Abstract

Latar Belakang Alokasi dana besar pada persediaan berpotensi menimbulkan idle money, dan biaya yang tidak sedikit untuk pemesanan dan penyimpanan, serta risiko kerugian untuk setiap inventory yang kedaluwarsa. Di sisi lain, kondisi stock out berdampak pada hilangnya pendapatan, menyebabkan lamanya waktu tunggu pelayanan, serta dapat membawa konsekuensi serius pada keselamatan pasien. Di RS Pusat Pertamina, persediaan mengambil porsi 20% dari struktur biaya rumah sakit. Inventory turn over ratio di semester pertama tahun 2022 berkisar pada 1.13-1.60, kemudian bergerak menjadi 1.8-1.98 pada tengah tahun kedua. Angka kekosongan obat obat kurang dari 1% karena pencatatan yang belum memadai. Implementasi lean management diharapkan dapat menjadi solusi terhadap berbagai masalah tersebut. Metode Penelitian ini adalah penelitian kuantitatif dan kualitatif dengan desain action research, yang dilakukan di RS Pusat Pertamina, dengan mengintervensi seluruh data jumlah pemesanan obat, jumlah obat terjual, harga beli rata-rata, dan lead time periode Januari s.d Maret 2023 (pre intervensi) dan periode 2 Mei s.d 23 Juni 2023 (post intervensi). Data yang tidak lengkap dieksklusi. Focused group discussion dilakukan dengan staf Instalasi Farmasi yang terlibat dalam proses pengadaan obat. Hasil Teridentifikasi kecenderungan peningkatan inventory value pasca intervensi di PDSA siklus kedua sebesar 16,11%. Hal ini diikuti dengan data perputaran persediaan yang justru mengalami perbaikan menjadi 2.6 di bulan Mei. Efektivitas proses (VAR) keseluruhan sebelum lean 5%, di akhir siklus PDSA kedua 4%, dan di akhir siklus PDSA ketiga 8%. Pada sub proses evaluasi kebutuhan, di awal pengukuran diperoleh VAR 82%, lalu di akhir siklus PDSA kedua menjadi 75%, dan 91% di akhir siklus PDSA ketiga. Kesimpulan Implementasi lean management meningkatkan inventory value namun berhasil menurunkan insiden kekosongan obat, dengan perputaran persediaan yang baik. Efisiensi waktu proses teridentifikasi dari peningkatan nilai VAR, walaupun belum dapat dilihat kemaknaannya secara statistik karena keterbatasan penelitian. Abstract

Daerah Istimewa Yogyakarta menempati persentase kelahiran melalui tindakan sectio caesarea yang cukup tinggi. Peran faskes primer menjadi penting khususnya mendukung ibu hamil tanpa faktor resiko untuk melakukan persalinan normal. Klinik yang akan bertahan di era kompetisi butuh model pelayanan yang berfokus kepada pasien (patient center care). Keunggulan pelayanan klinik terletak pada keunikan serta kualitas seperti apa yang diberikan. Faktor yang paling penting dalam penguatan layanan primer adalah kompetensi SDM bidan sebagai garda terdepan dalam memberikan pelayanan kesehatan ibu dan anak. Bidan dituntut tidak hanya mempunyai kemampuan skill klinis yang baik tetapi asuhan non klinis seperti sikap, kepedulian, pendekatan woman center care untuk mensupport dan membangun persepsi positif tentang persalinan normal menjadi penting guna meningkatkan mutu pelayanan kesehatan. Bagaimana peran bidan bisa menjadi agent of change di faskes primer untuk memberikan pengalaman positif ibu bersalin. Abstract

Background: Cesarean section (SC) is one of the most performed surgical procedures in the world, as well as in Bethesda Lempuyangwangi Hospital (RSBL). This action is included in the high volume and high-cost category, so a guide is needed to reduce variations in patient care. Clinical pathway (CP) is a tool to improve the process of preparation, management, and clinical outcomes of patients. RSBL has developed a CP for SC since 2019, but there has never been an evaluation of the benefits of its implementation. Objectives: To evaluate the benefits of SC CP implementation on laboratory test compliance, prophylactic antibiotic prescribing compliance, incidence of surgical wound infection, length of stay, and APGAR score. Methods: This was an observational analytic study with an ex-post facto research design. The subjects of this study were all patients undergoing elective SC surgery at RSBL who met the inclusion criteria in the period before and after the implementation of SC CP. The variables evaluated were compliance with laboratory tests, compliance with prophylactic antibiotic prescribing, incidence of surgical wound infection, length of stay, and APGAR score. Results: Compliance of laboratory tests increased from 6.7% to 100%, compliance of prophylactic antibiotic prescribing increased from 3.4% to 84.3%, the incidence of surgical wound infection decreased from 1.10% to 0.0%, and variation in the length of stay decreased from 2-4 days to 3 days (100%). The mean first-minute APGAR score did not change (7), while the 5th and 10th-minute scores decreased from 9 to 8, and 10 to 9, respectively. Conclusions: The implementation of CP SC was beneficial in improving laboratory testing compliance and prophylactic antibiotic prescribing compliance, reducing the incidence of surgical wound infection, and reducing variation in the length of stay, but could not improve the APGAR score. Abstract

RSUD Bagas Waras Klaten has some potential wastes of process which leads to the need of lean management to improve service quality and efficiency. Although implementation of lean management has been proven in improving quality, patient safety, effectivity, and cost efficiency, it is also potential to fail if some readiness elements are not met. Lean management readiness assessment is required to describe inhibiting as well as supporting elements in implementing lean management.

Abstract

ABSTRACT Background: Readmission is often regarded as an indicator of poor quality of care and is used to assess hospital performance. The readmission rate also reflects the hospital's response to the shift in payment systems following the implementation of BPJS as Indonesia's national health insurance. In 2023, RSUP Persahabatan reported a readmission rate index (RaR) of 12%. Objectives: This study aims to analyze the factors affecting readmission events among BPJS inpatients in 2023 at RSUP Persahabatan and to formulate strategies to reduce the readmission rate. Methods: This research employed a mixed-methods sequential explanatory design. Quantitative analysis was conducted on BPJS inpatient records from 2023 for patients aged 18 years or older who experienced readmission. Data sources included BPJS claims and electronic medical records. Qualitative analysis was conducted through focus group discussions. Quantitative data were analyzed using chi-square tests and multivariate logistic regression. Results: Significant relationships were found between readmission and variables such as comorbidity, Charlson Comorbidity Index (CCI), severity level, polypharmacy, length of stay, admission through the emergency department, history of ED visits and inpatient care, and alignment with the clinical pathway (p<0.05). Patients who did not receive care aligned with the clinical pathway had a five-fold increased risk of readmission. Strategies to reduce readmission involve three stages: during admission, while the patient is still in care, and after discharge. Conclusions: Readmission at RSUP Persahabatan is prevalent among chronic patients with severe comorbidities. Adherence to the clinical pathway is crucial for reducing readmission rates and is a reflection of service quality. Efforts to reduce readmission must involve management and clinicians, requiring the integration of technology and a commitment to comprehensive patient care.
