Abstract

Stunting is a condition where toddlers are too short for their age. One of the preventions is by giving health education about stunting to the parents. The aim of the study is to determine the effect of counselling on parents' knowledge and attitudes towards stunting at Lhoknga Health Center, Aceh Besar. The study was conducted from November 7 to 23, 2022. This type of study was quasi-experimental with a one-group pre-test-post-test pre-experimental design. The participants were parents who were the main caregivers of toddlers in Lhoknga Health Center area, Aceh Besar. A total of 105 respondents were recruited for this study. In regards to knowledge about stunting, pre-counseling test showed that most respondents (71.5%) had sufficient knowledge while post-test showed that all respondents (100%) had good knowledge. Meanwhile, 92.4% respondents had good attitude towards stunting prior to the counseling, and increased to 100% after the education session. The Wilcoxon test showed an increased in the average knowledge score, from 12.83 during pre-test to 18.67 on post-test. An increased in the average attitude score was also found, from 72.05 to 92.77. Statistical analysis showed that there was an effect of counseling on increasing parents’ knowledge (p0.001) and attitudes (p0.001) towards stunting. It can be concluded that counseling can increase parent's knowledge and attitudes towards stunting.

Abstract

Submandibular space is part of the deep neck fascial spaces, bordered by the oral mucosa of the mouth floor on the above and the superficial layer on the deep cervical fascia on the bottom, as it extends from the mandible to hyoid bone. Submandibular space frequently involves odontogenic infections, which can cause significant mortality and morbidity. Open surgical incision and drainage are the treatment of choice for submandibular space abscesses. This is a case report of a 47-years-old female with main complaint of mass on the right submandibular that she has experienced for 2 weeks. The mass release pus which is felt flow down the throat. Physical examination revealed a soft, tender mass of 55 mm x 20 mm diameter in the right submandibular region. Oral status of the patient revealed presence of oral candidiasis too. There was no complication of trismus, swallowing pain or paralysis of the lower lip. The patient’s temperature was 38.5⁰C. Laboratory examinations showed a raised white blood cell count and slightly decreased albumin level. Intraoral swelling and redness of gingiva on the right upper middle incisor were observed. The patient was successfully treated by abscess drainage, oral antibiotics and nystatin.

Abstract

Severe sepsis and septic shock are complex syndromes with high mortality that frequently affect ICU patients. Contributions such as the Surviving Sepsis campaign have led to the issuing of guidelines for managing these patients. This manuscript aims to provide a review of state of the art using a systematic review of resuscitation and the use of vasopressors in patients with septic shock. Using MESH terms and Boolean operators, systematic research for information was carried out in PubMed in February 2023. We included 28 articles of which 18 were selected using the prism algorithm that met the inclusion criteria (analysis of septic shock, fluid resuscitation, or the use of vasopressors in septic shock in humans). In conclusion, the management of septic shock should be performed based on clear objectives to improve outcomes, for which maintenance of physiological levels of oxygenation, serum lactate, and mean arterial blood pressure are crucial.

Abstract

Indonesian law regulates that physicians might work in maximum three places. This policy results in increased workload, which might lead to burnout, affecting the quality care. This review aimed to explore how physicians’ workload affect the quality healthcare in Indonesia. We found that most Indonesian physicians worked long hours due to the current policy allowing them to work in maximum three medical centers. Vast majority of doctors in Indonesia also had to deal with abundant administrative works, especially since the implementation of Jaminan Kesehatan Nasional (JKN). Excessive workload increased work-related stress and job dissatisfaction, which eventually increased physician’s burnout. All these results in poorer quality healthcare. One of the solutions is to regulate physicians to only work at one healthcare facility (Mono-loyalty). However, a careful consideration and thorough preparation to adopt this policy is warrant.

Abstract

The use of herbal medicines has become increasingly popular worldwide. Elderly has been associated with higher consumption of many medications, including herbal medicines. However, there is a lack of documentation of medicinal herbs use among Acehnese population. Our study aimed at evaluating the profile of herbals use among the elderly population in a rural area of Aceh Province, Indonesia. A cross-sectional study was conducted from October to November 2021 in Nagan Raya Regency of Aceh. To collect information on the use of herbs, a set of structured questionnaires was utilized. The data were presented in percentage (%). A total of 100 respondents were included comprising 54.0% female and most of the respondents aged between 60–64 years (43.0%). The largest percentage of the participants made their own herbal medicines (45.0%) and used them routinely as a habit (36.0%). About 75.0% of the elderly felt better and almost all of them (96.0%) reported no adverse reaction following herbs consumption. A total of 39 plant species were used by the respondents with Annona muricate L. was the most frequently used medicinal plant (16.0%) and leaves were the most commonly used parts (68.4%). Most of the herbs were prepared through boiling and squeezing (32.6%), and mainly used in the form of a drink (77.3%). Cough treatment with herbs made up 19.3%, followed by hypertension (18.0%), flatulence (7.6%), dyspepsia (5.8%), and fever (5.8%). Our investigation revealed consistency in the use of some medicinal herbs in the present study with published experimental data. However, since elderlies are often considered vulnerable individuals, the practice of herbal self-remedies among this population should be well-monitored.

Abstract

Nasopharyngeal carcinoma (NPC) is a tumor arising from the epithelial cells that cover the surface and line the nasopharynx. In Indonesia, NPC is the 4th most prevalent. Nasopharyngeal Carcinoma stage IVB as distant metastasis. The therapy in nasopharyngeal carcinoma (NPC) is Chemoradiotherapy. Concurrent chemoradiotherapy is either an established treatment modality or actively being investigated. Chemoradiotherapy have demonstrated decreased incidence of distant metastases compared with radiation only, however only if its treatment done completely. Here we present a rare case of unfinished chemoradiotherapy that caused metastases nasopharyngeal carcinoma patient. A 41-years-old consulted to ENT-HN department with main complaints of progressive cancer pain for 3 month and got worse in past 1 month. Patient also complaints for the pain at the waist, chest and headache currently. However, there was formal history of nasopharyngeal carcinoma for this patient since 2019 that had been treated with 3 cycles of chemotherapy and 26 cycles of radiotherapy. The patient was diagnosed with recurrent nasopharyngeal carcinoma post chemotherapy and radiotherapy with metastatic process. Unfortunately, the treatment underwent was not complete. A nasopharyngeal biopsy was performed and the histopathology result revealed a non-keratinizing squamous cell carcinoma nasopharynx. Lumbar CT-scan showed distant metastatic to lumbar vertebrae I-V. Nasopharyngeal carcinoma (NPC) represents a rare disease that showed as carcinoma that appears in the nasopharynx. The unfinished treatment of NPC leads to recurrent NPC and metastatic process to lumbar vertebrae I-V. Nevertheless, 20% of patients are radioresistant and experience a recurrence, which in most cases manifests with distant metastases.
