Abstract

Pulmonary Tuberculosis is a chronic infection that caused by Mycobacterium tuberculosis (M.tb) infection and it is still the major health problem worldwide. Mycobacterium tuberculosis infection can induce oxidative stress. Some studies has proved that active TB patients have an association with excessive oxidative stress which causes glutathione (GSH) level decrease and free radicals increase. Glutathione (GSH) facilitates the control of M.TB intracellular bacterial growth in macrophages and has direct antimicrobial activity.  N-acetylcysteine (NAC) is thiol, a precursor of L-cysteine and glutathione synthesis (GSH) that has been used for decades as a mucolytic agent in the treatment of respiratory diseases. Some studies report beneficial role of NAC as immunomodulator, besides NAC also has anti-inflammatory and antimicrobial effect in TB management.

Abstract

Background: Scavengers in landfills have high risk to pollution exposure. Carbon monoxide (CO) is one of important pollutants produced by burning process. Exhaled CO levels in scavengers is predicted to increase, caused by pollution exposure in workplace area. There has been no data on exhaled CO levels in scavengers, especially in Bantar Gebang, Bekasi.Methods: This study used cross-sectional design to the scavengers who work and live in Ciketing Udik, Bantar Gebang, Bekasi. This study was conducted in January - March 2015, by doing interviews and giving questionnaires, and conducted exhaled CO test.Results: Total sample was 108 subjects. Exhaled CO test result was 46 subjects (42.6%) had exhaled CO 5-10 ppm, 36 subjects (33.3%) > 10 ppm, and 26 subjects (24.1%) ≤ 4 ppm. Multivariate analysis between exhaled CO levels with characteristics of the subjects found smoking habits were significantly correlated with CO exhalation levels (p=0,000).Conclusion: There were 33.3% scavengers who had exhaled CO > 10 ppm. Smoking habits were the only factor that significantly correlated with CO in scavengers’ exhalation.

Abstract

Background: Most anti-tuberculosis (ATD) drugs are relatively safe, but unusual serious reactions can occur. Thrombocytopenia is an uncommon but potentially life-threatening complication of certain ATDs and is characterized by rapid destruction of platelets whenever an offending drug is taken by a susceptible person. Rifampicin is the most common cause of thrombocytopenia.Case: A 25 years old woman came with chief complaints, shortness of breath since 1 week before admission and cough with phlegm since 2 months before admission. The patient received antibiotic and ATD. In the course of improving on sepsis and pneumonia, the patient had thrombocytopenia accompanied by melena on day 4 of treatment.Discussion: Thrombocytopenia is defined as a disorder, which showed an abnormality on the low amount of thrombocyte. Thrombocytopenia was commonly cofounded when Complete blood count (CBC) was performed. The majority of the mechanism associated with thrombocytopenia is the immune. Drug-induced Thrombocytopenia (DITP) is an exclusion diagnosis, which is obtained by ruling out other underlying causes that resulted in thrombocytopenia.Conclusion: This case illustrates that the discovery of isolated thrombocytopenia in a patient taking several medications presents a challenging clinical problem. Laboratory confirmation of drug-induced thrombocytopenia at the time of initial presentation is not possible because tests for drug-dependent anti-platelet antibodies are not available in most clinical laboratories. The diagnosis of drug-induced thrombocytopenia can be supported only by resolution of thrombocytopenia after discontinuation of therapy with the suspected drug.

Abstract

Background: Lung cancer is one of the deadliest cancers in the world. The percentage of non small cell lung cancer (NSCLC) is about 80% of the incidence of lung cancer. The type of NSCLC, adenocarcinoma, is usually found in the presence of epidermal growth factor receptor (EGFR) mutations.Case. A male patient aged 70 years, an active smoker, works as a farmer. He has complained of shortness of breath, and chest pain for three months. There was no family history of suffering from malignancy. The cytology result of the right pleural fluid indicated adenocarcinoma. He was diagnosed with pulmonary adenocarcinoma (D) stage IV positive mutation of EGFR exon 18 (G719S), 20 (T790M) and 21 (L858R) carnofsky score 70. He could survive for more than 11 months with the treatment of EGFR TKI, and received a good therapeutic response. Initially, for the first six months it was such a progressive disease, and for the next eleven months it became stable.Conclusion: In addition to the exon mutations found in this case, the cells in the tumor will continue to grow and develop into new mutants that are immune such drugs and rapidly split themselves into new, different forms. The therapy for complex mutations is still being developed. EGFR TKI therapy in this patient had a relatively good response. Further understanding of the molecular biology of lung cancer is seriously required.

Abstract

Lung cancer is the primary cause of mortality in the world. It is able to manipulate the host immune response system through many mechanisms, such as through alteration of cytokines structure, forming regulator T-cells, obstruction of cellular immunity function, and the interference of tumor antigen presenting process. The new therapy approachment is produced by stimulating anti-cancer immune response, therefore the growth of lung cancer is able to inhibit. Immune checkpoint inhibition is considered as therapy for non-small cell lung carcinoma (NSCLC) after the unsuccesful treatment by platinum-based chemotherapy. Recent study shows that immune checkpoint inhibition monotherapy is more distinguished as first line therapy than platinum-based chemotherapy. Nonetheless, the effect of immunotherapy is only available for small population (30%) which has more than 50% PD-L1 programmed by the tumor. Therefore, some strategies are investigated to solve this issue. Nowadays, immunotherapy is expected to overcome lung cancer which is still being investigated in many studies.
