Abstract

Objective lower level of "adiponectin" were observed to be related with obesity, type 2" diabetes" and "CAD" because of the significance of it in this field this consideration was assigned to the role of  "adiponectin" on rate ofT2DM crosswise over classes of BMI, , baseline  glucose and insulin. We likewise proposed to contemplate the relationship of "adiponectin" with rate of incidence of myocardial infarction (MI) regardless the BMI and other traditional risk factors. Subjects and methods: This case-control think about was completed in patients took care of Marjan teaching hospital in Babylon province in Iraq from September 2016 until January 2017. The contemporary investigation was incorporated 54 patients, among of them. 27patients experiencing T2DM and 27 patients experiencing MI contrasted with 25 healthy volunteers of a similar age and sex. Blood sample was taken for evaluation of "adiponectin", and insulin resistance by Elisa technique. Totally the patients experienced a therapeutic checkup and estimated their BMI.Results: the primary consequences of contemporary examination were revealed a critical reduction in the sera level of "adiponectin" in patients with T2DM paralleled with those of control group(P<0.05) . What’s more, a very noteworthy increment in the sera level of HOMA-IR in patients with T2DM contrasted and those of control group (P<0.01). Additionally present outcomes demonstrated reduction "adiponectin" level significantly (P<0•04) contrast with control group• notwithstanding that present result did not demonstrate any significant correlation between "adiponectin" and BMI in MI patients. Conclusions Present examinations suggested that, "adiponectin" consider as predictor for occurrence of T2DM based on the increase BMI that lead to development of insulin resistance, also contemporary study showed that "adiponectin" was linked with MI incidence with no association presented with BMI, sex, age and other CAD risk factors.

Abstract

To reduce maternal mortality due to the relay referral system, initial doses of Sulfas Magnesium (SM) therapy have been established in primary care before being referred, but in practice, magnesium sulfate drugs are often not available at the primary care level or are not used according to guidelines. This study aims to explore the management of emergency preeclampsia-eclampsia in primary services in Surabaya. This research was conducted in 5 primary health services in Surabaya. The sample in this study was health workers who carried out emergency management of the preeclampsia and were selected by disaproportionate stratified random sampling, which consisted of doctors, midwives, and nurses. Data were analyzed using Partial Least Square (PLS). The results showed that organizational characteristics and individual characteristics significantly affected the management of emergency preeclampsia-eclampsia. The magnitude of the coefficient estimate for the characteristics of the officers for the management of emergency preeclampsia-eclampsia was 0.277669 and for the management of emergency preeclampsia / eclampsia was 0.284305. The coefficient was positive, meaning that the better the characteristics of the organization and health workers, the higher the accuracy of the management of emergency preeclampsia. The results of the study indicated that primary health services in Surabaya need to increase the provision of facilities and infrastructure as well as drugs for emergency management of preeclampsia and eclampsia, for health workers it is advisable to increase knowledge especially about the administration of initial doses of magnesium sulfate in preeclampsia-eclampsia patients before being referred. Abstract

Underground spaces are frequently applied in defense areas due to their natural and environmental conditions. Passive defense is a good tool for reducing the possible losses and increasing the efficiency of these structures. By studying the past events, it can be observed that, despite the popular belief, these structures have incurred irreparable damages during the earthquake. To this end, the present paper evaluated the seismic behavior of two specimens of rectangular and oval tunnels applied in passive defense using the plaxis 2D software. These structures were placed under three particular earthquake accelerographs in a dense and loose soil (soil environment of Bandar Abbas), and it was attempted to study the seismic behavior of these tunnels affected by the form of the structure and the soil-structure interaction using this dynamic analysis. The analysis of results showed that the earthquake had a greater impact on rectangular tunnels in loose soil, and the construction of oval tunnels will be more justified in the environment of Bandar Abbas. Keywords: Underground spaces, Passive defense, Earthquake, Soil-structure interaction.

Thyroid hormones play vital roles in maintaining energy physiological state and regulation energy expenditure. Their physical effects, mediate at multiple target organs, primarily to stimulate cell metabolism and activity. Thyroid illness may be a medical condition that affects the perform of the ductless gland (the endocrine organ found at the front of the neck that produces thyroid hormones). The symptoms of thyroid illness vary reckoning on the sort of Thyriodism. In individuals with gland disease, the thyroid doesn't build enough endocrine to stay the body running usually. Common causes of hypothyroidism, autoimmune disorder, surgical removal of the thyroid, and radiation treatment.  Patients typically have enlargement of the ductless gland (goiter) and either traditional (euthyroid) or gently attenuated (hypothyroid) metabolic state. The T3 , T4 , and thyrotrophic hormone levels is also high in what seems to be a counteractive  mechanism to confirm maintenance of euthyroid and eumetabolic states  The Synapsin one (SYN 1) may be a member within which that job as neural protein, that interface with the living substance surface of conjunction vesicles.
