Abstract

Abstract Background: Late Onset Hypogonadism (LOH) is a disease associated with advancing age, characterized by symptoms and a deficiency in serum testosterone. It is important to choose which patient suitable for testosterone replacement therapy (TRT), but there is no one consensus that fits for all. After treating patients with testosterone replacement therapy (TRT), several parameters need to be evaluated. Case: A 74-years old male came with chief complaint of difficulty to maintain erection since 2 years ago.. PADAM questionnaire was positive and IIEF-5 score was 6. Physical examination showed an underweight condition. Total testosterone level was 3,65ng/mL, and patient chose to be given TRT instead of evaluating his free testosterone. PDE-5 inhibitor and non-pharmacologic treatment was also given. Follow-up showed that his erection was improved. Discussions: At which level should testosterone be substituted is still debatable. Several consensuses issued by several organization still cannot be used universally. Study in Indonesia showed that symptoms of LOH had been occur when the testosterone level still in normal range. After giving TRT to our patients, routine follow up is needed. Non pharmacologic treatment also needs to be addressed to improve the outcome. Conclusions: Symptoms of LOH had been occurred even though testosterone level is still in normal range. The consideration made by clinician is the most important thing to be evaluated, whether to give TRT or not. Somatic and laboratory parameters mentioned in this study is important to be evaluated.

Abstract

Background: Assisted Reproductive Technology (ART), such as intrauterine insemination (IUI), can be an option for infertile couples who are expecting a pregnancy. However, the success of IUI can be influenced by various factors, one of which is varicocele.   Objective: This study aims to determine the association between varicocele and IUI success. Methods: This study is an analytical cross-sectional study using patient medical records. The sample of this study was male patients aged 25-45 years with infertility and undergoing IUI in the period April to June 2021 at Rumah Sakit Ibu dan Anak Puri Bunda Denpasar. The sampling technique used, purposive sampling, was selected based on inclusion and exclusion criteria. Based on the criteria, this study selected 32 samples. Result: From 32 participants, 5 had varicocele (15.6%), and 27  were without varicocele (84.4%). In this study, we found 12 success pregnancy after IUI (37.5%), with 3 participants having varicocele (9.4%) and 9 participants without varicocele (28.1%). The chi-square analysis carried out obtained the results (p>0.05), and the calculation of the relative risk got the result (RR=0.600). Conclusion: This study showed no significant association between varicocele in infertile men and the success of IUI. The RR calculation showed that varicocele was not a risk factor for IUI failure.

Abstract

Introduction Over the past decades, infertility has become a serious health problem worldwide and until nowadays numerous studies have shown it can be a negative prognostic factor for infertility.1 During, natural intercourse, ejaculation of seminal fluid into vagina may deposit spermatozoa, contaminants, and bacteria into the uterus by pelvic or penile thrusting and belling of the glans penis during coitus. In that case, the risk factor of infection in both women and man will happen post natural intercourse. So, the relationship between endometritis and increasing white blood cells level in sperm with infertility such as repeated implantation failure and recurrent miscarriage has recently emerged as an area of inquiry. Aims Aims this study, Correlation of endometritis that found in hysteroscopy examination and increasing white blood cell level in semen in sub fertile couples. Methods This study was conducted from 492 Infertility couple who underwent hysteroscopy from January 2021 to December 2021. The semen analysis was performed at least twice with range two weeks one from the other based on WHO guideline. Hysteroscopy was performed during endometrial proliferation phase. All data analysis performed using IBM SPSS 23. Results The result showed that mean leucocyte level range difference was significant in patient non-endometritis was 0.25 and endometritis patient was 0.66. The leucocyte number had correlation with the result of the pH level and slow progressive motility in semen specimen. Conclusion This research result showed the correlation women with resulted endometritis in hysteroscopy and white blood cell in semen specimen and had correlation with pH level and slow progressive motility in semen specimen. Abstract

Background: A good erectile function and normal sexual arousal are indicators of male health. Impaired sexual function due to loss of libido and erectile dysfunction can interfere with quality of life and relationships with partners. Erectile dysfunction can be caused by vasculogenic, endocrinopathy, hormonal imbalance, neurogenic, trauma, iatrogenic and also due to psychological causes. This research to understand and overcome the problems related to sexual function in man with genetic abnormality. Case: A 32-year-old patient came for a sperm analysis examination because he was planning to get married soon. At first, there were no sexual problems complained of but the patient admitted that he was not too much interested in sexual matters since he was young. Nocturnal and morning erection happened rarely. Puberty at 15 years old and currently in a relationship with a woman and planning to get married. At this time libido was good. There was no history of mumps and orchitis. From the physical examination, it was found that the height was 180 cm, weight was 100 kg, and the arm span was 186 cm. waist circumference 104 cm. It means that he was obese. Other physical examinations were within normal limits. Examination of the genitalia showed testicle size of 4 cc right and 4 cc left and soft in consistency. Penis size 8 cm in stretched condition. Sperm analysis showed azoospermia. Cytogenetic examination showed 46XY inv(9)(p11q13) and no Klinefelter syndrome was found. Hormone examination results showed LH 11.92 mIU/ml, FSH 30.29 mIU/ml.He showed hyperprolactinemia 25.89 ng/ml, estradiol 16 pg/ml and total testosterone 2.33 ng/ml. HbA1c 5,9 %. Discussion: The patient was treated with cabergoline 0.25 mg 3 times a week and to overcome hormone imbalance ,we used letrozole 2.5 mg once a day for 2 months.After two months have passed, there is an increase in sexual arousal, morning erections occur at least 4 days a week, erections begin to improve. weight 94 kg, arm span 186 cm. waist circumference 99 cm. Other physical examinations were within normal limits. Examination of the genitalia showed testicle size of 5 cc right and 5 cc left and soft in consistency.Recent investigations showed LH 7.89 mIU/ml, FSH 14.53 mIU/ml, prolactin 15.00 ng/ml, estradiol 10 pg/ml and total testosterone 1.69 ng/ml. Until now, evaluation and treatment is still ongoing. Conclusion: Management of erectile dysfunction can not only be seen from one side but also requires a holistic and multidisciplinary approach. Risk factors that can cause erectile dysfunction problems must be assessed and controlled every single cause. The goal of erectile dysfunction management is not only for sexual satisfaction but also to improve fertility and quality of life.

Abstract

The decrease in erectile function performance is the inability to sustain an erection long enough during intercourse (NIH Consensus Conference, 1993). The etiology of decrease in erectile function performance is multifactorial; organic factors, and psychogenic factors involved in the ability to get and maintain an erection (Agarwal, 2006). The decrease in erectile function performance in this study was measured using IIEF-5 questionnaire. This study is a cross-sectional observative research that use the results of the questionnaire answers of the respondents. Obtained a population of 161 people who were then screened to 109 people who fit the criteria of this research by using insidential sampling method. The results of this study showed a decrease in erectile function performance of 56.88% of the total respondents. Correlation test results showed a significant association of each variable age, duration of smoking, number of cigarettes per day, diabetes, dyslipidemia, and hypertension with a decrease in erectile function performance. Then for Body Mass Index (BMI) in this study did not show any significant relationship with decreased in erectile function performance. This is because the respondents in this study consist only of normal wight (18.5 to 24.9) and over weight (25 to 29.9). Advice given to avoid decrease in erectile function performance is to reduce cigarette consumption, and improve or treat an unhealthy condition such as BMI above normal, Diabetes, Dyslipidemia and hypertension. This study only retrieve data from the questionnaire so that the data are less and incomplete clear risk factors.

Abstract

Examination of semen / ejaculates / sperm is the first screening examination that a subfertile / infertile couple needs to undertake in an attempt to find out the cause of infertility that may come from his side. Besides spermanalysis, it is also necessary to do physical examination, especially about internal and external genital tool because as it is known abnormal result of sperm analysis can be caused by anatomical abnormalities and or testis function. To determine the relationship of testicular volume to semen analysis on male patients in Andrology clinic RSUD Dr. Soetomo Surabaya. The research is a kind of observational analytic research to find out the correlation of testicular volume to semen analysis on male patients in Andrology clinic RSUD Dr. Soetomo Surabaya by using secondary data. Based on the results of statistical tests of 64 samples by using chi square test can be seen that the value of significance or p-value 0.005 <0.05. So it can be concluded that there is a relationship between testicular volume with morphology, motility and sperm concentration. Data were analyzed using chi square test to determine the relationship between testicular volume and semen analysis. Testicular volume is one of the risk factors of decreased semen analysis results
