Abstract

Abstract Background : The ampulla of Vater carcinoma is a rare gastrointestinal malignancy but has a better prognosis than pancreatic cancer. Severe malnutrition is common in patients with gastrointestinal cancer and worsens clinical outcomes. Objective: To report the implementation of structured perioperative nutrition therapy in a severely malnourished patient with carcinoma of the ampulla of Vater. Methods: Case report of a 51-year-old woman with severe malnutrition and obstructive jaundice, who underwent a Whipple procedure . Nutritional status assessment was performed using Subjective Global Assessment (SGA), anthropometry, and hand grip strength (HGS). Perioperative nutritional therapy was carried out in stages, with clinical and laboratory monitoring. Results: Nutrition therapy was started preoperatively with the ERAS method and continued gradually postoperatively through enteral and parenteral routes, until the patient was able to accept a full oral diet and vitamin supplementation. The target energy of 1840 kcal and protein 78 g/day was achieved within 7 days. Clinical status and physical function improved during treatment. Conclusion: Appropriate perioperative nutritional therapy supports metabolic recovery, improves nutritional status, and enhances clinical outcomes in patients with advanced gastrointestinal cancer undergoing major surgery. Abstract

Background: Systemic lupus erythematosus (SLE) is a chronic systemic autoimmune disease characterized by the production of autoantibodies and multisystem involvement, predominantly affecting women of reproductive age. Vitamin D is known to have immunomodulatory effects, including inhibition of T and B cell activation and suppression of pro-inflammatory cytokine production. Objectives: This evidence-based case report aims to evaluate the effect of vitamin D supplementation on disease activity in SLE. Methods: A literature search was conducted using PubMed, Scopus, and Cochrane. Medical Subject Headings (MeSH) terms and relevant keywords based on the clinical question components were applied. All retrieved literature was screened using predefined eligibility criteria, followed by a critical appraisal of eligible studies. Results: One meta-analysis of randomized controlled trials (RCTs) and two RCTs met the eligibility criteria. Almost all of the included studies recruited patients with SLE who had vitamin D deficiency/insuficiency. The methodological validity of all three studies was considered acceptable. The studies demonstrated heterogeneity in terms of baseline disease activity levels among their study populations. The meta-analysis and one of the RCTs reported non-significant results, while the other RCT showed a significant improvement in disease activity following vitamin D supplementation. Conclusion: Current evidence is insufficient to recommend vitamin D supplementation as an adjuvant therapy for improving disease activity in patients with SLE and further research is needed.

Abstract

Background: Dry skin affects 50%–80% of Indonesia. Data from the Geriatric Department of the Dermatology and Venereology Clinic at Dr. Cipto Mangunkusumo Hospital (RSCM) in Jakarta between 2008 and 2013 indicate that dry skin is one of the ten most prevalent illnesses. Objective: Regarding skin moisture, this study tries to determine the impact of vitamin C on young adult participants. Methods: This is an observational analytical study with a quasi-experimental design carried out at Maranatha Christian University in Bandung. Medical students between 19 and 29 years old at Maranatha Christian University's Faculty of Medicine served as the study participants. Based on gender, sixty participants were split into two groups. TEWL levels were measured using the Skin Moisture Analyzer FCM-1. Vitamin C intake was obtained from a food frequency questionnaire containing food sources of vitamin C, which was then analyzed using the Nutrisurvey-2007 program, as well as vitamin C from supplementation. Results: Employing paired t-tests, the p-value for both male and female groups was <0.001, suggesting a substantial change in skin moisture levels before and after vitamin C intake. Conclusion: Young adults' skin moisture levels are affected by vitamin C intake.

Abstract

Background: Severe burns cause microbiota dysbiosis, impaired intestinal motility, and constipation, worsening patient conditions and prolonging hospitalization. Research Objective: To evaluate the effectiveness of synbiotic supplementation in improving bowel movement frequency in burn patients. Case Illustration: A study conducted in the Burn High Care Unit at RSUPN Cipto Mangunkusumo involved a 42-year-old male patient with 32% TBSA burns and a history of kidney transplantation. Treatments included debridement, skin grafts, electrolyte correction, and synbiotic supplementation starting on day 6. During 29 days, energy intake, fiber intake, bowel movement frequency, and clinical status were monitored, showing significant improvements until discharge. Discussion: The 42-year-old male patient with 32% TBSA burns experienced impaired intestinal motility and constipation consistent with microbiota dysbiosis and decreased SCFA due to severe burns. The patient received enteral nutrition and Rillus® synbiotic supplementation from day 6, theoretically enhancing beneficial bacterial colonization and intestinal motility. Bowel movement frequency improved from day 7, indicating synbiotic-related benefits. Micronutrient supplementation according to ESPEN supported wound healing and immunity. Clinical condition and nutritional status improved with significant wound healing until discharge on day 30. Conclusion: Synbiotic supplementation effectively increased bowel movement frequency and improved intestinal function in severe burn patients with constipation. Abstract

Introduction: Pneumonia is a major cause of illness and death, especially in critically ill patients. Probiotics have been proposed as a supportive intervention to enhance immune function, maintain gut microbial balance, and strengthen the intestinal barrier against inflammation. Research objectives: The aim of this review is to systematically identify relevant evidence regarding the relationship between probiotics and pneumonia. Methodology: The article employs a systematic review as its methodology. A comprehensive literature search was conducted across PubMed, Google Scholar, and Scopus. The selection of articles for the review was based on established inclusion and exclusion criteria, adhering to the guidelines set forth by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Result: Based on 50 articles, 10 articles were filtered that met the inclusion criteria. The reviewed studies indicated that probiotic supplementation may reduce the severity of pneumonia, particularly ventilator-associated pneumonia (VAP), and improve immune response in critically ill patients. Other research supports that probiotic administration can reduce ICU stays and improve overall good immunity. However, other studies have shown that probiotic administration does not affect the length of stay or therapy in the ICU for pneumonia patients. Conclusion: The administration of probiotics in pneumonia patients provides multiple benefits, particularly by enhancing immunity through the modulation of macrophages within the gut–lung axis. Abstract

Burn injuries result in tissue damage due to exposure to thermal sources or extreme cold. Following burn trauma, systemic inflammatory responses can impair gastrointestinal (GI) function, often leading to constipation. While dietary fiber supplementation is commonly recommended to prevent constipation in the general population due to its ability to enhance intestinal motility, there is limited literature regarding its role in managing constipation among patients with major burn injuries. This case series presents two patients admitted with 34% and 35% total body surface area (TBSA) burns, respectively. Both patients experienced delayed defecation during the first week of hospitalization, without experiencing hypovolemic shock, and had not been administered any sedative agents or vasopressors. Their average dietary fiber intake were 11.1 g and 6.5 g, respectively. One male patient, who was diagnosed with diabetes mellitus (DM), continued to experience constipation until the second week, which improved following dietary modification to a DM-specific regimen. Meanwhile, the female patient showed an improvement in bowel movements in the second week, despite her low fiber intake. Keywords: fiber intake, defecation, major burns

Abstract

Gastrointestinal cancer patients frequently experience treatment-related anorexia, gastrointestinal toxicity, and metabolic stress during chemotherapy, placing them at high risk for malnutrition and muscle loss. Adequate protein intake is essential for maintaining muscle mass and functional capacity, yet many patients struggle to meet protein requirements without structured nutrition support. To evaluate the effects of individualized high-protein dietary counseling on nutritional and functional outcomes in adults with gastrointestinal cancers receiving chemotherapy. A systematic search of PubMed, Cochrane Library, and ScienceDirect identified interventional studies involving individualized dietary counseling aimed at improving protein intake. Eligible studies included adults with colorectal or gastric cancer undergoing chemotherapy or chemoradiotherapy and reported outcomes related to muscle mass, handgrip strength, or physical performance. Five studies met inclusion criteria and were synthesized narratively due to heterogeneity. All interventions led to improved energy and protein intake compared with usual care. Several studies reported significant improvements in nutritional status, PG-SGA scores, and serum albumin. One high-intensity intervention preserved skeletal muscle index and improved handgrip strength during adjuvant chemotherapy. Counseling also consistently enhanced quality of life, treatment tolerance, and symptom control. However, not all interventions prevented muscle loss, particularly in advanced disease or metastatic settings, highlighting the influence of disease burden and inflammation. Individualized dietary counseling exerts benefits through tailored symptom management, behavioral reinforcement, and sustained support for protein intake, even during periods of reduced appetite. These improvements may contribute to better clinical resilience and reduced treatment toxicity. Individualized high-protein dietary counseling is a valuable supportive care strategy for patients undergoing chemotherapy and should be integrated early in oncology practice.

Abstract

Background: Skipping Breakfast (SB) is an irregular eating habit with a prevalence of 24% to 87% worldwide, emerging evidence suggests that SB may disrupt gut motility, alter gut microbiota, and influence hormonal balance, which can lead to gastrointestinal (GI) symptoms such as dyspepsia, constipation, and abdominal pain. This study aims to investigate the connection between SB and GI symptoms. Method: We used SB to define the habit of not eating before 11:00 AM at least three times per week and was examined in this scoping review following PRISMA2020 guidelines. A systematic search across six databases (PubMed, Science Direct, Scopus, Wiley, ResearchGate, and ProQuest). Inclusion criteria comprised adult studies (2015–2025) on breakfast skipping and gastrointestinal symptoms. Results: Of 1,286 records screened, 8 met all criteria, with extracted data covering study characteristics, symptom types, and outcomes. SB is consistently linked to GI symptoms i.e.: functional constipation, abdominal pain, bloating. Associations SB with heartburn, gastric acid reflux, diarrhea, and dyspepsia were observed but less consistent. SB is common among young adults, particularly medical students during exam periods, and is associated with increased stress and a higher risk of functional constipation. Reduced meal frequency in SB with dyspepsia may also reduce acid-suppressive therapy effectiveness. However, most studies were cross-sectional and Asian-based, limiting generalizability. Conclusion: Consistent evidence links SB with various gastrointestinal symptoms, suggesting a preventive role for regular morning meals, though further research is needed to confirm causality and clarify underlying mechanisms. Abstract

Background: Cardiovascular disease (CVD) remains the leading cause of mortality worldwide. Hypertension is one of the most prevalent forms of CVD. Although systemic therapies have substantially advanced, adjunctive interventions are still needed to improve clinical outcomes. Green tea has been extensively investigated as a potential adjunctive therapy in the management of CVD. Objective: To evaluate the effect of green tea consumption on blood pressure. Methods: A systematic literature search was conducted using the PubMed, Cochrane Library, and Google Scholar databases, focusing on publications from 2020 to 2025. Eligible studies included randomized controlled trials (RCTs), systematic reviews, and meta-analyses involving adult patients with cardiovascular disease who consumed green tea. Study quality was assessed using the criteria of the Oxford Centre for Evidence-Based Medicine (CEBM). Results: Three studies met the inclusion criteria. The systematic review and meta-analysis of RCTs by Rezaei et al and Renfan et al. reported a significant effect on the improvement of both systolic and diastolic blood pressure. In contrast, the RCT by Pathania et al. demonstrated improvement only in systolic blood pressure, with no comparable effect on diastolic pressure in the intervention group. These studies reported inconsistent findings, with much of the available evidence derived from observational data lacking clearly defined dosages. Major limitations included small sample sizes, heterogeneous populations, and inconsistent outcome reporting Conclusion: Green tea consumption may reduce both systolic and diastolic blood pressure. However, stronger evidence from large-scale clinical trials is required to confirm its efficacy and support clinical implementation.

Abstract Background: Patients with head and neck cancer undergoing radiotherapy are at high risk of nutritional deterioration due to unavoidable oromotor side effects that reduce oral intake and lead to unintended weight loss, worsening treatment tolerance and outcomes, while proactive nutritional management may mitigate this risk as suggested by clinical trials, although findings remain variable. Objectives: This study aims to evaluate the role of proactive nutritional management in preventing unintended weight loss in patients with head and neck cancer undergoing radiotherapy. Methods: A literature search was conducted in PubMed, Scopus, and Cochrane databases. Articles were screened for duplicates, relevance, and eligibility criteria. Selected studies were critically appraised for validity, clinical importance, and applicability using the Centre for Evidence-Based Medicine (CEBM) worksheet from the University of Oxford. The level of evidence was determined to guide the grade of recommendation. Results: Two randomized controlled trials met the inclusion criteria. Both studies demonstrated good validity, clinical importance, and applicability, with a level of evidence of 1. Findings were consistent, indicating that proactive nutritional management initiated at the start of radiotherapy can prevent unintended weight loss. This effect persisted despite oromotor dysfunction during treatment. Overall, the evidence supports a strong recommendation (grade A) for implementation. Conclusion: Proactive nutritional management can effectively prevent unintended weight loss in patients with head and neck cancer undergoing radiotherapy.
