Abstract

Introduction: Atopic dermatitis is a relapsing chronic inflammatory skin disease, 15-20% occurring in children. Heredity in the family is the most crucial risk factor for the incidence of atopic dermatitis. Therefore, improving the barrier function of the skin as early as possible in high-risk infants may be an option as primary prevention for atopic dermatitis. Objective: Conducting a systematic review of randomized controlled trials (RCT) that test the effects of giving moisturizers to high-risk infants and the incidence of infantile atopic dermatitis. Methods: A literature search was carried out through three electronic databases: PubMed, Cochrane, and Embase. Those articles were critically reviewed and assessed for the risk of bias using the Cochrane Risk of Bias Tools. Results: After a critical review of the full text, there were 4 RCT articles. Those four articles found a similar outcome: the incidence of atopic dermatitis was lower in the intervention group. Discussion: The link between heredity and the incidence of atopic dermatitis in children forms the basis of inclusion criteria for samples in this systematic review. Moisturizer administration has been shown to improve TEWL values and stratum corneum hydration. Conclusion: The use of moisturizers in high-risk infants may be considered as one of the preventive interventions for atopic dermatitis. However, further research is needed to support this hypothesis.

Abstract

Pruritus and xerotic skin often accompany patients with chronic kidney disease (CKD) and have no clear pathophysiology. Hypercalcemia, hyperphosphatemia, and uremia might play a role in the development of pruritus. A descriptive study with a cross-sectional study design was conducted on 39 patients at the Hemodialysis Unit of Atma Jaya Hospital in August 2019. The degrees of pruritus and xerosis were assessed by the 5-D Pruritus Scale and Overall Dry Skin score (ODS). Blood urea, creatinine, calcium, and phosphorus levels were examined to determine their relationship to pruritus and xerosis using linear regression. All patients in this study had xerotic skin, with an incidence of pruritus of 46.2%. A decline in creatinine level was associated with a higher pruritus score. It indicated that a decrease in creatinine level was associated with an increase in pruritus scores (β = -0.658; p <0.05). There was a significant positive association between phosphorus levels with pruritus and xerosis scores (β = 1.44; p <0.05; β = 0.282; p <0.05). We did not find any significant relationship between ureum and calcium with the degree of pruritus and xerosis.

Abstract

Erythema multiforme (EM) is a rare cutaneous or mucocutaneous eruption, characterized by target lesion. EM is caused by hypersensitivity reaction to various infections and drugs, including varicella and acetaminophen. A 23-year-old woman had typical target lesions preceded by varicella 6 days before. Varicella’s blister dried-up after valacyclovir, acetaminophen, and multivitamin were given. On the fourth day of varicella, hives arose and on the sixth day, typical EM lesion arose. Complete blood count was within normal limit, except slight eosinophilia. Patient was diagnosed as EM minor caused by drug eruption with differential diagnosis EM minor caused by varicella and Degos disease. Improvement was found after therapy of methylprednisolone, desloratadine, and moisturizing lotion. EM occurred 6 days after patient had varicella and consumed drugs. Drug eruption was suspected as the culprit because varicella lesions had improved with valacyclovir and there were hives preceding the target lesions. Among the drugs used, acetaminophen was suspected as the cause. Serologic testing for varicella and patch test or oral provocation test for drug allergy must be considered to determine the definitive diagnosis.

Abstract

Azadirachta indica (neem) and Hypericum perforatum formulation have been used for centuries in traditional medicine. Neem oil contains diterpenoid and triterpenoid, while Hypericum perforatum flower oil extract is quite popular as a treatment for excoriations, wounds, and bruises. Both of these plant extracts have bacteriostatic, antifungal, immunomodulatory, and antiinflammatory effects, and also safe to use as an external treatment for wounds. Several studies have shown the benefit of this formulation for diabetic foot ulcer. A 41- year-old woman had a wound on her right leg that had not been healed for the past two months. Dermatological examination showed an ulcer with an irregular border, measuring 7.5x3.4x0.3 on the lateral region of the right foot. The ulcer’s edge was elevated, while the wall was gently sloping. The surrounding skin was unaffected. The base of the ulcer consisted of granulation tissue and was clean, without any pus formation. The ulcer was painless and odorless. Laboratory tests showed hyperglycemia and hypercholesterolemia. Doppler ultrasound examination showed deep vein thrombosis in the right leg.A topical formulation containing Azadirachta indica and Hypericum perforatum was given. After 49 days of therapy, diabetic ulcer wounds had healed completely.

Abstract

Background: Scabies is a dermatologic disease caused by Sarcoptes scabiei var hominis infestation. It is commonly found in developing country with high population density. There are currently several available treatments for scabies. In Indonesia, there are two treatments provided by goverment which are 5% permethrin cream and 2-4 ointment (2% salicylid acid + 4% sulphur). Permethrin is the first line treatment for scabies. The aim of this evidence based case report is to compare the efficacy of topical sulphur with permethrin for treatment of scabies. Method: Systematic literature searching was conducted on Pubmed, Cochrane, Sciencedirect, EBSCO, Proquest and Scopus using relevant keywords. Studies comparing topical sulphur with permethrin in scabies patients were included in this study. The included studies were critically appraised to give recommendation on scabies treatment. Result: From screening of 11 fulltext articles, three articles were suitable for this studies which consist of one meta-analysis and two randomized controlled trial. Discussion: All the articles included showed permethrin has better efficacy compared to topical sulphur in treating scabies. Treatment with permethrin had higher cure rates with less side effect.Conclusion: Based on current best evidence, topical sulphur is inferior compared to permethrin in treating scabies, therefor topical sulphur should not be chosen unless permethrin can not be used.

Abstract

Morbus Hansen is one of the great imitator skin diseases. The destruction of nasal cartilage, which may lead to nasal septum perforation and saddle nose, can be seen in lepromatous leprosy. Despite its rarity, the acute exacerbation of systemic lupus erythematosus (SLE) can also lead to nasal septum perforation. A 47- year-old female, presented with a 7-year history of prurituc erythematous macules on the scalp, face, chest, back, and both arms. There was also an area of baldness on the scalp. Those lesions enlarged and increased in numbers gradually. There were cicatricial alopecia, moon face, saddle nose, and nasal septum perforation, without any peripheral nerves thickening. Slit skin smear examination revealed no acid-fast bacilli . Anti- nuclear antibody, anti-dsDNA, and anti-nucleosome were reactive. The histopathological findings and direct immunofluorescence result supported the diagnosis of chronic cutaneous lupus erythematosus (CCLE). The patient was advised to avoid excessive sun exposure and apply sunscreen regularly. She was treated with topical corticosteroid, methylprednisolone, chloroquine, and azathioprine. The involvement of nasal mucosa may be ignored and need more attention in assessing patients with SLE.

Abstract

Neurofibromatosis type 1 (NF-1) is a genetic disorder with an autosomal dominant inheritance pattern. It is caused by heterozygous mutations of the NF-1 gene located within the chromosome 17q11.2. Plexiform neurofibroma is one of several clinical manifestations that can be found in NF-1 cases. A 10-year-old girl was referred to the Dermatovenereology Clinic of Sanglah Hospital with the suspicion of Becker’s nevus. From the alloanamnesis, there were multiple blackish-brown spots in the left back since she was six months of age. Those spots have begun to thicken and protrude during the past two months. The brown spots appeared and spread into other parts of the body since the patient was four years old. Another complaint from the patient was back pain, starting at the age of 8. Histopathological examination supported the diagnosis of neurofibroma. No specific therapy was given in this case. The patient was then consulted to the orthopedic surgery department for her scoliosis. Eventually, the patient is planned to be consulted to the ophthalmology, pediatric, and neurology department for more comprehensive management.

Abstract

Cutaneous lichen amyloidosis is a subtype of primary localized cutaneus amyloidosis developed as a result of amyloid mass deposition in dermal papilla. It is usually found without organ involvement. The main characteristic of primary localized cutaneous amyloidosis is the discrete pruritic, hyperkeratotic, and hyperpigmented papules,. This is a case report of 35 years old female, presented with pruritic hyperpigmented papules, hyperkeratosis on her right and left limbs, since 5 years ago. Histopathology examination revealed lichen amyloidosis. She was treated with a combination of topical treatment which gave a good result. Cutaneous lichen amyloidosis is a rare entity, and the treatment for this condition is still a challenge for clinicians due to its persistence and recurrency. The combination potent topical corticosteroid, 6% salicylic acid mixed and diluted spirits, was applied twice a day. The treatment was resumed for 3 months, where it was effective for regressing and clearing the lesion.

Abstract

Epidermodysplasia verruciformis (EV) is a rare genodermatosis characterized by increased susceptibility to β-human papilloma virus (HPV) infection, especially HPV type -5 and -8 and risk of malignant transformation. This condition can be inherited in an autosomal recessive manner, as well as an acquired condition. Skin lesions develop early in childhood, presenting as flat warts, seborrheic keratosis-like, or pityriasis versicolor-like.This report described a 58 year-old man with history of warts since childhood. There was no history of parental consanguinity and immunosuppression. Skin lesions are persistent and progressive, manifests as multiple hyperpigmented plaques resembling seborrheic keratosis, some of them has verrucous surfaces and hypopigmented. Histopathology examination showed hyperkeratosis and coilocyte. Treatement consists of topical retinoic acid and sun avoidance. Diagnosis of EV can be made easily based on the history, physical examination and skin biopsy. HPV serotype testing can help determine the associated EV-HPV type. Several therapeutic modalities have been tried including topical and systemic retinoids, interferon, immunotherapy, electrodesication, cryotherapy and surgery. However, definitive therapy is not yet available. Early diagnosis, patient education, protection against sunlight and regular monitoring for malignant transformation are important to improve patient’s quality of life.

Abstract

First line therapy for mild to moderate plaque psoriasis is topical therapy. Topical corticosteroids are first line therapy for psoriasis, but it has many side effects. Topical vitamin D is an alternative sparing therapy topical corticosteroid for psoriasis. Purpose to review various studies on the effects of topical vitamin D monotherapy for psoriasis. Literature search through PubMed and Science direct for published articles within the past 10 years. Analysis follows inclusion and exclusion criterias. Articles are included if it reviews the effectivity of topical vitamin D as monotherapy for psoriatic lesions. Review includes 13 randomized controlled trials, involving 8.391 psoriasis patients. Clinical evidences show therapeutic effects of topical vitamin D for psoriasis. Ointment as vitamin D vehicles has better penetration. Effectivity of topical vitamin D as monotherapy is inferior when compared with combined therapy or corticosteroid combination. Long term monotherapy with topical vitamin D has minimum side effects.
