Abstract

This study contributes to solving problems of steady increase in cholelithiasis incidence with high percentage of associated complications, high mortality rate and unsatisfactory outcomes. This study rises from universally recorded increase in the incidence of gallstones, from the prevalence of complicated disease forms, from high mortality rate and unsatisfactory treatment outcomes. This study aims to improve the outcomes of surgical treatment in patients with complicated cholelithiasis who underwent a differentiated surgical intervention. This study analyses treatment outcomes in 140 patients who undergone surgery for cholelithiasis complicated by choledocholithiasis and obstructive jaundice. Patients underwent surgical interventions in a clinical unit under the West Kazakhstan Marat Ospanov State Medical University in 2014/2018. The findings show that a surgical approach towards the treatment of patients via minimally invasive surgery should depend on the severity of obstructive jaundice. Postoperative complications were found in 7.8% of patients in the main group, and the death rate was 1.1%. By contrast, in the control group, 32% had postoperative complications and the death rate was 4%. If differentiated, minimally invasive surgical treatment of cholelithiasis complicated by choledocholithiasis and obstructive jaundice allows avoiding serious complications when correcting abnormalities in the biliary system via traditional laparotomy with a wide incision. These findings suggest that a differentiated approach can significantly improve treatment outcomes in patients with cholelithiasis complicated by choleducholithiasis and obstructive jaundice.Keywords: Cholelithiasis, Choledocholithiasis, Obstructive jaundice, Endoscopic interventions.

Abstract

Currently, the number of requests for orthodontic care has increased significantly. This is facilitated by a number of factors -the technical capabilities of orthodontists, which have grown many times due to technological progress and the ability to provide the necessary assistance at almost any age; the need for an aesthetic result, which is formed under the influence of the media. As a result, there are high requirements for both yourself and your appearance, as well as for the orthodontist. Thanks to new technologies, such as cortical support, it became possible to solve the treatment of dental anomalies, which previously could only be cured by surgery. But at the same time, such an important issue as the retention of the results achieved remains open. We studied the objective parameters that changed as a result of orthodontic treatment (teleradiography orthopantomography, analysis of control and diagnostic models), and compared them with the subjective assessment of patients. We have found that the patient's motivation, desire to cooperate with the doctor, a clear understanding of the treatment goals allows you to get a high-quality result and keep it for a long enough period of time. Abstract

Background: The most significant occupational injuries among nurses are associated with biological agents which include multiple underlying cases. Therefore, the design and implementation of educational programs with operational objectives and based on needs assessment on prevention of occupational risks can be increasingly effective on maintain and improvement of nursing staff health. Methods: Statistical population of this cross-sectional study includes Kermanshah Ayatollah Taleghani Hospital nurses and a sample of 70 nurses participated in study. Data collection tools included demographic information questionnaires and 10 items related to prevent nursing occupational hazards biological agents. When responses were evaluated, the collected data were analyzed using SPSS18 software, independent t, Spearman and Pearson tests and results were presented in terms of statistical indicators. Results: 30% of participants in study were nurse's aide and 70% were nurses. Two items are introduced as the first priority of educational needs including familiarity with transmissible diseases in wards or hospital and care oneself during disease, injury or skin lesions of hands until complete recovery. In most cases, there was statistically significant relationship between variables and items. Conclusion: Nurses' knowledge on safety rules is more important in prevention of occupational hazards and as a result, application of these principles. In this regard, considering biological agents has been focused and needs assessment of educational planning will be effective on increased participation of nurses in teaching and learning process and will promote their knowledge and skills.Keywords: Nurses, Nurse's aide, Occupational hazards, Biological agents.

Abstract

One of the indicators of efficiency of administrative system in developed countries is improving the quality of services offered to the citizens. One of the appreciable indices of public satisfaction is customer’s satisfaction with the delivery of services by the same organizations.This research’s design is to fulfil the intentions of the companies, which is to obtain the relative satisfaction and to receive valuable feedbacks of subscribers. The methodology of this study is surveyand is conducted as an applied research. The population of this study consisted of all households covered by the gas company in Isfahan province, which is equal to 191152 households. Sampling in this study is random cluster sampling and questionnaire and library resources are two major methods which are used to gather information and data. Statistical methods and data analysis methods to measure customer satisfaction are CSM,descriptive and inferential statistics using SPSS, and performing adaptive comparisons of the results obtained fromthe homes and offices and results of honoring client. Some of the main questions raised in this research are: optimum level of service received from Isfahan Province Gas Company and the average satisfaction with the service received? Time span of receiving service of Isfahan Province Gas Company (in minutes or days) and consistence with the standard time to service delivery? After the necessary calculations results were as follows: The results showed that in the majority of cases, the provision of services by the gas company is proportional to the standard time. Due to the lack of familiarity with the standards of work time, in most cases it is possible that the services provided in standard time be realized, but the customer satisfaction with the service received is still not so high.Keywords: Satisfaction, service recipients, Isfahan Province Gas Company, CSM.

Abstract

Dox, is still widely used in modern cancer treatments for different type of malignancy despite the advent of targeted therapy. However, its beneficial effect was limited by its toxicity on various organs. The objective of this study was to investigate the hepatoprotective effect of menaquinone-7 against hepatotoxicity induced by doxorubicin in rats. Sixty adult rats of both sexes were used in this study; the animals were randomly enrolled into six groups of 10 animals each. Group I: negative control; Group II: Menaquinones-7 at a dose of 16µg/kg; Group III: Menaquinones-7 at a dose of 48µg/kg; Group IV: positive control (Doxorubicin 15mg/kg); Group V: Menaquinones-7 at a dose of 16µg/kg administered prior to a single dose of Doxorubicin 15 mg/kg; Group VI: Menaquinones-7 at a dose of 48 µg/kg administered prior to a single dose of  Doxorubicin 15 mg/kg. On day twelve of the study, the liver of each animal was excised for homogenate preparation and estimation of malondialdehyde, total antioxidant capacity and caspase-3 by ELISA technique as the markers of oxidative stress and apoptosis respectively. High dose of Menaquinones-7 significantly (P<0.05) decreased malondialdehyde content, and increase total antioxidant capacity content in group VI compared to group IV. However, neither group V nor group VI exhibited significant (P>0.05) differences in caspase-3 activity in liver tissue homogenate compared to positive control group. Menaquinones-7 may have hepatoprotective effect against doxorubicin-associated hepatotoxicity in rats.Keywords: Menaquinone-7, Doxorubicin, Hepatotoxicity, Rats, Tissue homogenate.

Abstract

Schiff  Bases (SB)  stand for  multipurpose  ligands  created  from  prime  amines  condensing  with carbonyl groups. These represent imperative compounds in pharmaceutical and medical domains as a consequence of their widespread spectra of organic performances. The majority of them exhibit antibacterial, antifungal and antitumor activities. Transition metal compounds based on biological SB ligands were extensively investigated. Abstract

This paper portrays a recently developed, optimized and validated isocratic RP-HPLC strategy for the separation of two anti-diabetic drugs Vildagliptin (VLD) and Metformin (MET).The optimization of the HPLC methodology included several variables, of which the impacts of each was studied. After a sequence of preliminary-screening experiments, the composition of the mobile phase and the pH of the added buffer solution were set as the examined variables, while the capacity factor (K1) of first peak i.e Metformin (MET), resolution (Rs) between Vildagliptin (VLD) and Metformin (MET), Separation (S) for both the drugs and the retention time (Rt2) of the second peak i.e Vildagliptin (VLD) were chosen as the dependent variables. Using the D-optimal design, the ideal chromatographic conditions for the separation were identified. The technique proved to show good harmony between the experimental data and predictive values throughout the studied parameter range. The optimum assay conditions were carried out with a Onyx C18 Monolithic column (100mm× i.d., 5µm) connected with an Onyx C18 guard cartridge (4mm×3mm i.d., 5µm) and a mixture of methanol (MeOH)/acetonitrile (ACN)/potassium dihydrogen orthophosphate (KH2PO4) (pH 4, 0.003 mol L−1) (31.362:10:58.638, v/v/v/v) as the mobile phase at a flow rate of 0.4 mL min−1 and detection wavelength of 220nm. The run time was under 8 min, which is a lot shorter than the prior optimized methods. The improved analytical method was validated as per FDA guidelines.

Abstract

Background: Seroma remains an unresolved problem Post mastectomy surgery and several techniques have been reported to prevent or decrease this complication. The aim of this study is to evaluate effectiveness of Quilting or tacking mastectomy flap in preventing or reducing seroma formation.  Patients and Method: This hospital-based prospective cohort study included patients with diagnosed breast cancer between December 2010 and December 2016. Mastectomy with axillary clearance surgery was done for them. Socio-demographic Characteristics which include age, BMI, smoking history and family history of breast cancer were taken for all patients. Data were collected on the day of surgery and followed for two weeks postoperatively. Patients were divided in two groups according to the technique of closure of mastectomy flap . Non quilting technique was done to the first group of patients between December 2010 till January 2014 and the quilting techniques were done for second group of patients between February 2014 till December  2016. Primary outcome were the presence of Seroma complication and the amount of fluid collection that require aspiration. Secondary outcome were the mean differences of fluid drained by the drainage system, time to remove drain, stage of disease, level of axillary clearance, distribution of patients according to type of malignancy, Oestrogen receptor, progesterone receptor and Her2 /Neu receptor status. Result: During the study period between December 2010 and December 2016, 82 Patients with diagnosed breast cancer were included in this study. Majority (84.1%) of patients presented with Invasive ductal carcinoma. There was significant association between type of operation and  presence of seroma, more seroma occur among patients treated with Non quilting technique (46.8%) in comparison to quilting technique (20%)  (P- value 0.012) and more time needed to remove drain among patients treated with Non quilting technique (with mean 5.74 days) in comparison to quilting technique (with mean 4.74 days). There were significant differences between means of amount of aspiration among patients with seroma (n=29) by study groups, more amount of aspiration among patients treated with Non quilting technique (with mean 144.54 ml) in comparison to quilting technique (with mean 60.71 ml). There was significant association between presence of seroma and BMI (P- value <0.001) and level of clearance (P-value 0.021) and no association between presence of seroma and Smoking, family history and stage of disease. Conclusion: Quilting or tacking mastectomy flap to the underlying muscles and fascia to obliterate the surgical dead space is effectiveness in prevention and reduction of seroma formation.Keywords: Breast cancer, Mastectomy flap, Quilting technique.

Abstract

Background: Increasing production of free radicals is one of the factors that cause hyperglycemia. Chronic hyperglycemia can increase the production of reactive oxygen species (ROS), resulting in oxidative stress. The aim of this study is to determine the effectiveness of binahong (Anredera cordifolia (ten) steenis) leaf extract in fixing the pancreas β-cell damage rate in alloxan induced hyperglycemic Wistar rat. Methods: This study used randomized pre and post-test control group design. Fifty Wistar rats were divided into two control groups and three experimental groups administered with difference doses of binahong leaves extract; 0.5 mg/kg BW (P1), 2.0 mg/kg BW (P2), and 5.0 mg/kg BW (P3). Hyperglycemic state obtained by inducing the rats with alloxan. Results: The result showed that mean blood glucose levels were decreased in the positive control group (glibenclamide) (185.98 mg/dL) and the experimental groups; P1, P2, and P3 (181.36 mg/dL, 184.61 mg/dL, and 163.36 mg/dL, respectively). The mean differences between P1 and P2 and between P1 and P3 were statistically significant (p < 0.05). The AGEs level for the positive control group (glibenclamide) is 0.224 mol/L and for the experimental groups are 0.219 mol/L, 0.212 mol/L, and 0.188 mol/L for P1, P2, and P3, respectively. The differences were statistically significant. Conclusions: Administration of binahong leaves extract can improve the damage rate of pancreas β-cell through lowering the blood glucose and AGEs level in hyperglycemic Wistar rat and that the effect is significant at the dose of 5.00 mg/kg bw per day.Keywords: Anredera cordiofolia (ten) steenin, blood glucose and AGEs, hyperglycemia.

Abstract

Introduction: Probiotics have been used widely in medical world, but its use in infertility, especially in male infertility, is yet still unbeknown. The aim of this paper is to review the usefulness of probiotics in enhancing sperm quality in order to treat male infertility. Methods: Narrative review on literature surfing in the Pub med database, as well as  key words, that is probiotic, Lactobacillus, sperm quality, male infertility, sperm motility, sperm concentration, sperm morphology, sperm DNA fragmentation. Results: The results on literature surfing are mainly yielding to the positive effect of probiotics in enhancing sperm quality in animals, and in human, although there are some literature that shows conversely. The production of Gamma-Amino Butyric Acid and antioxidants by the probiotics are thought to be the main mechanism for the improvement of the sperm quality, especially in the enhancement of the sperm concentration, motility and the decrease of the DNA fragmentation. Contrarily, negative results left another mystery for researchers to solve. Conclusions: Although the probable mechanisms of probiotics role in enhancing sperm quality have been unveiled, still more researches should be performed to find more evidence in this issue.Keywords: Probiotic, Sperm quality, Sperm motility, Sperm concentration.
