Abstract

Traditional bone setting is common in developing nations. The practitioners are barely educated and rely so much on experience. Although they are popular in public, traditional bone setter are known in hospital practice for their failures and complications which present to hospitals for remediation.The purpose of the study is to report the orthopaedic emergency cases  caused by the practice of traditional bone setter. This was an observational descriptive study, 36 patients with complications after practice of traditional bone setters and subsequently presented to emergency ward in Hasan Sadikin Hospital between 1st January 2015 and 31st August 2017 were evaluated.Among emergecy orthopaedic cases at Hasan Sadikin Hospital caused by bonesetter practice, compartment syndrome and gangrene are the most common complications occured. Twenty patients (56%) with infected wound and compartment syndrome had successful operation., thirteen patients (36%) with gangrene of the extremities had amputations, and three patients (11%) with multiple organ failure died. Twelve percent of amputations in our centre were related to bonesetter practice.The result of this study indicates that traditional bone setters practice may caused patients to develop complications, lose their limbs even lives due to inappropriate treatments.

Abstract

Background: Scar formation has been a classical issue in the healing of skeletal muscle defect which will results in decrease of strength and function. Platelet-rich plasma (PRP) has been studied to enhance the healing of muscle defect. The disadvantages of it include the risk of immunologic reaction, time-consuming preparation, and lack of scaffold element. Platelet-rich fibrin (PRF) is a new generation of platelet concentrate that provides good source of growth factors and scaffold element necessary for the healing. This study is to provide a fondation of PRF preparation and implantation for the healing of experimental defect of gastrocnemius muscle in an animal model.Purpose: The goal is to provide a fondation of PRF preparation and implantation for the healing of experimental defect of gastrocnemius muscle in an animal model.Methods: Animal Care and Use Committee, Airlangga University approved the study. Twenty New Zealand white rabbits were divided into two groups. In control group, a defect was created in the right gastrocnemius and no implantation was done. In study group, similar defect was created with implantation of PRF. Two and four weeks after surgery, the defect was examined immunohistochemically for the expression of Pax7 protein.Results: The implanted group showed higher IRS score in two and four weeks compared to the control group.Conclusion: The use of PRF facilitates the healing of muscle injury. This study will be used as a fondation for further study on PRF.

Abstract

Background: limb deformity, especially lower leg deformity is a common case in orthopaedic practice. Limb deformity can be classified according to cause (Congenital, developmental, post trauma), geometry (angulation, rotation, shortening), severity and progression. In severe and complex deformity, the treatment choice is gradual correction using Ilizarov system. Benefit of the gradual correction using Ilizarov system are: (1) reduce the risk of neurovascular injury, (2) minimal soft tissue disturbance, (3) multiplanar and multidirectional correction.Cases: We have performed 11 deformity correction using ilizarov device during january 2013- September 2017. Evaluation data was obtained retrospectively from medical record. Nine patients were managed by Ilizarov device for bone lengthening indication and 2 patients for bone transport indication. The treatment result was evaluated using ASAMI scoring system for bone condition and functional condition.Results: Elevent patients were managed by Ilizarov system for gradual deformity correction. Nine patients already finished the treatment and the frame was removed. We cannot evaluate two patients because the correction was not finished. The ilizarov device was changed with other fixation because implant failure during correction phase. Evaluation using ASAMI score showed good bone score result for 9 patients and 7 excellent results for functional score.Conclusion: Ilizarov system has a promising result to correct lower limb deformity patients in Dr Soetomo General Hospital.

Abstract

Background: Cartilage has limited capacity for self-repair in case of defect. Stem cell based tissue engineering has become an alternative therapy for the treatment of cartilage defect. The use of decellularized cartilage bovine can serve as scaffold to support proliferation dan differentiation of the stem cell.Purpose: This research is to compare the effect of decellularization technique on collagen type II and matrices porosity of cartilage bovine scaffold.Methods: This experimental laboratory research used cartilage bovine scaffold which was decellularized using physical, chemical and enzymatic techniques with different time and concentration variations, then collagen type II measurement was performed by immunohistochemical method and evaluation of porosity size with electron microscopy.Results: In the preparations with the highest concentration and the longest immersion time of 5% SDS for 72 hours, the decellularization process occurs perfectly. The collagen type II content was found to have significantly higher concentrations than the controls and other preparations, and have interconnected porosities in large size.Conclusion: High concentrations of collagen type II  and large diameter of pores after decellularization process in cartilage bovine scaffold confirm that this scaffold still has optimum biochemical cues that can be a good mechanical framework and microenvironment stem cell in effort to repair the cartilage defects. This result was expected to become a standard for the decellularization making of cartilage bovine scaffold.

Abstract

Background: Neck femur fracture has a poor prognosis due to its anatomical structure. Fractures that occur in the elderly often have a heavy comorbid so that the mortality rate in 1 year in this case reaches 30-50%. The techniques that have been developed at this time are not satisfactory results with non-union rate of about 30%.Purpose: Assessing the long-term effectiveness of fibular auto strut graft techniques for neck femoral fracture therapy.Methods: The study was conducted retrospectively using primary data in the form of clinical evaluation and secondary data in the form of medical record data and questionnaires of neck femur fracture patients who performed non-vascularized autofibular strutgraft therapy in the period January 2005 to August 2015 in operating room IRD RSUD Dr. Soetomo Surabaya. Patients were evaluated with Harris Hip Score.Result: Medical record data shows that surgery was performed in January 2005 until August 2015 in the IRD hospital operating room. Soetomo Surabaya with 15 patients that fit criteris inclusion. The final result of Harris Hip Score is excellent in 66.66% of patients, good in 20% of patients, and poor on 13.33% of patients. It can be seen that in large part, the technique of autofibular strutgraft (Surabaya Technique) gives good results to the sufferer.Conclusion: Autofibular strutgraft and reinforcement with cancellous lag screw on neck femur fracture can generally provide good functional results in long term evaluation. From the assessment of functional aspects, abnormal anatomical conditions, and Range of Motion (ROM), obtained satisfactory results. This supports autofibular strutgraft as a neck femur fracture therapy as a major therapeutic option at a young age.

Abstract

Background: The treatment for acromioclavicular joint injury are debatable, there are fixation options include tension band wiring, AC joint reconstruction and hook plate These fixations are capable of providing a stable fixation, but controversy still exists that mentions the superiority of each of these fixationsPurpose: To compare biomechanical stability of 3 fixation include tension band wiring, double endo button, and hook plate to provide a scientific basis of the fixation.Methods: This research is an experimental in vitro. Using 27 acromioclavicular joints cadaver divided into three groups that performed tension band wiring fixation, double endo button and hook plate. Each fixation evaluated with 10, 20, 50 and 100 times repetitions with 100N traction force.Results: Tension band wiring gives the smallest displacement. In 10 times repetition average displacement of tension band wiring 0.056 mm (p = 0.000) compared to double endo button 1.622 mm and hook plate 0.867 mm. In 20 times repetitions, tension band wiring 0.1667 mm (p = 0,000) compared to double endo button 3.1778 mm and hook plate 1.1111 mm. In 50 times repetition, tension band wiring 0.3111 mm (p = 0.000) with double endo button 4.7778 mm and hook plate 1.3556 mm. In 100 times repetitions, tension band wire 0.556 mm (p = 0.000) while double endo button 5.4444 mm and hook plate 1.4556 mm.Conclusion: Tension band wiring have a good stability compared to double endo button and hook plate. But all of fixation provide stability for acriomioclavicular joint motion.

Abstract

Background: Osteosarcoma is the most common bone neoplasm found in the community but evaluation osteosarcoma cases in RSUD Dr. Soetomo has not been updated since 1995.Purpose: This study o osteosarcoma patient characteristic as well as therapy at Dr. Soetomo General Hospital in 2007 to 2016. It expects to show survival rates of osteosarcoma patient, so it can be a reference for searching the problems in the treatment of osteosarcoma cases and helps to decide treatment for osteosarcoma.Research Methods: Descriptive retrospective study, conducted on osteosarcoma patients at Dr. Soetomo General Hospital during 2007-2016 periods. Data were obtained from Ortho tumor patient database, and contacting them by phone or home visit.Results: Osteosarcoma patients was found mostly in 2015, while the least in 2008, with trend increasing by time. Majority of the patients came with advanced stage. Osteosarcoma treated mostly by amputation, either with or without chemotherapy. The survival rate in the first, second, or the fifth year was found lower than other references. Most common cause of mortality was the metastasis.Conclusion: Awareness of the society about the cancer sign of cancer and desire to use medical treatment as a priority is still low. This causes a low early detection rate of osteosarcoma and a high rate of metastatic cases because of inappropriate early treatment. Further socialization and increased awareness of medics about the suspicion of osteosarcoma are needed to improve the success rate of treatment as well as the survival rate.
