Abstract

Islamic Hospital (RSI) Jemursari Surabaya is a hospital that serves all segments of society, religion, and socio-economic levels. Employee performance appraisal is one process for determining the quality of human resources (HR) at the RSI Jemursari Surabaya. During this time the performance appraisal conducted in RSI Jemursari Surabaya still uses such assessment form sheets of paper so that it can slow down the process of employee performance evaluation and wasteful use of paper. Based on the above problems, it needs to be made web-based application that can handle the performance appraisal process. This web-based application used by three users, namely employee, boss Employees and HR Department. In the assessment of applications, there are several processes, namely the process of making master criteria for performance and master type of violation, the employee performance appraisal process (co-employee, myself, and subordinates), and the process of making the performance appraisal report. Based on the results of trials that have been done with the RSI Jemursari Surabaya then this app may conduct performance appraisals for employees coworkers, yourself, and the subordinate employee. This application can also generate the value of employee performance and employee supervisor recommendations regarding the results of the performance appraisal to the trainee employees, first contract employees, second contract employees and permanent employees. Application performance appraisal may be expanded by the addition means such assessment checklist, an essay, or a recording of critical events and can be developed into a system that is integrated with a presence system and payroll system.

Abstract

CV. Bina Rukun is a company engaged in manufacturing namely paper processing raw materials into semi-finished to bag of souvenirs and printing and paper cutting services to the motif of a bag of souvenirs.In a day CV Bina Rukun produces raw materials semi-finished paper a number of 3000 kg/day. Inventory is very important in CV Bina Rukun to maintain stability or smooth production operation company and remove risk delay the coming the raw materials that needed. Demand for the number of the raw material that uncertain in any stretch out part of the barn is make difficulties in estimating the amount of the needs of raw materials to the future. Incorrectness in determining the time of reserving resulting in running out of stock and delay filling orders raw materials half-baked for customers.Based on problems going on up , can be given a solution that CV Bina Rukun need an application that can help in the process of control supplies raw materials that is , help determine how many the amount of raw will ordered and when they do it reservations repeated to the raw materials.Based on the results of the tryouts that have been made this application can solve the problems occurring by means of help determine how many the amount of raw will ordered and when they do it reservations repeated to raw materials. It is in good trial results by application by using black box testing by focusing on testing the function of the input of user in the system that gets the value of 100% means the application is worth used.

Abstract

UD Eka is a manufacturing company who produces shoes, rubber slippers, and raw materials to produce shoes such as soles and heels. Production activites are conducted based on “made to order” and “made to stock”. Currently, company determines the selling price based on the selling price from competitor’s company. After that, company compares with the result of estimates production costs and non-production costs. Therefore, company can’t determine the selling price independently and appropriately so the company can’t calculate profit expected (mark up). Based on this problem, have made application which can help to determine the selling price use full costing’s method. The method is selected because can help company to determine the selling price and mark up by paying attention non-production costs and return on investment. Based on test results, application which have been built can generate the selling price and mark up for each product by paying attention the calculation of non-production costs and return on investment appropriately. So, the selling price can be used a reference for company to determine the selling price.

Abstract

System Application and Product in Data Processing (SAP) is an application for financial administration process at PT KAI (Persero) Daop 8 Gubeng Mosque Surabaya based desktop. SAP can be a competitive advantage for PT KAI. It makes SAP developers want to know the acceptance and variables that affect the acceptance of SAP for employees of PT KAI. The solution of the problem is to analyze the variable of acceptance of SAP usage on PT KAI employee using Unified Theory of Acceptance and Use of Technology (UTAUT) model.This study aims to determine the level of acceptance and influential variables in the use of SAP by using methods UTAUT, Structural Equation Model (SEM), Smart Partial Least Square (PLS). Sampling by sampling technique with Purposive Sampling method at PT.KAI employee and determination of number of samples using slovin formula.Based on the results of analysis on UTAUT model of 68 respondents it can be seen that variables that influence acceptance of SAP usage: behavior to use a technology influenced by the intention to beperilaku, while the intention to behave and behavior to use a technology not influenced independent variable that is performance expectation, effort expectancy, Social influences and facilitating conditions that are moderated by gender, age and experience.Keywords: Reception SAP, PT KAI, UTAUT

Abstract

PT. Ichtiar Teknik Utama is a company engaged in manufacturing services and spare part supplier contractor tools, lathe, electric welding shop. To conduct its business processes PT. Ichtiar Teknik Utama serve ordering Spare part of contractor equipment based on consumer demand. The ordering system that runs on the company is too many involve the consumers which causes the ordering process complicated and difficult for consumers. In addition, There Lots of using hardcopy as media storage. It’s cause the Listing of ordering in the ledger is not good and will be difficult in making ordering report. Other than that, some of the data obtained can be lost, tucked, and also difficult to read. It takes a long time and complicates the preparation of reports. To improve efficiency and ordering listing process better, so it will need Ordering Information Systems of Spare Parts Contractor Equipment. The Information System has many processes including Registration, Ordering, Listing of Goods Orders, design, calculation of the price of goods, Approval, Payment and invoicing also reporting which used for decision making by the director of workshop. By implementing this information system, each section can perform the work more easily and be better when recording the order.Keywords: Information Systems, Ordering spare part, Contractor Equipment.

Abstract

Clinic House of Healthy Living Center (H2LC) is a clinic Aesthetic & Anti Aging consistently apply the concept of healthy living by combining elements of beauty and health of the skin and body. Constraints faced H2LC Clinic is the start of record-keeping, duplication of card history data medical records of patients, health progress / results of treatment patients were still recorded on medical record cards, prescriptions that are difficult to read, and payments are still using hand-written memorandum.To overcome the above problems the development method used in Information Systems Web-Based Medical Record On H2LC Clinic Surabaya is to use the System Development Life Cycle (SDLC) waterfall. Medical record system electronically application is beneficial not only administratively. But can be felt doctors and health workers is the ease in accessing patient information that ultimately helps in clinical needs.The results based on the implementation and evaluation of the banks have done the research was able to make Medical Record Information System Web-Based Clinical H2LC In Surabaya that produce information and manage the registration process assessment of the patient, record-keeping, and payments. Generate reports to consider the development of patient medical record information to be more easily and quickly.

Abstract

UD Eka is a manufacturing company who produces shoes and sandals, also footwear raw materials like soles and heels. UD Eka was established in 1990 and located in Krian Sidoarjo. Currently, the company does not calculate production costs, the company only estimates the cost of production for charged to each product. There is no calculation of production costs, making the company has no fixed basis in setting the selling price. Based on thats problems, this research result is make the application of the determination of cost of goods manufactured that can produce information on raw material costs, direct labor costs, factory overhead costs and cost of production. The method of determining the cost of production in this application is use full costing method. This method is chosen because this method concerns all production costs both fixed and variable in calculating the cost of production. So, the cost of production produced reflects the actual costs incurred in production activities and can determine the selling price in accordance with the desired profit. Based on the test results, the applications that have been built can generate direct material costs, direct labor costs, fixed overhead costs, variable overhead costs and cost of production.

Abstract

     Rail Document System (RDS) PT. Kereta Api Indonesia is an application media that can be used for all employees that will do correspondence process. This application is made with expectation for helping the smooth of the correspondence process in PT. KAI Daerah Operasional Surabaya (Dan op 8). It make the developer wants to know the succession and the any variables that influence the succession of RDS for PT. KAI’s employees. The solution of the problem is doing the RDS’s succession variables analysis in PT. KAI Daop 8 by using Delone and Mclean methods.     The purpose of this study is for knowing the succession and the variables that more influence in the use of RDS by using Structured Equation Model (SEM) Partial Last Square (PLS). Taking sample is using Stratified Random Sampling (SRS) method in PT. KAI’s employees and the determination of sample number by using slovin formula.      Based on the results of the analysis on the Delone and McLean models of 68 respondents it can be seen the variables that affect the success of RDS: net benefits are influenced by the use and satisfaction of users, while the use is influenced by the quality of information and service quality. For user satisfaction is not affected by the independent variables of system quality, information quality and service quality. Abstract

Palang Merah Indonesia (PMI) is an organization of national associations in Indonesia which is engaged in social humanity, one of the services performed by the PMI is a disaster handling. In the event of a disaster, the Red Cross Indonesia will dispatch a team named SATGANA. Teams that has been dispatched must make a report on the disaster that has occurred. Right now in PMI Surabaya, SATGANA team is not well monitored, the letter of assignment is often too late to be given, and the assessment should be reported at least 30 minutes after arriving at the site, often delayed or even not given. Another problem that arises is the absence of a system that provides information to PMI regarding status of each report is written, it is not stored properly, so many reports were missing or damaged. Based on the above problems, it takes an application that is able to facilitate the process of monitoring for the PMI. It also needed a system that helps the PMI for making the reports of the disaster. Developed systems made using the Waterfall method. Based on the results of trials that have been done, the system can meet the needs of the PMI Surabaya City in the process of disaster news, monitoring and process team SATGANA disaster reports.

Abstract

Homestay “The Rumah Kita” Lumajang, East Java which has the concept of a family inn. Curently Homestay “The Rumah Kita” Lumajang has a total of 13 rooms are divided into three types. Homestay "The Home" also provides facilities to pamper guests, such as restaurant, Snack 'n Coffee shop, minimarket, meeting room, Room Service, and free wifi area. For a hotel, the speed and accuracy of the data needed to make record relating to the management of guest data, room reservations, lodging transactions, transactions additional orders (food, snack, or drink), as well as other amenities. While the administration at Homestay "The Home" still requires a lot of changes, especially for the data collection process is not computerized and centralized.Information Systems Management with Visual Basic application is expected to simplify and accelerate the recording of transactions carried out by Homestay "The Home", can manage data into information that is accurate and fast, and can improve service to guests.
