Abstract

Objective: Measuring levels of cobalamin (vitamin B12) in blood is frequently utilized to diagnosis deficiency in patients with recurrent oral ulceration. This article aims to report an interesting rare case of recurrent oral ulceration with a high level of cobalamin.Methods: a 44-year-old female came to dental clinic with chief complaint of recalcitrant oral ulceration since half a year ago. Patients basically had history of periodic oral ulcer for quite long time with short occurrence and only one or two ulcer in one period. However, the ulcer become alarming after she was hospitalized due to extreme fatigue. Patients subsequently instructed for routine blood along with cobalamin and IgE total examination.Results: The test came back with highly decreased level of red blood index indicates that the patients on severe anemic condition. Unexpectedly, the levels of cobalamin in blood increased by 70% from the referral score. Patients urgently refer for hospitalization to recover her Hb level. After blood transfusion, patients discharged from the hospital but under hematologic observation for the exact etiology of her anemia, including the high cobalamin level. After two months observation, patients Hb gained normal level, however the vitamin B12 level remain. Patients reported decreased in number of ulcers.Conclusion:  It is worthy to always check on cobalamin level for patients with recurrent oral ulcers since lower level can directly causing it or the high level can lead to some life-threatening diseases which can also trigger the recalcitrant oral ulcers.

Abstract

Although a benign condition of no medical concern, patients in Pakistan commonly complain of “black gums” especially those with high lip line and esthetic concerns. Dentists have been correcting this condition with a wide range of treatment modalities, including diamond bur gingivectomy, scalpel gingivectomy with or without subsequent free gingival autografting, electrosurgery, cryosurgery, laser surgery. In this case series, a diode laser was used for depigmentation, which yielded good esthetic results, patient satisfaction and no pain or infection.

Abstract

Objective: This case report aims to describe the management of single-visit endodontic treatment in asymptomatic irreversible pulpitis case.Methods: A 25-year-old woman came to RSGMP Unhas with complaint of a cavity in the lower left back tooth, had felt pain ± 2 weeks ago. Intraoral examination showed thermal test (+), percussion (-), palpation (-). The periapical radiograph shows a normal periapical and the root appears straight. The case diagnosis was asymptomatic irreversible pulpitis. Root canal were treated with single-visit endodontic treatment. The root canal was prepared using single length techniques and obturated with single cone techniques.Results: Patient showed no complaints of pain after treatment, the teeth continued to function properly and nothing disturbed the patient`s comfort especially when chewing.Conclusion: Endodontic single-visit treatment provides good and satisfactory results in asymptomatic cases of irreversible pulpitis.

Abstract

Objectives: This report describes 2 cases diagnosed with minor RAS treated with LLLT to relieve pain and promote the healing of the ulcers.Methods: A 21-years old female presented of ulcer on labial mucosa for about 4 days with VAS=5. The ulcer always appears 1-2/month before or on her menstruation period. A 21-years-old female presented of ulcer on right buccal mucosa for about 2 days with VAS=8. The ulcer always appears once a month. The predisposing factor in this case were trauma and stress.Results: We treat the patient with LLLT and educated the patients for management the predisposing factor. After treatment with laser, the pain decreased immediately. The lesions healed completely within 4-5 days and scoring of the Ulcer Severity Scores (USS) also decreased.Conclusion: Low level laser therapy is recommended for patients with minor RAS since it can relieve pain, reduce the healing period and lesion size and recurrence of the lesions.

Abstract

AbstractObjective: This case report aims to describe a case of cerebral abscess due to a Spingomonas paucimobilis infection preceded by intracerebral hemorrhage at the same location.Methods: Approach and management of a 48-year-old man with worsening consciousness and fever with a history of intracerebral hemorrhage that occurred eight weeks agoResults:A previous 8-week head CT scan showed a well-defined, oval-shaped, hyperdense lesion in the right frontotemporoparietal region, along with perifocal edema, left lateral ventricular compression, and midline shift to the left of 1.3 cm, with an impression of right temporoparietal lobe intracerebral hemorrhage. A head CT scan at the time of initial admission showed a well-defined, round hypodense lesion with ring enhancement, perifocal edema, left lateral ventricular compression, and a midline shift to the left of 1.4 cm at the same site with previous intracerebral hemorrhage, with the impression of a cerebral abscess. Spingomonas paucimobilis was discovered in the brain matter culture. The diagnosis of cerebral abscess can be established.Conclusion: Cerebral abscess may be preceded by a previous intracerebral hemorrhage. A cerebral abscess can be caused by a Spingomonas paucimobilis infection, especially in immunodeficiency conditions. With additional monitoring, switching from intravenous to oral antibiotic therapy can be considered if there is a clinical and radiographic improvement.

Abstract

Coronavirus disease 2019 (COVID-19) is an infectious disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) that presents with a multitude of clinical presentations in various body systems. In addition to hematological, radiological, and biochemical findings, oral manifestations of COVID-19, including but not limited to xerostomia, loss of taste, and oral lesions have been reported. To our knowledge, this is the first case to report on a series of multifactorial and concurrent viral, autoimmune, and fungal oral lesions following a COVID-19 infection, namely the reactivation of herpetic gingivostomatitis, herpes-associated erythema multiforme, and concomitant oral candidiasis. This study aims to present a case of compounded immunological involvements of COVID-19 and frame it as a possible predisposing factor to the development of oral lesions and symptoms due to COVID-19’s profound role in immune dysregulation. Further studies are needed to investigate the etiopathogenesis and pathophysiology of the recurrence of oral lesions following COVID-19 infections.
