Quality is an important aspect in enhancing the competitiveness of the product. Quality's role is to give satisfaction to the customer and be able to compete with similar products. During of the production process there is a chance that the products are not produced in established standards. Products that not suitable with the specifications is a defect that would cause harm to the company. Trim Rear Quarter Right is a component of the APV Arena car which is each production of Trim Rear Quarter Right there are at least one or more defective products. This of course would lead to losses for the company. The method used in this case study is the six sigma method because this method has been proven effective. Defect in Trim Rear Quarter Right caused by five factors that is negligence of the operator in set up the machine, the age of the machine , the quality of the materials, the application of the method is not maximized and noisy environments. The set up process of injection molding machine is the most influential in causing the defect product with RPN (Risk Priority Number) value is 256.

Rendang is a traditional Minangkabau food. Rendang is generally made of meat, coconut milk and seasoning. Generally rendang semi-wet, but now it has been modified into a dry rendang. Dry rendang widely produced at Payakumbuh, West Sumatra Province. This research was conducted in July to August 2013 at Company " Rendang Erika " Payakumbuh. Research purposes is plotting the maximum profit and optimum production quantities with minimal use of resources. Processing data using the program LINDO (Linear Interactive Discrete Optimizer). The results of the study on the optimum conditions obtained maximum profit of Rp 84,049,360,- per month. The use of herbs and spices and coconut milk have been optimum.

PT Semen Padang is a company that produces cement and it can not be separated from issues related to the effectiveness of the machinery/ equipment caused by factors such as six big losses. It can be indicated from the frequency of damage caused to machinery/ equipment because of the damage so that production targets are not achieved. Another result of damage caused by machinery/ equipment is the quality of products where the products are not meet the quality standards. Therefore, it is necessary to measure effective and efficient in the maintenance of machinery/equipment int term of solving such problems. TPM is a method which developed in Japan that can be applied to improve the productivity and efficiency of production. The applied method is one of useful methods to mantain machinery/equipment utilization including, losses that called by the six big losses. One goal of TPM is to increase the effectiveness to improve function and performance of machinery/ equipment used and reduce the six big losses contained in the machine/ equipment. Study has been conductedin W1 and W2 Kiln engine on Indarung Factory II / III PT Semen Padang. The first stage is measuring the effectiveness of W1 and W2 Kiln machine using OEE method. Furthermore, we are y measure OEE Six Big Losses and another losses that lead to the low efficiency of the Kiln machine. The data has been collected based on year 2013. Result showed that the value of OEE in W1 kiln ranged from 49% to 96%. While the value of OEE in W2 kiln ranged from 60% to 98% . It is related to the level of availability W1 kiln (average 92%) were also quite low when it is compared to the level of availability W2 kiln (average 94%) due to the length of time the kiln W1 engine damage. Refer to the value of the performance rate W1 and W2 kiln, it can be concluded that the average value of both engine performance rate is almost the same, ranging between 91% and 92%. Analysis of OEE Six Big Lossesshowed that cause of low OEE on kiln machine is breakdown loss. It also affected the reduced speed loss.

PT . Mitra Kerinci is agro-industry that have tea plantation and some factory facilities consist of tea processes, green tea processes and black tea processes. PT Mitra Kerinci become the largest green tea producer in Southeast Asia. The Company continually make improvements in every line of the company to produce high quality products. Workers are an important element of the company as well as a critical corporate asset. The company always keeping workers comfortable and maximal doing his job, for the activities associated with the production of green tea such as picking tea and green tea production process and should be evaluated and developed to be improved towards better way. This study aimed to measure the workload physiological and psychological workload on the operator plucking tea and green tea production operators and provide recommendations based on the results obtained to improve the work system at the plucking tea and green tea production PT Mitra Kerinci. Measurement of physiological workload done by calculating calorie requirements, CVLpercentage and consumption of each carrier by measuring the pulse and temperature measurement operator workload while the psychological conducted using the NASA-TLX.

PT Incasi Raya is one of the major companies in Indonesia which produce edible oils. One of the stages in the production activities are packaging products. Used packaging made of plastic material, with the capacity and different type. Plastic pouch is one of the few types of packaging used in the packaging of edible oils. Quality packaging affects the distribution of production to the consumer. If the packaging is damaged, then the product can not be distributed to consumers. Therefore, the quality of the packaging must be kept for the successful marketing of the product. One way to implement a quality control methods Statistical Processing Control (SPC). The data used in this study is a secondary data provided by the company. Data collected is the number of production reject every month in packaging activities. The data processing of the data collection that is making control map p. The results of data processing show that many reject the production of packaging that are outside the control limits. Data that are outside the control limits indicates there is a problem in the quality control of the company. Of all types of packaging, only one or two months of production reject packs that are in the control limits. This suggests that the dominant packaging reject each month of production is outside the control limits. Reject the production was analyzed using a causal diagram. Factors influencing the presence reject packaging production is based on the analysis of human, machine, environment, materials, and methods within the company. After analyzing of the causal diagram, the data is revision. Making a map of the proposed p controls the data that has been revised is the end result of the data processing is done. Quality control companies are advised to be on the boundary control such as control map p recommended.

PT. Moradon Berlian Sakti is Die Casting manufacturer of Honda motorcycle spare parts and other types of products, such as chair. Along with the rapid advancement of technology, PT. Moradon Berlian Sakti always strive to maintain and improve its existence. One of important factor that need to be noticed is the productivity issue. Productivity assessment can be conducted based on the comparison of company input and output in terms of materials, capital, labor and energy. After learning the level or productivity index, company leader can identify which condition has productivity shrinkage and make decision to increase productivity company based on those condition. Based on the calculations, the levels of productivity index in 2012, was virtually uniform from January to December. But in October the productivity is high and reach the level 94.33%. That is because that performance indicators are in accordance with expectation of company leader. While in December productivity dramatically decreased to 4.67%, it is because less supervision and control from the company.

Leadership, motivation, work environment, and discipline is some of important factors where its influence in employees performance need to be studied, in order to improve better performance or provide positive influence on the performance itself. Employees performance in the welding section 1 A, has significant effect in the production process at PT. XYZ Motor, which manufactures parts of the body frame, in order to complete the daily production target, the influence of these factors on the employees performance needs to be determined. This study uses a questionnaire for collecting data from samples of 80 respondents taken from 158 employees in the welding 1 A. Multiple linear regression is used to test hypotheses for each variable. Result showed that the discipline variables has positive influence on employee performance with significance value of 0,04, where the value is less than 0,05. Other factors has value greater than 0,05.

Hydropower is one of the power plants that supplies electrical energy. It has several machines and equipment that have a time limit of use. To anticipate the limitation of the use, maintenance of machinery and equipment are needed in order to return its original state and can perform its function as usual. Therefore some indicators required to determine maintenance work performance level to provide the optimal results. The Traditional measurement methods have some limitations that make it difficult to implement in industrial environments. This requires a performance measurement method that can provide a balanced perspective on the overall system performance. One of them is implementing the Maintenance Scorecard and Objective Matrix (OMAX) method to assess the performance of hydropower maintenance. Objective research in this assessment is to integrate Maintenance Scorecard and Objective Matrix (OMAX) approach in Maninjau Hydropower. Key Performance Indicators (KPI) are defined by 6 perspectives: productivity perspective, cost effectiveness perspective, a safety perspective, an environmental perspective, quality perspective and learning perspective. The priorities are determined by using Analytical Hierarchy Process (AHP). We then measure the performance of maintenance by using Objective Matrix (OMAX) until we get the level of achievement of the performance in the current situation.The results obtained in the process of designing the scorecard maintenance on the Maninjau hydropower that produces overall 20 KPI consists of 9 KPI for productivity perspective, 4 KPI for quality perspective, 2 KPI for safety perspective, 2 KPI for environmental perspective and 3 KPIs for learning perspective. There are 5 KPIs for corporate level, 8 KPIs for strategic level and 7 KPIs functional level. The operation results show the number of KWH that produced, Capacity Factor and Equivalent Outage Force Factor are in the yellow zone. It means the attentions and improvements are needed to improve performance in the next period.

In palm oil supply chain (POSC) the smallholder farmers sell their fresh fruit bunch (FFB) to Palm Oil Mills through traders. Palm Oil Mills convert the FFB into crude palm oil (CPO). CPO is sold to the refinery, who converts CPO into frying oil and sends the product to the distributors. The distributors subsequently sell them to the consumers. Each member of the POSC will try to optimize its added value. The aim of this paper is to develop an added value formulation as a function of risk, investment and technology levels of each of the POSC member. To facilitate fair distribution of rewards a concept of added value utility based on rsk, investment and technology level was introduced. To optimize the added value distribution between the members the concept of stakeholder dialogue was used. The selling prices were negotiated between the actors until each reached a satisfactory value, which was ruled by the levels of optimum added value utility. This research is important because the developed model can facilitate a better formula to calculate the fair distribution of added values, therefore ensure its sustainability and improve the total supply chain added value.
