Abstract

Although the Indonesian government has conducted various efforts to prevent and deal with Tuberculosis disease, new TB cases consistently appear from time to time in this country. Accurate steps and techniques are needed to diagnose tuberculosis as early as possible, thereby reducing transmissions and curing patients as early as possible. One method to conduct early diagnosis of tuberculosis is the microscopic screening of Mycobacteriu Tuberculosa.The purpose of this journal is to get an overview of sputum removal procedures and sputum quality of TB suspects at Gajah II Public Health Centre, Demak District. The first method is spontaneous expectoration, and the second method is warm steam inhalation before sputum removal.In this study, the sputum samples were taken from 32 TB suspect patients who visited Gajah II Public Health Centre for the first time during the period of September 2019. The examination results of sputum quality were recorded into three (3) categories or groups, namely: (1) purulent, (2) mucopurulent, and (3) saliva. The study indicated as follows: using the first method, 2 (two) patients (6.25%) produced purulent quality of sputum, 8 (eight) patients (25%) with mucopurulent quality, and 13 (thirteen) patients (40.62%) with salivary quality, while 9 (nine) patients (28.13%) failed to produce sputum. Using the second method, by which patients were asked to inhale warm steam before removing sputum, the study revealed the following: 3 (three) patients (9.37%) with purulent sputum quality, 13 (thirteen) patients (40.62%) with mucopurulent quality, and 11 (eleven) patients (34.37%) with salivary quality, while 5 (five) patients (15.62%) failed to produce sputum.From the above, using the spontaneus expectoration or first method, most patients produced poor quality of sputum while using the second method, by which patients were asked to inhale warm water vapor before removing sputum, the patients produced improved quality of sputum for smear examination.Keywords: Tuberculosis, sputum quality, Mycobacterium Tuberculosis screening.

Abstract

Background: Patient satisfaction was one of the indicators of hospital service quality. Service excellent concept in hospital aimed to fulfill the patient expectation of hospital services. One of the factors which affect service excellent is leadership style. Objective:This research aimed to analyzed the relationship of transformational leadership style of the inpatient room head and service excellent practice of nurses in inpatient unit of Telogorejo Hospital. Method: This was observational research with cross sectional approach. Data collected using questionnaires. Sample was chosen with random sampling method and divided proportionally for each inpatient room as much as 184 respondents. Results: Research showed that idealized influence (p-value: 0,029), inspirational motivation (p-value: 0,037), intellectual stimulation (p-value: 0,018), individual consideration (p-value: 0,049) dimension from transformational leadership had significant relationship with service excellent practice of nurses in inpatient unit in Telogorejo Semarang Hospital. Conclusion: There were significant relationship between idealized influence, inspirational motivation, intellectual stimulation individual consideration of inpatient room head and service excellent of nurses in inpatient unit.Keywords: Transformational leadership, service excellent, nurse, hospital, patient expectation

Abstract

Radiology services as integrated parts of health services as a whole are part of the community's need for health services. Therefore, the radiology services should provide quality services. The study was a descriptive analytic research with qualitative and quantitative approaches or mix method that used data collection in the form of waiting time duration for radiology services, in-depth interviews, observation, and document review. Sampling using proposition estimation with random sampling technique with the number of 106 patients. The informants in primary data collection at the Radiology Installation at Syarif Hidayatullah Hospital were the head of the radiology installation, radiology specialist, head of the radiology and radiographers unit.The average waiting time duration for radiology services is 4 hours 59 minutes. The result has not met Kepmenkes Standard No.129/Menkes/SK/II/2008, which is ? 3 hours. The standard of waiting time duration for radiology service is not met the rule regarding to there is no doctor who is the standby or full timer while the radiology service to the patient is given for 24 hours.It is expected that Syarif Hidayatullah Hospital has teleradiology which can be used as a solution to meet the waiting time duration for radiology services, especially the reading of photos and expenditure of expedition results and can meet the quality of services that appropriate to the standards.Keywords: waiting time duration, radiology services

Abstract

Background: 5R (Ringkas, Rapi, Resik, Rawat, dan Rajin) is a work culture that must be applied in all workplaces in both the formal and informal sectors. The application of the 5R aims to make workers feel comfortable and safe when working. Besides, the application of the 5R can make work more effective and efficient because it can eliminate time wasted because more time to search for goods, work areas that look full due to overload of equipment and goods that are not needed or dirty environment. Machine shop X in Mlati Subdistrict, Sleman, Yogyakarta is one of the informal sectors that has not implemented 5R. This can be seen from the condition of the work area which is full of unnecessary, messy, and dirty equipment and goods. Therefore researchers researched the analysis of the application of 5R in Machine shop X. Objectives: The purpose of the study was to find out the application of the 5R principle in Machine shop X. Methods: This research design qualitative design. The research was conducted by direct observation and using the 5R checklist. Research results: The results showed that the application of 5R in Machine shop X was still poor. This can be seen from the achievement of the overall 5R which is below 50%. Conclusion: The research concludes that the implementation of the 5R in Machine shop X is still poor so that training on 5R and internal audits is needed periodically.Keywords: Implementation, 5R, machine shop

Abstract

Medical Record Document (MRD) guarantees patient safety. Besides, it related to claim accuracy to BPJS (Badan Penyelenggara Jaminan Sosial) as patient health insurance operator. In fact, from the survey conducted with 15 inpatients as the subjects, 10 MRD approved (approximately 66.67%) and the other 5 MRD or about 33.34% rejected. This happened as the consequences of whether supporting examination result which is not being submitted on 2 MRD (40%), 1 MRD or 20% hold incomplete BPJS document and other 40% caused by the inaccurate operation code in MRD. Hence, the aim of this study is to analyze the role of Person In Charge in Medical Record Department to improve MRD comprehensiveness to reach BPJS claim accuracy.The type of this study is qualitative with cross-sectional approach. Primary and secondary source observation and interview are used in collecting data. Purposive sampling with four interviewees from Person In Charge in Medical Record Department, and as the main interviewee is the head of Medical Record Department also one employee of BPJS Rumah Sakit Wongsonegoro as the triangulator.So, the main function of assembling employee is to ensure the quality of Medical Record Document (MRD), quantitatively and qualitatively. Whereas, RSWN already applied Person In Charge of Medical Record Department with concurrent analysis in controlling MRD comprehensiveness concept. According to observational result of 15 inpatients, found incomplete MRD, 13% occurs in identity section, also 13% each found both in recording and reporting. While, the authentication completes 100%. From consistency analyzing result discovered inconsistency recording about 13%. Main diagnose and inform consent recording reached 100% in consistency and 0% things that can cause loss.That is, the role of Person In Charge in Medical Record of inpatient unit needs yo be improved. The main and assembling function performs by PIC in Medical Record is both coding and indexing. Input standard, Standard Operational Procedures (SOP) and minimum services standard are already available and applied to control PIC in Medical Record performance quality. However, input, process and the output are not focus in controlling MRD comprehensiveness yet, especially in BPJS participant patient. So that, to control BPJS inpatient MRD quality, needs operational standard in methods and comprehensiveness control procedures, considered with PIC in Medical Record also performing ICD code and Medical Record Document quality requires coordination of all parties.The researchers suggest to add more points in controlling incomprehensiveness BPJS Medical Record Document procedures. Furthermore, enhance input in standard structure, operational job description in Standard Operational Procedure, also minimum services standard comprehensiveness which applied has to be reached 100% qualitatively and quantitatively.Keywords: PIC in Medical Record, BPJS claim, quantitative comprehensiveness, qualitative comprehensiveness

Abstract

The number of leptospirosis cases in Demak district had a significant increase in 2018 which reached 92 cases with IR = 7.91% and the death of 24 people with CFR = 28.09%. Bonang Subdistrict is a sub-district with a case of the highest leptospirosis in Demak district. The aim of this research is to spatial analyze the environmental factors of leptospirosis in Kecamatan Bonang include flood/rob history, garbage storage, river, paddy field, fishpond, and population density. This type of research is quantitative descriptive with cross sectional design. The samples in this study numbered 24 people. This research was conducted in June 2019 in Bonang Sub-district of Demak Regency. The results showed that the pattern of the leptospirosis case in sub-district Bonang was scattered uneven. In this study can be concluded that the most influential risk factors in the distribution of leptospirosis cases in Bonang sub-district is a bad condition of individual garbage storage and the existence of paddy fields and rivers that are close to the house cases.Keywords: Spatial Analysis, GIS, Leptospirosis
