Abstract

Hepatitis is a inflammation process which may present in an acute or chronic phase. A decrease of serum albumin level may befound in liver disease. Ratio de Ritis in mild hepatitis is < 1 while in alcoholic hepatitis is >1. To analyze serum albumin level withratio de Ritis in hepatitis B patients. A cross sectional study was carried out comprising 46 subjects from October 2005 to August 2006measuring albumin level SGOT and SGPT levels using Lyasis autoanalyzer. Thirty eight males and eight females, the level of serumalbumin decreased with an average of albumin level 2.98 gr/dL (p<0.05). Twenty two subjects showed a ratio de Ritis ≤ 1 with theaverage albumin level 3.00 gr/dL and 24 subjects were included in group ratio de Ritis > 1 with average albumin level 2.96 gr/dL(p value = 0.658). There was a decrease of serum albumin level in hepatitis B patient but no significant difference between decrease ofserum albumin level with ratio of de Ritis ≤ 1 and ratio of de Ritis >1.

Abstract

To face the global competition of clinical laboratory services, some techniques are needed for strategic formulation. SWOT, external,and internal analysis have been known as techniques for those purposes. SFAS technique was introduced recently as a simple procedureto be used as complementary tool. To evaluate an program prioritization using SFAS technique. Qualitative descriptive case analysis.Observation was done into Clinical Pathology Department of Sardjito Hospital, Yogyakarta during its activities in the year of 2006.IFA and EFA matrices showed the total scores > 2.5 and SWOT analysis revealed that the position was in 1st quadrant of the matrix.SFAS technique showed the total score > 3.0 that mean the prioritization was appropiate. SFAS technique is valuable in supportingthe formulation of strategy.

Abstract

The prevalence of Dengue Hemorrhagic Fever (DHF) in South Sulawesi is still high. Proper and immediate management is importantto reduce the disease morbidity and mortality The aims of this study are to know the between correlate in stages of the disease and valuesof blood IgM-IgG, routine haematology analyses as will as infection status. A cross sectional study was done at Wahidin SudirohusodoHospital Makassar in January – June 2006. The diagnosis of DHF following WHO criteria (1999) and Immunocromatographic unit(ICT) for sera IgM-IGg have been conducted. From 83 collected samples, 37 male (44.6%) and female 46 (55.4%) ranged betweenage 6 months up to 47 year old. The decrease of trombocyte counts (mean 70.120/μL) and increase of (mean 10.09%) were observed.Serological unit result showed 10 (12%) primary infections and 68 (82%) secondary infections. Immunoglobulin G could be detectedin the second day of fever. The samples, 56 (67.5%) were stage I, 15 (18.1%) were stage II, and 12 (14.4%) were stage III or IV, thatmeans there were correlation between infection status and disease’s stages (p<0.01). It can be concluded they routine blood analysisis still useful for diagnosis of DHF, because it was confirmed by rapid serological unit specifically for IgM-IgG in early phase of DHF. Sothey an adequate treatment came out in order to prevent severe case of the disease.

Abstract

CD4 test and Viral Load have been done for initiating, controlling, and deciding the treatment and prognosis of HIV/AIDS.If bothtests cannot be done, clinical symptoms and total lymphocyte count (TLC) can be used. TLC and Hb can be used for monitoring theHIV progressiveness. To evaluate routine blood test results, TLC, and CD4 of HIV/AIDS patients. Cross sectional and descriptive studyfrom October 2005 to April 2006, of 30 patients, i.e. 21 men (70%) and 9 women (30%). Leucopenia was found in 16.67% men and13.33% women. Anemia Hb in 70% men and 30% women with mean value 2.52 g/dl; lymphocytopenia in 46.67% men and 10%women; and netropenia in 16.67% men and 10% women. CD4 was discovered decline in 66.67% men and 30% women with mean value63.4 cell/mm3. TLC in 43.33% men and 16.67% women with mean value 1081.21 cell/mm3. In the study we found i.e. leucopenia,anemia, limfocytopenia, and neutropenia. CD4 and TLC are mostly decline. Further researches in the bigger populations are conductedto consider the parameter of routine blood tests, TLC, and CD4 on HIV/AIDS patients for initiating therapy.

Abstract

Leptospirosis is a frequent zoonosis in the tropics including Indonesia with 5–30% incidence and 5–20% mortality rate due toacute renal failure (ARF). To determine the frequency of leptospirosis by sex, age and glomerulus’s filtration rate (GFR) of the patient.A retrospective descriptive study on 32 subjects was done by measuring the creatinine clearance using Lyasis Auto analyzer in ClinicalPathology Laboratory of Wahidin Sudirohusodo Hospital from January 2004 to June 2006, of 32 patients, 25 (78%) male and 7 (22%)female, with the most frequent age range from 30–40 years (34.4%). Of the patients, 2 (6.3%) have normal GFR, 6 (18.8%) withmild GFR reduction, 10 (31.3%) with medium GFR reduction, 10 (31.3%) with severe GFR reduction and 4 (12.5%) with acute renalfailure. Leptospirosis is most frequent found in male with age between 30–40 years old. with reduction of GFR from mild to severe, andeven terminated with acute renal failure.

Abstract

A rapid immunochromatographic test that incorporates blue particle conjugated with recombinant dengue envelope protein,antihuman IgM monoclonal antibodies and antihuman IgG monoclonal antibodies (Virotec Dengue IgG/IgM XP; Indec Diagnostics,Indonesia using technology of USA) was evaluated by using IgG and IgM captured ELISA (Dengue Duo ELISA, Panbio) which has a99% sensitivity and a 92% specificity as the “gold standard”. This assay, cassette in shape, is performed in 15–30 minutes and detectsboth immunoglobulin M (IgM) and IgG in a capture format. There were 57 paired sera of dengue haemmorhagic fever (according toWHO 1997 criterias) and 72 sera of non dengue haemmorhagic fever patients (typhoid fever, UTI, malaria, chikungunya, hepatitis andpneumonia). For the diagnosis of dengue virus infection, the rapid test showed a sensitivity of 98%, positive predictive value of 88%,negative predictive value of 98% and specificity of 99%. The rapid test is a useful aid in rapid diagnosis and differentiation betweenprimary and secondary dengue virus infection.

Abstract

Rubella infection with occurs during pregnancy, especially during the first trimester often caused by Congenital Rubella Syndrome(CRS). CRS can resulting abortions, miscarriages, stillbirth, and severe birth defects. The baby diagnosed with CRS when characterizedby signs or symptoms from the following two categories A (Cataracts/congenital glaucoma, congenital heart disease (most commonlypatent ductus arteriosus or peripheral pulmonary artery stenosis), loss of hearing, pigmentary retinopathy) or one categorie A andone catagorie B (Purpura, splenomegaly, jaundice, microencephaly, mental retardation, meningoencephalitis, radiolucent bone disease.Laboratory confirmation can be obtained by any of the following: virus isolation, serologi test (pasif hemaglutination, latex agglutinationtest, hemaglutination inhibisi, Flouresence immunoassay, Enzyme immunoassay), RNA test.

Abstract

Nephrotic syndrome (NS) is characterized by proteinuria, hypoalbuminemia accompanied by oedema and hypercholesterolemia.Nephrotic syndrome is an often relapsing disease (75%) and often the diagnosis is too late. This disease is 15 times greater in childrenthan in adult and the male to female ratio is 2:1. Laboratory examination is needed to rapidly detect and evaluate the progress of thedisease for treatment. To know the distribution of NS based on aged, gender, cholesterol, and albumin concentration and the urinesediment profile. The data in this retrospective descriptive study were collected from 56 patients with NS at the Wahidin SudirohusodoHospital, Makassar, in the period of January 2004 – June 2006. NS were found in 36 male patients (64.3%) and 20 female patients(35.7%). Cholesterol concentrations were 250 mg/dl in 50 patients (89.3%) and 250 mg/dl in 6 patients (10.7%). Albuminconcentration was 2.5 g/dl in 21 patients (37.5%) and < 2.5 gr/dl in 35 patients (62.5%). In urine sediments, there were found in 23patients (41.1%) with positive proteinuria (+++), 51 patients (91.1%) with positive erythrocytes, 54 patients (96.4%) with positiveleucocytes, and 33 patients (58.9%) with positive cylinders such as rugged granular and fatty cost. More NS were found in male patientsin comparison to female, and many were aged + 6 years. Hypercholesterolemia, hypoalbuminemia, proteinuria, hematuria, leucocyturia,and positive cylinder (rugged granular and fatty cast) were found in the urine of most of the NS patients.

Abstract

In this report we describe a patient with adult acute lymphoblastic leukemia with bilineage phenotypic. He was found to have massive right pleural effusion with mediastinal shift to the contra lateral side. There was also a smaller left pleural effusion. He had multiple bilateral cervical lymphadenopathy, tense ascites and bilateral pedal oedema up to the shins. He was otherwise clinically stable.His full blood count on admission showed Hb of 11.4 g/dl, platelets of 59 X 109/L and WBC of 12.99 × 109/L with blasts of 26%. His renal function was normal with a creatinine of 107 micromoles/L. Bone marrow trephine biopsy showed features consistent with acute lymphoblast leukemia-L1. Flow cytrometry of his blood was suggestive of bilineage phenotypic acute lymphoblast leukaemia. It showeda single population of blasts (about 31%) which expressed cCD3+, CD4-, CD7+, CD5-, CD19+, CD34+, TdT+, cytoplasm IgM, CD79a+ and 30% are CD10+, and there was aberrant CD33+ expression with no evidence of MPO or CD117 expression. Cytogenesis of the bone marrow trephine biopsy showed numerical and structural abnormalities in nine out of the seventeen cells analysed. These abnormalitiesare 43~47,XY, add (1)(p34.2), add(2)(p13), i(17)(q10), +21[cp9].
