Abstract

OBJECTIVE: To prove that glucose treatment in human prostatic stromal cells (HPSC) culture would increase PKC alpha isozyme activity. MaterialS & Methods: Primary cultures of HPSC were obtained from patients with bladder outlet obstruction caused by benign prostatic enlargement. The effects of various concentrations of glucose (5, 10, 15, 20, 25, 30, 35 micro-Mol) on PKC-alpha isozyme activity were examined quantitatively by spectrophotometry methods. ResultS: Spectrophotometry analysis, which is used to assess isozyme activity quantitatively, showed that exposures to various doses of glucose resulted in increased isozyme activity. The increment of activity is linear with the increment of glucose dose. There is a strong correlation between glucose doses and PKC-alpha activity, with R2 = 0,91. Conclusion: At different concentrations glucose increases PKC-alpha isozyme activity of HPSC.

Abstract

Objective: To determine the correlation of TGF-b1 and estrogen with prostate volume in BPH patients, old patients without BPH and young patients. Materials and methods: We enrolled 30 patients and allocated them into three groups. First group consists of BPH patients, second group consists of old male patients with more than 55 years of age, and third group consists of young male patients with less than 40 years of age. We analyzed the level of TGF-b1 and estrogen in these patients. Transrectal USG was used to measure the prostate volume. We analyzed the correlations of TGF-b1 and estrogen level with prostate volume. Results: We found a positive strong correlation between estrogen and prostate volume in the BPH patient group (p=0,021; r coeff=0,786). Other group showed no significant correlations. Conclusion: Estrogen plays an important role in BPH pathophysiology. Further research on the influence of estrogen in BPH patients is recommended.

Abstract

Objective: To study the effects of different bladder volumes on the length of prostatic protrusion as measured by transabdominal ultrasonography. Materials & Method: We did an observational study on 48 patients with BPH and urinary retention in Urology outpatient clinic of Cipto Mangunkusumo Hospital Jakarta from 1 March 2006 to 28 February 2007. The bladder of each patient was filled with 50 ml normal saline and increased stepwise by 50 ml each time until either an urge to void or a volume of 500 ml was reached. Length of prostatic protrusion was measured by transabdominal ultrasonography at each volume of bladder filling in every patient. Results: The average length of prostatic protrusion in 50, 100, 150, and 200 ml bladder filling was 11,14; 11,42; 11,88; 10,20 mm respectively. There is no statistically significant difference in the length of prostatic protrusion with transabdominal ultrasonography in each bladder volume. Conclusion: Prostatic protrusion length can be measured with bladder volumes between 50 ml to 200 ml and shows no significant difference.

Abstract

Objective: To investigate the efficacy of diclofenac 50 mg (Non-Steroidal Anti-Inflammatory Drug) in the treatment of nocturnal polyuria. Materials and methods: Twenty four patients (23 male and 1 female) with a mean age of 61 years (range 34 – 80) diagnosed with nocturnal polyuria were recruited. The study period comprised 2 weeks of either placebo or active medication. Following one-week rest period, patients were crossed over to the other medication for a further 2 weeks. Frequency volume charts were completed during the second week of each of the two study periods along with feedback forms to assess any subjective improvement in symptoms during each of the study periods. Results: A significant improvement in the symptoms was noted for diclofenac when compared with the placebo. The mean nocturnal frequency decreased from 3,61 ± 2,45 to 2,39 ± 1,1 (p<0,05) and the mean 24 h urine volume decreased from 1139,64 ± 365 to 1089 ± 324 (p>0,05), but there is no statistically different. No significant side effects were reported. Conclusion: Natrium diclofenac 50 mg are effective in the treatment of nocturnal polyuria causing a decrease in nocturnal frequency with subjective symptom improvement. Our study suggests, Natrium diclofenac not to prolonged treatment. Further research to find the alternative treatment is recommended.

Abstract

Objective: To define the use of Prostate Specific Antigen (PSA) as a predictor of prostate volume in catheterized BPH patients. Material & methods: Data were collected from medical record of symptomatic BPH patients who underwent prostate biopsy at Cipto Mangunkusumo Hospital Urology Clinic since 1994 until 2006. Data of age, serum PSA, indwelling catheter, maximal flow, urine residue, and prostate volume were recorded as variables. Patients who had histopathological result of BPH with or without prostatitis were included; while who had prostate carcinoma, high grade prostatic intraepithelial neoplasm (HGPIN), and BPH with atypical small acinar proliferation (ASAP) were excluded. Subject were then divided into 2 groups, catheterized or not. Data were analyzed using Mann-Whitney test, analysis of variance, correlation test and linear regression test. Results: Of  2283 patients were performed prostate biopsy, 1036 patients had results of BPH with or without prostatitis, 766 patients had prostate carcinoma, while 581 patient’s biopsy result were not recorded. Of the 1036 patients, 347 were catheterized, 192 non-catheterized, and 457 were not recorded. In the catheterized group the average age were 67,04 years old, serum PSA 26,88 ng/ml, prostate volume 54,33 ml, PSA density 0,39. In non-catheterized group the average age were 66,32 years old, serum PSA 16,58 ng/ml, prostate volume 53,94 ml, PSA density 0,33. Regression test showed that serum PSA could be used as a predictor of prostate volume (F=12,830; p < 0,001); in non-catheterized group it was stronger (F=12,989; p<0,001) than in catheterized group (F=6,814; p<0,01). The accuracy of serum PSA as a prostate volume predictor based on Receiver Operating Curve (ROC) as indicated by Area Under the Curve (AUC), in the catheterized group was 0,678 - 0,725 (p<0,001); in non-catheterized group was 0,723 - 0,784 (p<0,001). Conclusion: Serum PSA could be used as a predictor of prostate volume in patients with BPH wether catheterized or not. This predictor function is stronger in non-catheterized patients. Catheter has the effect of lowering the function of serum PSA as a prostate volume predictor in patients with BPH.

Abstract

Objective: To describe the prostate cancer management in Dr. Moewardi Hospital Surakarta on  January 2000 – December 2006. Materials & Methods: This study is a retrospective descriptive. The data was collected from Dr. Moewardi Hospital Surakarta’s medical record in seven years, from January 2000 to December 2006. Results: We found that thirty patients with prostate cancer with age above 50 years and mostly found on age 71 – 75 years (23,3%). The most common complain that can be found are LUTS (76,7%). Malignancies was found on digital rectal examination on 76,7% patients. About 28 patients with PSA level > 4ng. Most of the patients (20/66,7%) comes with T2 stadium and seven of them (23,3%) with metastize to the pelvic bone. All of the patient was administered with TUR-P, 7 patients with metastize administered to orchidectomy and hormonal therapy (23,3%), and only one of them continued with radiotheraphy. All of the prostate materials that analyze on pathyologic examination are adenocarcinoma of the prostate with 50% speciments are poorly differentiated. Conclusion: From thirty patients diagnozed with prostate cancer, most of them was found between 71 – 75 years old. The common complain are urinary retention, LUTS and pelvic bone metastize. The pathologic examination we shows all of the speciments are adenocarcinoma of the prostate and they are poorly differentiated.
