Jurnal ARSI (Administrasi Rumah Sakit Indonesia) was initiated by the Center for Health Administration and Policy Studies (CHAMPS) Faculty of Public Health, University of Indonesia (FKM UI) and is currently managed by the Department of Health Administration and Policy, Faculty of Public Health, University Indonesia . Jurnal ARSI (Administrasi Rumah Sakit Indonesia) was published with the Indonesian Hospital Association (PERSI) and the FKM UI Hospital Management Association (IKAMARS). Jurnal ARSI (Administrasi Rumah Sakit Indonesia) is a peer-reviewed journal that focuses on service administration and management in hospitals in Indonesia. The articles or scientific manuscripts published in the Jurnal ARSI (Administrasi Rumah Sakit Indonesia) include original research, case studies, and reviews supporting corporate governance, clinical governance, or both (bridging). This journal is published electronically, featuring articles in either Bahasa or English. Printed versions are produced only by request. This journal also provides direct open access to its content with the principle that research publications are freely available to the public for broad benefit.

Abstract

The focus of this study is about outpatient journey of “Dharmais” Cancer Hospital in 2014 with purposes to find the unit with highest variation of patient journey and to analize the cause of the variation. This research is cross-sectional qualitative. The results of this research show that APJ Instalation is the unit with the highest variation of patient journey, and the causing factors are softtare of the computer supporting the service, human resource, infrastructure, and service procedures. The researcher found out that high variation of patient journey will reduce the quality of the service.

Abstract

Irrational use of antibiotics lead to antibiotic resistance. Implementation of the policy on the use of antibiotics in Sanglah Hospital in Denpasar has never been evaluated. The purpose of this study to know the implementation of policy on the use of antibiotics, preventing antibiotics resistance and determine the application ofthe WHO interventions to improve rational drug use. This research is aqualitative descriptive study, with interviews and document traces. Data analysis through content analysis. The result shows the implementation of antibitics usage policy and application of WHO intervention has not gone well. Prevention of antibiotic resistance have not been well coordinated by team of four pillars of Antimicrobial Resistance Control Program. Required revision and dissemination of antibiotic usage policy traces, as well as prevention and control of infection.

Abstract

A hospital as an institution needs to implement good corporate governance and good clinical governance to improve service quality continuously. Public Service Agency (BLU) is a government agency established in order to provide services to the community in the form of supply of goods and/or services being sold without profit and doing activities based on the principles of efficiency and productivity. The government realized the needs for flexibility in healthy business practices of various institution, so it has issued Government Regulation No. 23, 2005 (PP 23/2005) of the Financial Management BLU and allow its application to be implemented in a variety of government agencies including the hospitals. The purpose of this research is to evaluate governance at four (4) type A vertical hospitals in Java and Bali. There are differences in the implementation. These include differences in achievement of the completeness document on good corporate governance as well as governance requirements documents as well as differences in the 4 (four) elements of governance from PP 23/2005 that includes 12 (twelve) related factors of Donabedian1.a and Glickman’s2.a scheme: corporate culture, BLU establishment, hospital by laws,strategic planning & business plan budget, service development, procurement of goods and services, service standards, tariffs, management officer, remuneration, staffing, training and supervision. This research suggested the need for improved understanding of work force management officer, the quality of guidance and supervision, the establishment of specialized managers in BLU in Ministry of Health, the establishment of an integrated team involving all stakeholders. Improvements needed in change organizational culture, BLU’s hospital selection & evaluation, BLU’s institutional requirements, reward & punishment system, clear rules and strategic plan & business plan budget. Appropriate public policy to determine the success of a country in achieving its objectives is needed.

Abstract

This qualitative study discusses the inaccuracies on medical record entry charging, and inaccuracy on coding that caused provider bear the financial risk in the case of Tetralogy of Fallot in INA-CBG at the Pediatric Cardiology and congenital heart disease unit RS Harapan Kita from January-September 2013. This study showed that 21.4% of cases with incomplete secondary diagnosis and the difference between the claim of Rp 251.273.615,- (4%). Factors contributed incomplete secondary diagnoses are: responsiblity, physical facilities, standard operating procedure of medical record, training, monitoring, and socialization. Therefore, tt is recommended to improve: physical facilities, structure and infra-structure, standard operating procedure of medical record, socialization, training and monitoring at regular intervals

Abstract

The treatment duration of MDR-TB patients is very long approximately, 18 until 24 months and the cost is very expensive. This study aimed to find out the treatment outcome as well as the cost variation of MDR/XDR-TB patients. This is an operational research using quantitative and qualitative mix methods. The sample were treatedMDR/XDR-TB patients who started the treatment from August 2009 until December 31, 2010. Total number of sample were 104 patients. The study revealed that the duration of treatment for XDR-TB patients is longer than MDR TB patients with lower success rate. The cost per patient of TB XDR were cured and completed treatment was USD 9,357 and USD 7,373 for the XDR-TB patients and statistically significant. The cost spent for XDR-TB patients who died during the treatment was higher compare to MDR-TB patients, USD 6,453 and USD 3,484 respectively. Additional time for length of treatment would give the probability of spending USD 11,75 per day. Male TB-MDR patients who live in East Jakarta with length of treatment less than 569 days have the curing chance 1.7 fold as compared to females TB-XDR patients with the same condition in term of length of stay.

Abstract

Cancer is a chronic disease and its prevalence is increasing nowadays. Uptudate, Indonesia doesn’t have a comprehensive program for National Cancer Control. This study aims to develop a strategic plan for the development of National Cancer Center for Indonesia. This study is conducted by using operational research approach, and is conducted during the months of March to December 2013 by involving various stakeholders. In Indonesia, cancer is the third killer disease among the NCD (Non Communicable Disease), the incidence is increasing as well as the funding for cancer treatment. All informants expressed the importance of establishing a national institute that plays a role in cancer control in a comprehensive manner. The most ideal institute is a non structural form and to achieve it, a National Cancer Center Development Team to be formed that coordinates with the Director of Cancer Hospital "Dharmais" which is a top referral center for cancer at this time. In conclusion, it is really important for national institute to take part in a comprehensive cancer control manner. The most ideal form of organization for the above function is a non-structural institution.

Abstract

This study aims to identify the continuous action plan in Fatmawati General Hospital outpatient services in order to increase the Fatmawati General Hospital’s image. The study is conducted through interviewing patients, health workers, managements staff, and the macro analysis is done through policy analysis. The results showed that thegap in outpatient service is in the compatibility quality (service speed, drug completeness, comfort) and communication. The dissatisfaction occured in Fatmawati General Hospitals is the cultivation of the main root problem which is the rising numbered of patients as a result of the referral system ineffectiveness. The action plan designed in a macro scale is to improve the referral system with a focus on balancing the role of public health unit and the individual health unit and in the micro scale the action plan is conducted through out hospitals internal improvements. This efforts also needs to be strengthened through Primary Health Care System as a strategy to ensure the affordability of essential health services provided universally to the public in order to achieve the common goal of better health.
