Abstract

Pre Exposure Prophylaxis (PrEP) is the use of Anti Retroviral (ARV) drugs by HIV-uninfected people to block the acquisition of HIV before exposure to HIV. There are concerns that PrEP introduction and scale up may pose risks, such as sexual behavior change in specific populations, and have an impact on the budget in already constrained health systems. Therefore, public health measures for the prevention of HIV and other STIs need to be enhanced, such as the prioritization of PrEP, coupled with more effective STI screening and treatment. Integration of STI and PrEP programmes can be viewed bi-directionally (not only integrating STI services into PrEP services but also considering STI clients as people also at risk for HIV and therefore potentially eligible for PrEP). This approach fosters synergies and efficiencies from a public health perspective. However, there are many challenges to programmatic integration, including siloed funding streams and programmes, the availability and costs of expanded etiological STI testing, and gaps in capacity and training for STI management. Keywords : PrEP, STI, services

Abstract

ABSTRACT  Background: Management of cases of difficulty swallowing in Indonesia has so far only used standardized forms of hospital food consisting of ordinary foods, soft foods, chopped foods, filtered foods and liquid foods. In developed countries, the use of commercial food thickener is commonly used for cases of swallowing disorders. Commercial food thickener products are difficult to obtain in Indonesia and the price is quite expensive so that substitute products that are derived from local and cheap food are needed.It is necessary to develop an alternative formulation of food thickener made from local which is easily obtained and cheaper in price as an alternative solution to the problem of eating difficulties. Some ingredients that have properties as a food thickener are glucomannan porang and xanthan gum. Porang glucomannan has a high fiber content while xanthan gum has high viscosity in low concentration. Objective: The purpose of this study was to determine the texture and viscosity of local thickener food in accordance with the consistency of commercial food thickener as a gold standard. Design: eksperimental laboratorium research which is held at the Nutrition Department in UGM Academic Hospital on June until November 2019, with the research steps are: 1) Make a standard gold solution of 6 grams of commercial food thickener 2) Measuring the texture and viscosity of a standard gold solution used as a comparison with other products 3) Conducting subjective and objective tests of local ingredients, namely glucomannan porang and xanthan gum. Results: From the research conducted in the laboratory by using the RVA method, all 4 flour (cornstarch, sago flour, arrowroot flour, tapioca flour and porang glucomannan flour) showed a viscosity close to the thickness of commercial food thickener. The advantage of glukomanan flour is that after the maximum viscosity value is reached, the viscosity does not change with an increase in temperature or when the temperature decreases. Conclusions: Porang glucomannan has a texture similar to commercial food thickener (gold standard), the viscosity of porang glucomannan tends to be stable with changes in temperature and when the temperature decreases so that it can be used as an alternative food thickener for dysphagia patients.

Abstract

Survival rate of ischemic stroke has been improving which increases long-term disability in consequences.  GBD 2017 shows stroke as the third most common cause of morbidity. Hypertension is the most common risk factor for ischemic stroke incidence and affecting long-term functional outcome. Therefore, hypertension control in ischemic stroke is expected to improve functional outcome and quality of life. Therefore, this study was conducted to know the role of hypertension as risk factor of poor functional outcome in primary ischemic stroke patients. Study was conducted by observational retrospective cohort from May 2018 until April 2019 in 7 hospitals in D.I. Yogyakarta. Functional outcome of primary ischemic stroke patients was assessed by physician at day 30 by using Barthel Index. Bivariate analysis was analyzed by using Chi-Square with 95% CI and p-value <0.05 indicated statistical significance. Based on the study, there were increased risk for having dependent functional outcome in hypertension (RR=1.12, 95% CI=0.74-1.69), diabetes mellitus (RR=1.041, 95% CI=0.71-1.53) but these results were not statistically significant (p>0.05). From the study it can be concluded that hypertension is one of the most common risk factors of stroke but hypertension has no role in predicting functional outcome.  However, hypertension increases risk for having poor functional outcome in ischemic stroke patients.

Abstract

Background: About 80-90% of patients LBP stated that they did not do any attempts to treat his illness. Spondylolisthesis (spon-dee-lo-lis-thee-sis) is a condition where one of the spinal vertebrae to shift forward or backward with the next vertebra. The disruption that can be caused due to this condition include a painful tap on regio lumbar muscle spasm, occurs a decrease in muscle strength, limitation of motion, can also happen pain in the limbs. The purpose of this research is to know the influence of the use of the Lumbo Sacral Orthosis semirigid type to the strength of  Trunk muscles in case of Low Back Pain (LBP) e. c Spondylolisthesis.Subjects and method:  this type of research was quasi-experimental design pre-test and post-test design. Location of the study Area General Hospital in Yogyakarta, Gadjah Mada University Academic Hospital, Dr. Soeradji Tirtonegoro Hospital, Tugurejo Semarang Hospital. The samples in this research are  35 patients with purposive sampling.Results :  there  was the influence of the use of the Semirigid Type LSO against muscle flexion trunk before and after treatment (Z =-5.099 with p value = 0.000),  trunk rotation muscle (Z =-4.600 with p value = 0.000), trunk extension muscle strength (Z =-4.472 with p value = 0.000).Conclusion:  As for the results of this research are the greatest influence has decreased muscle strength of the trunk after wearing the LSO within one month is on the muscle group Flexor (0.75), then extensor muscles (0.58) and rotator trunk muscles (0.65).Keywords: Lumbo Sacral Orthosis semirigid type, The strength of Trunk muscles, Low Back Pain (LBP) e. c Spondylolisthesis.

Abstract

Background: Appendicitis is an inflammation of the appendix vermiformis, which is the most common cause of acute abdominal emergency and requires immediate surgical treatment. Anamnesis and clinical examinations in Alvarado score results can be used as early detection to determine further treatment. The use of histopathological examination is to confirm the diagnosis of appendicitis. This study was to determine the correlation between the Alvarado score and the histopathological profile of appendicitis. It assesses appendicitis diagnosis accuracy based on the Alvarado score at the Academic Hospital of UGM.Methods: This study was a cross-sectional design study with a diagnostic test method using secondary data from 173 appendicitis patients from 1 January to 31 December 2019, who met the inclusion and exclusion criteria.Results: According to the histopathology result, the finding of acute appendicitis was 95.95%, which Alvarado score of ≥7 was 71.68%. The overall sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of Alvarado score for acute appendicitis were 73.49%, 71.43%, 73.41%, 98.39 %, 10.20%, respectively. Conclusion: The Alvarado score is a sensitive, specific, and accurate scoring system that functions as a diagnostic tool for acute appendicitis. 
The Impact of Reuse Dialyzer Application on Routine Hemodialysis Patients Adequacy and Its Correlation with Cost Efficiency in Type D Hospital Rulita Ayu Kusuma Ningtiyas

Abstract

Background: Hemodialysis (HD) is one of the high cost service unit in the hospital which it implemented the cost effciency policy by the application of reuse dialyzer. According to claim recieved from Jaminan Kesehatan Nasional (JKN) system, the HD procedure for type D hospitals received the smallest claim payment if it compared to type A, B and C hospitals. This policy will be measured its effect on the effectiveness in the form of Ureum Reduction Ratio (URR) and Kt/V values and it associated with cost efficiency in type D hospital settings. Objectives The aims of this study were to 1.) Measure the corellation between the policy of using reuse dialyzer and the HD adequacy value and 2.) Evaluate the cost efficiency policy implemented by type D hospital HD unit in the form of using reuse dialyzer. Methods: This study was conducted with a cross-sectional design study from URR and Kt/V data on routine HD patients at 4 of type D hospital in Yogyakarta and Cantral Java in period Januari-March 2021. HD adequacy values were compared between the groups using the new and reuse dialyser. Cost efficiency was calculated based on the cost difference between cost of the HD procedure using a reuse dialyser and the projected cost if it is carried out with a single-use dialyzer.Result and Discussion: HD adequacy data were obtained from 112 subjects consisting of 22 with a new dialyzer, 32 with a reused (R) dialyzer first and R2, 27 with R3-R4 and 31 with R5-R7.The analysis of the effect of using reuse dializer on URR and Kt/V in the group with the new and reuse dialyser was obtained p = 0,90 and 0,91. The HD adequacy value in using reuse dialyser was not significantly different from using the new dialyser. The result of the cost efficiency analysis was 20.55% with using reuse dialyzer when it is associated with a source of income for HD services in type D hospitals.Conclusion: The application of reuse dialyzer is proved that it did not affect HD adequacy achievement when it compared to the use of a new dialyzer, and saved costs on HD services in type D hospitals. Keywords: Hemodialysis, reuse dialyzer, HD adequacy, efficiency

Abstract

Background: The prevalence of stroke in Indonesia according to Basic Health Research (RISKESDAS) 20131 was 7%, increasing to 10.9% in the Basic Health Research (Riskesdas) 20182. Malnutrition is a kind of complication which stroke patients often experience. Complications that often arise in stroke patients are malnutrition. The impact of hospital malnutrition increases patients' length of stay and disease complications.Objective: To determine the differences in clinical outcomes and length of stay in patients with acute ischemic stroke based on nutritional status at Academic Hospital UGM.Research Methodology: The research used a retrospective cohort design with purposive sampling. All respondents were selected because they met the inclusion criteria. The respondents' data were taken from medical records of inpatients at the UGM Academic Hospital from January to December 2019. Data analysis was analyzed by using the Kruskal Wallis test because the data were not normally distributed.Results: The result showed that stroke was more common in men (54.3%) with an average age of 60-74 years (55.5%). After being hospitalized, most of the patients' condition was improved (89%) with a no-risk screening score (85.5%). The nutritional status in stroke patients is good (44.5%). The length of stay (LOS) of stroke patients mainly was 4-7 days (75.7%) with an average NIHSS score of 5-15 or moderate (47.4%). The statistical test showed a value of p <0.05 to assess the difference in clinical outcomes of LOS and NIHSS scores on nutritional status.Conclusion: There were differences in clinical outcomes in LOS and NIHSS scores based on the nutritional status of acute ischemic stroke patients. Patients with poor nutritional status had higher LOS and NIHSS scores, followed by normal nutritional status and overweight status. Keywords: Acute Ischemic Stroke, clinical outcome, nutritional status

Abstract

 Background: Sleman Hospital is a referral hospital for Covid-19 patients. The covid-19 pandemic appeared in the current budget year, the budget for handling COVID-19 is not yet in the 2020 Budget Determination Document (DPA). The duties as Commitment Making Officer (PPK) and procurement officer at Sleman Hospital are additional tasks outside of the main duties and functions. . The implementation of the COVID-19 procurement activities at the Sleman Hospital adjusts the translation of the existing funds as determined through the Director's Decree.Objective: To evaluate the implementation of the procurement of goods/services in an emergency period to fulfill the service needs of COVID-19 patients at the Sleman Hospital.Method ; This study uses a qualitative method. Evaluating the implementation of procurement of goods/services during the COVID-19 emergency period in the period April-December 2020. The research subjects are Commitment Making Officers (PPK), Budget User Authorities (KPA), Procurement Officers, Technical Implementation Officers (PPTK), and Goods Users .Results and Discussion: Sleman Hospital in carrying out goods/services procurement activities during the COVID-19 emergency through 3 stages, namely planning, implementation and completion of work. The implementation of the procurement of goods/services is carried out by the Commitment Making Officer (PPK) and the Procurement Officer. Procurement implementation uses the direct appointment procurement method. direct procurement, e-purchasing through LKPP e-catalog and fast tender/auction. Contract signing is carried out at the stage of execution of work. The use of general procurement procedures is carried out prioritizing the principle of procurement of goods/services in an emergency, namely the speed and accuracy of emergency handling. Goods/services procurement activities that use pure APBD funding sources have good achievements, namely above 70%.Conclusion: The implementation of procurement activities for goods/services at RSUD Sleman is guided by special emergency procurement regulations and general procurement regulations regarding government procurement of goods/services  sourced from pure APBD funds.

Abstract

Background: The development of a business unit is based on assessments given by competent parties both internally and externally. Nur Hidayah Hospital received a recommendation to become a Type C hospital from the Bantul District Health Office, the Association of Indonesian Hospitals (PERSI) DIY Region, and the DIY Hospital Supervisory Agency (BPRS) during visitation for the purposes of extending the operational permit of the Hospital in 2020.Objective: To determine the financial analysis of the feasibility of developing Nur Hidayah Hospital into a type C hospital.Design: This research is a qualitative descriptive method with a case study design. The data was taken at Nur Hidayah Hospital in September-October 2021. The data was obtained using questionnaires and secondary data from internal hospital. Data is processed by presentation of data and withdrawal of conclusions.Results: Based on data analysis, the Net present value (NPV) is 45,162,803,090 or positive value, with an internal rate of return (IRR) 28,48% and a payback period (PP) of 7 years and 2 months.Conclusions: The development of Nur Hidayah Hospital into a Type C hospital is feasible. The results of this study can be used by the management and the Nur Hidayah Foundation to develop the Nur Hidayah Hospital as planned.

Abstract

Background: The annual report of Panti Waluyo Hospital Surakarta in 2020 shows deficit by disturbances in cash flow. The financial report is assessed and classified for its performance based on financial ratio analysis in accordance with YAKKUM Performance Indicators. Objective: Measuring the influence of the Covid-19 pandemic on the financial performance rating in 2020 by mapping problems, and establishing a management strategy for the Hospital so that financial performance remains in the Healthy category during the Covid-19 pandemic. Design: This research uses descriptive qualitative research with a case study approach. The data was taken from the financial statements in 2020 and interviews with the Directors. The data is processed by presenting and drawing conclusions. Results: Mapping the problem can effectively measure the effect of the Covid-19 pandemic on the financial performance in 2020. The implemented management strategy was able to stabilize financial performance to remain in the Healthy category. Conclusions: Hospital management must be able to understand the events that occur from the description of the movement of financial accounts, then from these considerations are encouraged to make decisions.
