Abstract

Abstrak Pengobatan malaria membutuhkan ketepatan metode pemeriksaan laboratorium yang akurat oleh tenaga laboratorium. Kemampuan tenaga laboratorium perlu dilakukan pengamatan dan pemantauan secara terus menerus untuk menjaga efektivitas pemeriksaan sediaan. Tujuan penelitian ini adalah mengetahui efektivitas diagnosis malariadi laboratorium swasta di Kota Padang. Penelitian ini merupakan uji diagnostik dengan rancangan penelitian Cross Sectional Study terhadap laboratorium swasta di Kota Padang. Sampel yang didapatkan dalam penelitian ini berjumlah 53 slide lalu slide tersebut dibandingkan dengan Cross Checker. Dalam penelitian ini ditemukan bahwa spesiesterbanyak adalah Plasmodium vivax dan Plasmodium falciparum. Hasil perhitungan nilai Kappa adalah 0,5 dan perbandingan hasil pemeriksaan parasit P. falciparum pada laboratorium swasta adalah 17,8% dan pemeriksaan Cross checker 1,88%. Untuk P. vivax pada laboratorium swasta adalah 43,01% dan pemeriksaan Cross Checker 13,2%. Kesimpulan hasil ini adalah P. faciparum dan P. vivax adalah spesies yang sering ditemukan dengan nilai perhitungan Kappa menunjukkan hasil sedang dan terdapat perbedaan hasil pembacaan sediaan antara laboratorium swasta dengan yang dilakukan Cross checker. Kata kunci: malaria, efektivitas, plasmodium falciparum, plasmodium vivaxAbstract  Treatment of malaria need the accuracy of laboratory examination method by a laboratory personnel. The ability of laboratory personnel is need to doing observation and monitoring continuously to maintain the effectiveness of examination preparation. The objective of this study was to determine the effectiveness diagnosis of malaria in a private laboratory in Padang. This was a diagnostic test with a cross sectional study research design to a private laboratories in the city of Padang. Samples obtained in this study amounted to 53 slides and the slides were comparedwith Cross Checker. In this study it was found that most species were Plasmodium vivax and Plasmodium falciparum. The result of kappa value was 0,5 and the comparison results of the parasite P. falciparum in private laboratories were 17,8% and the Checker Cross examination are 1,88%.   P. vivax in private laboratories are 43,01% and the checker cross examination are 13,2%. The conclusion is P. vivax and P. faciparum are species that is often found by calculating Kappa values showed moderate results and there is a difference between the dosage readings with aprivate laboratory that performed Cross checker.Keywords: malaria, effectiveness, plasmodium falciparum, plasmodium vivax.

Abstract

Abstrak Aktivasi sel endotel atau disfungsi endotel menjadi faktor utama patogenesis preeklamsia. Mekanisme ini ditandai dengan penurunan ketersedian Nitric Oxide (NO). Disfungsi endotel pada hipertensi diperkirakan berkaitan dengan konsentrasi hemoglobin. Tujuan penelitian ini adalah menentukan korelasi kadar hemoglobin dengan kadarNO pada preeklamsia dan kehamilan normal. Desain penelitian ini adalah analitik cross sectional, dilaksanakan di RSUP Dr. M. Djamil Padang dan RST Reksodiwiryo Padang dari 13 Mei sampai13 Juli 2014. Pemeriksaan kadar hemoglobin dilakukan di laboratorium Patologi Klinik RSUP Dr. M. Djamil Padang dan kadar NO di laboratorium Biomedik Fakultas Kedokteran UNAND Padang. Populasi penelitian ini adalah seluruh ibu hamil yang datang ke RSUP Dr. M. Djamil Padang dan RST Reksodiwiryo Padang. Sampel dipilih secara consecutive sampling jumlah sampel 17 orang kehamilan preeklamsia dan 18 kehamilan normal. Hasil penelitian menunjukkan pada preeklamsia kadar hemoglobin 12,39 ± 2,10gr/dl dan NO 29,88 ± 9,47µmol, sedangkan pada kehamilan normal 11,18 ± 0,98gr/dl dan NO 84,22 ± 54,26 µmol. Uji statistik menunjukkan ada korelasi positif yang bermakna (p = 0,02) antara kadar hemoglobin dan kadar NO pada preeklamsia. Tidah ada korelasi antara kadar hemoglobin dengan kadar NO pada kehamilan normal (p = 0,52). Kesimpulan dalam penelitian ini adalah peningkatan kadar hemoglobin cenderung meningkatkan kadar NO pada preeklamsia dan peningkatan kadar hemoglobin tidak mempengaruhi kadar NO pada kehamilan normal.Kata kunci:  hemoglobin, nitric oxide, preeklamsia, kehamilan normal Abstract Endothelial cell activation or dysfunction becomes a major factor pathogenesis of preeclampsia. This mechanism is characterized by the decrease of Nitric Oxide (NO) level. Endothelial dysfunction in hypertension is related to haemoglobin concentration, especially its role in vascular function. The objective of this study was to determine the correlation between haemoglobin level and NO level in preeclampsia and normal pregnancy. In this study, cross-sectional analytic design was used to determine relationship between haemoglobin and NO levels. Thisresearch was conducted in the clinic, emergency room, and delivery room of Dr. M. Djamil Hospital Padang and Reksodiwiryo Hospital in Padang from 13 th  May to 13 th  July 2014.  Haemoglobin levels were tested in clinical pathology laboratory of Dr. M. Djamil Padang Hospital and NO levels were tested in biomedical laboratory of Medicine faculty ofAndalas University. In this study, 17 preeclampsia pregnancies and 18 normal pregnancies were taken into samples in the hospitals, where the sample numbers were determined by using consecutive sampling method. The results show that in preeclampsia haemoglobin level is 12.39 ± 2.10g/dl and NO level is 29.88 ± 9.47 µmol, while in normalpregnancy, haemoglobin level is 11.18 ± 0.98g/dl and NO is 84.22 ± 54.26µmol respectively. Statistically there is positive significant correlation between haemoglobin level to NO level in preeclampsia (p = 0.02). There is no correlation between haemoglobin level to NO level in normal pregnancy (p = 0.52).  It can be concluded that haemoglobin level is inversely related to nitric oxide level in preeclampsia group. In normal pregnancy group, haemoglobin level is not associated to nitric oxide level.Keywords:  haemoglobin, nitric oxide, preeclampsia, normal pregnancy

Abstract

Abstrak Salah satu faktor pencetus akne vulgaris adalah diet tinggi lemak, terutama lemak jenuh. Tujuan penelitian ini adalah untuk mengetahui perbandingan konsumsi lemak berdasarkan tingkat keparahan akne vulgaris menurut kriteria Lehmann. Penelitian ini berupa studi analitik observasional dengan desain cross sectional melalui pemeriksaan statusdermatologikus menurut kriteria Lehmann dan pengambilan data konsumsi menggunakan FFQ (Food Frequency Questionnaire) pada 138 siswa kelas X dan XI SMK Negeri 1 Kota Jambi. Hasil penelitian menunjukkan terdapat akne vulgaris derajat ringan (49%), akne vulgaris derajat sedang (42%), dan akne vulgaris derajat berat (9%). Reratakonsumsi lemak total 89,35 ± 17,63 gram. Rerata konsumsi SFA (Saturated Fatty Acid) 37,07 ± 9,97 gram. Rerata konsumsi MUFA (Monounsaturated Fatty Acid) 15,30 ± 11,79 gram. Rerata konsumsi PUFA (Polyunsaturated Fatty Acid) 12,03 ± 9,25 gram.Uji oneway Anova menunjukkan tidak ada perbedaan bermakna rerata konsumsi lemak total (p > 0,05) dan SFA (p > 0,05) berdasarkan tingkat keparahan akne vulgaris. Uji Kruskal-Wallis menunjukkan tidak ada perbedaan bermakna MUFA (p > 0,05), dan PUFA (p > 0,05) berdasarkan tingkat keparahan akne vulgaris. Penelitian ini menunjukkan tidak ada perbedaan konsumsi lemak berdasarkan tingkat keparahan akne vulgaris pada siswa SMKNegeri 1 Kota Jambi.Kata kunci: akne vulgaris, FFQ, lemak total, SFA, MUFA, PUFA Abstract One of the precipited factors is high fat diet, especially saturated fats. The objective of this study was to determine the comparison of fat consumption which is based on the severity of acne vulgaris.This research wasanalytic observational study using cross sectional design, by examining dermatologic status according to Lehmann criteria and  taking consumption record with FFQ (Food Frequency Questionnaire) of  138 tenth and eleventh grade student in SMKN 1 Jambi. The result showed that mild acne vulgaris (49%), moderate acne vulgaris (42%), and severe acne vulgaris (9%). The average of total fat consumption was 89.35 ± 17.63 gram. The average of SFA (Saturated Fatty Acid) consumption was 37.07 ± 9.97 gram. The average of MUFA (Monounsaturated Fatty Acid) was 15.30 ± 11.79 gram. The average of PUFA (Polyunsaturated Fatty Acid) is 12.03 ± 9.25 gram. Oneway Anova test showed no significant difference of average total fat consumption (p > 0,05) and, average SFA consumption (p > 0,05) which was based on the severity of acne vulgaris. Kruskal-Wallis test showed no significant difference of average MUFA consumption (p>0,05), and average PUFA consumption (p>0,05) which is based on the severity of acne vulgaris. Abstract

Abstrak Dokumentasi keperawatan merupakan bukti tertulis pelayanan yang diberikan kepada pasien oleh tenaga keperawatan yang bertujuan untuk menghindari kesalahan, tumpang tindih dan ketidak lengkapan informasi. Dalam asuhan keperawatan agar terbinanya koordinasi yang baik dan dinamis antar tenaga keperawatan serta meningkatkan efisiensi, efektifitas dan menjamin kualitas asuhan keperawatan. Tujuan penelitian ini adalah  menganalisis sistim manajemen dokumentasi keperawatan pada poliklinik gigi rumah sakit berdasarkan standar pelayanan keperawatan. Penelitian dilakukan dengan metode analisis kualitatif, sumber data berasal dari observasi, dokumen dan wawancaramendalam. Informan penelitian berjumlah 10 (sepuluh) orang yang terdiri dari direktur rumah sakit, ketua PPGI Kota Bukittinggi, kepala ruangan poliklinik gigi/ kepala instalasi dan perawat gigi di pol iklinik gigi rumah sakit di Bukittinggi. Validasi data dilakukan dengan triangulasi, selanjutnya dilakukan analisis data, reduksi data, interpretasi dankomunikasikan makna temuan melalui laporan tertulis. Hasil penelitian didapatkan dokumentasi keperawatan pada poliklinik gigi belum terlaksana dengan optimal, hal ini mempengaruhi proses pendokumentasian, tidak tersedianya kartu khusus pencatatan pemeriksaan dan perawatan gigi juga sangat mempengaruhi sistim dokumentasi keperawatan pada poliklinik gigi. Kesimpulan penelitian ini ialahsistim penyelenggaraan dokumentasi keperawatanpada poliklinik gigi belum terlaksana dengan optimal karena belum tersedianya kartu pencatatan pemeriksaan dan perawatan gigi pada poliklinik gigi rumah sakit, belum adanya SOP dokumentasi keperawatan dan jika ada tidak pernah disosialisasikan. Kata kunci: dokumentasi, poliklinik gigi, manajemenAbstract Nursing documentation is written proof of service to patients by nursing staff that aims to avoid errors, and incompleteness of information overlapping in nursing so good and dynamic coordination between nursing staff and improve the efficiency, effectiveness and ensure the quality of nursing care. The objective of this study was to analyze nursing documentation management system in a hospital dental clinic based nursing care standards. The study was conducted with qualitative analysis methods, data sources derived from observations, documents and in-depth interviews. Informants numbered 10 ( ten ) members consisting of the hospital director, chief dental nurses union Indonesia ( PPGI ) of Bukittinggi, the head of the room / installation and head nurse at the dental clinic dental hospital in Bukittinggi. Data validation is done by triangulation, then performed the data analysis, data reduction, data display and conclusion. The results showed nursing documentation in the dental clinic has not done optimally, this affects the process of documenting, recording card unavailability of dental examinations and treatment also greatly affect the nursing documentation system in the dental clinic. In conclusion, the implementation of a nursing documentation system at a dental clinic is not performing optimally due to the unavailability of recording card dental examinations and treatment at the hospital dental clinic, lack of standard operating procedures (SOP) nursing documentation.
