Abstract

Paediatric patients with cancer are a high-risk patient population for medication misadventures. This study aimed to document and evaluate the role of pharmacists’ interventions during dispensing-related activities in minimising the occurrence of medication misadventure in haematology-oncology patients. The primary investigator observed and documented all clinical interventions during dispensing-related activities performed by clinical pharmacists in a haematology-oncology pharmacy during 33-day. A total of 359 interventions were performed for 1028 patients. The rates of intervention were 20.04 per 100 medication orders and 34.92 per 100 patients. Provision of drug information was the most common interventions constituting more than three quarters of all interventions. According to therapeutic groups, cytotoxic antineoplastics made up more than half of all interventions. Of all interventions, 22 involved recommendations leading to changes in patients’ treatment (active interventions), and all recommendations were accepted. The top three medication errors were due to inappropriate dosing, labelling error, and unfulfilled indication. Clinical pharmacists’ intervention during dispensing in a paediatric haematology-oncology pharmacy improved medication safety and patient care by minimising the incidence of medication misadventures.Key words: Haematology-oncology, medication misadventure, paediatrics, pharmacists’ intervention Intervensi Farmasis pada Pasien Pediatrik di Apotek Hematologi-Onkologi: Apakah Farmasis dapat Mengurangi Kecelakaan dalam Pengobatan? Pasien kanker pediatrik merupakan kelompok pasien berisiko tinggi mengalami kecelakaan pengobatan. Penelitian ini bertujuan untuk mendokumentasikan dan mengevaluasi peran intervensi farmasis selama dispensing (peracikan dan penyerahan obat) obat untuk meminimalisir kecelakaan pengobatan pada pasien hematologi-onkologi. Peneliti utama mengobservasi dan mendokumentasikan intervensi klinis selama dispensing oleh farmasis klinis di sebuah apotek satelit hematologi-onkologi selama 33 hari. Sebanyak 359 intervensi diberikan pada 1028 pasien. Prevalensi intervensi sebesar 20,04 tiap 100 obat yang diresepkan dan 34,92 tiap 100 pasien. Pemberian informasi obat adalah intervensi yang paling umum diberikan (lebih dari tiga per empat dari semua intervensi). Berdasarkan terapetik obat, antineoplastik sitotoksik terkait dengan lebih dari setengah intervensi. Sebanyak 22 intervensi melibatkan rekomendasi yang mengarah pada perubahan pengobatan pasien (intervensi aktif) dan semua rekomendasi diterima oleh dokter. Adapun tiga besar kesalahan pengobatan disebabkan dosis tidak sesuai, kesalahan pelabelan, dan indikasi yang tidak terobati. Abstract

Kualitas pelayanan dengan dimensi bukti langsung, daya tanggap, kehandalan jaminan, dan empati dapat memengaruhi kepuasan pelanggan yang selanjutnya berujung pada niat membeli pelanggan. Berdasarkan laporan bulanan instalasi farmasi hanya sekitar 30% pasien yang membeli obat di depo farmasi Anggrek dari pasien yang berkunjung ke Poliklinik Spesialis Anggrek RSUP Dr. Hasan Sadikin. Penelitian ini bertujuan untuk mengetahui pengaruh kualitas pelayanan dan kepuasan pelanggan terhadap niat beli pelanggan di depo farmasi Anggrek RSHS. Metode yang digunakan adalah metode survei analitik dengan rancangan penelitian potong lintang. Sampel yang digunakan sebanyak 200 pasien yang terdiri dari 104 pelanggan dengan lebih dari satu kali kunjungan dan 96 pelanggan yang satu kali kunjungan ke poliklinik tersebut. Pengumpulan data menggunakan kuesioner dan dianalisis menggunakan aplikasi Smart PLS V 2.0. Kualitas pelayanan berpengaruh terhadap kepuasan pelanggan dengan dengan nilai t-hitung 12,755 (lebih besar dari t tabel 1,983). Kepuasan pelanggan berpengaruh terhadap niat beli dengan nilai t-hitung 5,012 (lebih besar dari t tabel 1,983). Kualitas pelayanan berpengaruh terhadap niat beli dengan nilai t-hitung 1,455 (lebih kecil dari t tabel 1,983). Kualitas pelayanan memengaruhi pelanggan sehingga pelanggan tidak berniat membeli obat di depo farmasi yaitu ketidakteresediaan konseling, waktu tunggu yang lama, kebutuhan akan ruang konseling khusus, ruang tunggu yang luas, dan kelengkapan obat.Kata kunci: Kepuasan pelanggan, kualitas pelayanan, niat beli Influence Service Quality and Customer Satisfaction towards Drug Purchase Intention in Anggrek Outpatient Pharmacy Depo at Hasan Sadikin Hospital The quality of service is an evaluation which focused on customer’s awareness about a structural construction of a service or product that involves 5 main aspects which are tangibility, empathy, responsiveness, reliability and assurance. Based on monthly reports of pharmacy installation only about 30% of patients buy drugs in the Anggrek out patient depo out off patients visiting Anggrek out patient specialist clinic in Dr. Hasan Sadikin Hospital. The aim of this study is to determine the effect of service quality and customer satisfaction to purchase intention in the Anggrek out patient depo Hasan Sadikin hospital at Bandung. The method used in this study is analytical survey with cross sectional design. The samples used were 200 patients, consist of 104 customers who have visited more than one times and 96 first visit costumer to this clinic. Data was collected using a questionnaire and analyzed using Smart PLS V 2.0 software. The results of this study showed that the service quality with tangible dimensions, reliability, responsiveness, assurance, and empathy are affecting the customer satisfaction with a score of 12.755 t-count (greater than t-table 1.983 ) and a positive value of the original sample of 0.800. Customer satisfaction affecting the customer purchase intentions with t-count is greater than t-table values of 5.012 and 0.726 of the original positive sample. While the service quality does not directly influence customer purchase intention with the t-test is smaller than t-table is 1.455 and the negative of the original sample -0.287. Some of service quality influence customers that causes not purchasing drugs from the out patient depo there are effect of unavailability of counseling, long waiting time of service, the need for special counseling room, a spacious waiting room, and the completeness of drug availability.Key words: Customer satisfaction, purchase intention, quality of service

Abstract

Some health system in various countries in the world is highly fragmented and is unable to resolve health problems. With collaborative care system, it is expected that DM patients can be managed well. We conducted a study to determine the effect of collaborative care system on blood glucose levels of patients with type 2 diabetes mellitus. This study uses a quasi-experimental design. Data was obtained by measuring random blood glucose levels in patients with type 2 diabetes mellitus. The samples consisted of 66 diabetic patients divided into 2 groups: 35 patients in the intervention group in Puskesmas Fakfak  and 31 patients in the control group in Balai Pengobatan Misi. Data was analyzed using statistical paired sample t test in the intervention group, the Wilcoxon test for the control group, and the Mann-Whitney test to compare the difference of random blood glucose levels between the groups. The results showed that the collaborative care system in the intervention group for 1 month shows the average random blood glucose levels before CCS 276.11 mg/dL and after CCS 222.43 mg/dL. Whereas in the control group before CCS 217.32 mg/dL and after CCS 266.45 mg/dL. Statistical test result showed significant differences (p<0.05) between random blood glucose levels before and after the CCS in the intervention group and the control group The conclusion of this study is a collaborative care system affects the reduction in random blood glucose levels in patients with type 2 diabetes mellitus.Key words: Collaborative Care System (CCS), diabetes mellitus type 2, random blood glucose level Pengaruh Collaborative Care System (CCS) terhadap Kadar Glukosa Darah Pasien Rawat Jalan dengan Penyakit Diabetes Melitus Tipe 2 Beberapa sistem kesehatan di berbagai negara di dunia sangat terfragmentasi dan tidak dapat menyelesaikan masalah kesehatan. Dengan sistem collaborative care, diharapkan pasien diabetes melitus (DM) dapat dikelola dengan baik. Penelitian ini dilakukan untuk mengetahui pengaruh sistem collaborative care pada kadar glukosa darah pasien DM tipe 2. Penelitian ini menggunakan desain quasi-eksperimental. Data diperoleh dengan mengukur kadar glukosa darah sewaktu pada pasien dengan DM tipe 2. Sampel terdiri dari 66 pasien DM yang dibagi menjadi 2 kelompok, yaitu 35 pasien pada kelompok intervensi di Puskesmas Fakfak dan 31 pasien dalam kelompok kontrol di Balai Pengobatan Misi. Data dianalisis dengan menggunakan uji statistik paired sample t test pada kelompok intervensi, uji Wilcoxon untuk kelompok kontrol, dan uji Mann-Whitney untuk membandingkan perbedaan kadar glukosa darah sewaktu antar kelompok. Hasil penelitian menunjukkan bahwa sistem collaborative care pada kelompok intervensi yang dilakukan selama 1 bulan menunjukkan kadar glukosa darah sewaktu rata-rata sebelum SCC adalah 276,11 mg/dL dan setelah SCC adalah 222,43 mg/dL. Pada kelompok kontrol sebelum SCC adalah 217,32 mg/dL dan setelah SCC adalah 266,45 mg/dL. Hasil uji statistik menunjukkan perbedaan yang signifikan (p<0,05) antara kadar glukosa darah sewaktu sebelum dan sesudah SCC pada kelompok intervensi dan kelompok kontrol. Abstract

Saat ini pemerintah berupaya mewujudkan Sistem Jaminan Sosial Nasional (SJSN) bahwa setiap rakyat Indonesia berhak atas jaminan sosial untuk dapat memenuhi kebutuhan dasar hidup yang layak. Penelitian ini bertujuan untuk mengetahui gambaran manajemen obat farmasi rawat jalan RS X dan menganalisis hubungan manajemen obat dengan kualitas pelayanan farmasi rawat jalan RS X. Manajemen obat ini termasuk perencanaan, pengorganisasian, pengarahan, dan pengawasan. Penelitian ini menggunakan pendekatan kuantitatif, yaitu analitik observasional dengan rancangan cross sectional study dengan sampel penelitian pelanggan farmasi rawat jalan di RS X. Pengumpulan data menggunakan kuesioner dari 100 orang pelanggan rawat jalan dengan menggunakan metode consecutive sampling. Hasil penelitian mengunakan Pearson Correlation menunjukkan hubungan manajemen obat dengan kualitas pelayanan farmasi rawat jalan yang bermakna dengan nilai untuk aspek perencanaan (r=0,626; p<0,001), pengorganisasian (r=0,409; p<0,001), pengarahan (r=0,359; p<0,001), dan pengawasan (r=0,426; p<0,001) dengan R2 multiple 66,80%. Gambaran manajemen obat di farmasi rawat jalan RS X menghasilkan nilai rata-rata 96,90% sehingga berada dalam kategori sangat baik dan membuktikan hubungan kuat antara empat fungsi manajemen obat terhadap kualitas pelayanan farmasi rawat jalan RS X.Kata kunci: Evaluasi manajemen obat, kualitas pelayanan, Pearson Correlation  Evaluation Management of Drugs and Relations with Quality of Outpatient Pharmacy Services in One of Hospital Pontianak City Nowadays government policy which embodies the National Social Security System (SJSN) where the presence of this system that every Indonesian people entitled to social security to be able to meet the basic needs of living. This study aims to describe the pharmaceutical drug outpatient management Hospital X Pontianak City and analyze the relationship management with the quality of pharmaceutical care medicine outpatient Hospital X Pontianak. This medication management including planning, organizing, directing, and monitoring. This study uses a quantitative approach which is an observational analytic research using cross sectional study with a sample of outpatient pharmacy customer research in Hospital X Pontianak. Collecting data using questionnaires from 100 customers outpatient with consecutive sampling method. The results using Pearson Correlation analysis showed the drug management relationship with the quality of outpatient pharmacy services which means the value of aspects planning (r=0.626; p<0,001), organizing (r=0.409; p<0,001), directing (r=0.359; p<0,001), and controlling (r=0.426; p<0,001) with R2 multiple 66.80%. The description of pharmaceutical drug management in outpatient Hospital X produce an average value 96.90% so as to be in very good category, there by proving the existence of a strong relationship between the four functions of management of the quality of pharmaceutical care medicine outpatient Hospital X.Key words: Evaluation medication management, Pearson Correlation, service quality

Abstract

Diabetes mellitus (DM) was an annual disease characterized by parennials of blood glucose levels exceeding normal and impaired metabolism of carbohydrates, fat and protein caused by defficiency of insulin hormone relative as well as absolute. Management of patients with DM can be done by some effort, among others was consume an antidiabetic drug or antidiabetics herbal medicine. Antidiabetic herbal medicine were consisting of bitter, brotowali, salam leaf, and AAI (analegtic, antiinflamation, and immunomodulator). The resesarch aimed to know the influence of antidiabetic herbal medicine to decrese blood glucose levels in patient with DM at The ‘Hortus Medicus’ Scientification of Jamu Clinic, Tawangmangu, Karanganyar. The method was quasi experimental with one group pre-post test. The samples were 37 people of the patients with DM diagnose with purposive sampling technique methode. The research report that among 37 respondents, 32 (86.5%) of respondent got a decreased blood glucose levels. The mean value of blood glucose level before consuming antidiabetic herbal medicine was 290.30 mg/dl and the mean value after consuming the herbal medicine was 241.78 mg/dl with difference of the mean value before and after consuming the herbal medicine was 48.52 mg/dl. There were influence of antidiabetic herbal medicine to decrease blood glucose levels of diabetes mellitus patients at the clinic with p-value=0.00 (p<0.05).Key words: Blood glucose, brotowali, diabetes mellitus, herbal medicine, salam leaf, sambilotoPengaruh Obat Herbal Antidiabetes untuk Menurunkan Kadar Glukosa Darah pada Pasien Klinik Jamu Scientific “Hortus Medicus” Tawangmangu, KaranganyarDiabetes melitus (DM) adalah penyakit menahun yang ditandai dengan peningkatan kadar glukosa darah melebihi normal dan gangguan metabolisme karbohidrat, lemak, dan protein yang disebabkan oleh kekurangan hormon insulin secara relatif maupun mutlak. Manajemen pasien dengan DM dapat dilakukan dengan beberapa upaya, antara lain adalah mengonsumsi obat atau obat herbal antidiabetes (jamu). Jamu antidiabetes yang digunakan terdiri dari sambiloto, brotowali, daun salam, dan AAI (analgesik, antiinflamasi, dan imunomodulator). Penelitian ini bertujuan untuk mengetahui pengaruh jamu antidiabetes untuk menurunkan kadar glukosa darah pada pasien DM di klinik Saintifikasi Jamu ‘Hortus Medicus’, Tawangmangu, Karanganyar. Metode penelitian ini adalah kuasi eksperimental dengan one group pre-post test. Sampel yang digunakan adalah 37 orang pasien terdiagnosis DM dengan teknik pengambilan sampel purposive method sampling. Data penelitian  menunjukkan bahwa di antara 37 responden, 32 (86,5%) dari responden mengalami penurunan kadar glukosa darah. Kadar glukosa darah sebelum mengonsumsi jamu antidiabetes adalah 290,30 mg/dl dan nilai rata-rata setelah mengkonsumsi jamu antidiabetes adalah 241,78 mg/dl dengan perbedaan nilai rata-rata sebelum dan setelah mengkonsumsi jamu adalah 48,52 mg/dl. Abstract

  Osteocalcin merupakan senyawa penanda untuk pembentukan tulang sedangkan β-CrossLaps (β-CTx) merupakan fragmen hasil pemecahan kolagen tipe 1. Tujuan penelitian ini yaitu untuk menentukan N-MID osteocalcin (N-MID Oc) dan β-CTx sebagai penanda biokimia Bone Turn Over untuk deteksi dini osteoporosis pada wanita menopause. Nilai N-MID Oc dalam serum ditentukan dengan pereaksi Elecsys N-MID Osteocalcin kit dan nilai β-CTx dalam serum ditentukan dengan pereaksi Elecsys β-CROSSLAPS kit yang bekerja berdasarkan prinsip Electrochemiluminescence Immunoassay (ECLIA). Data yang diperoleh diuji dengan menggunakan Uji-T. Sampel yang dikumpulkan berasal dari wanita normal, osteopenia, dan penderita osteoporosis pada wanita menopause. Hasil penelitian menunjukkan terdapat perbedaan nilai N-MID Oc pada wanita menopause normal dengan wanita menopause osteopenia, wanita menopause normal dengan wanita menopause penderita osteoporosis dan wanita menopause osteopenia dengan wanita menopause penderita osteoporosis (α=0,05), sedangkan pada nilai β-CTx terdapat perbedaan antara wanita menopause normal dengan wanita menopause osteopenia, wanita menopause osteopenia dengan wanita menopause osteoporosis, tetapi antara wanita menopause osteopenia dengan wanita menopause osteoporosis tidak menunjukkan perbedaan (α=0,05). Penelitian ini menunjukkan bahwa N-MID Oc dan β-CTx dapat dijadikan parameter untuk deteksi dini osteoporosis.Kata kunci: N-MID Osteocalcin (N-MID Oc), Osteoporosis, β-CrossLaps (β-CTx) N-MID Osteocalcin (N-MID Oc) and β-Crosslaps (β-CTx) as Biochemical Markers of Bone Turnover in Menopausal Women Osteocalcin is a marker compound for bone formation while the β-crosslaps a fragment split from collagen type 1. The aims of this study were to determined of the value of N-MID osteocalcin (N-MID Oc) and β-CTx as a biochemical marker of Bone Turn Over as early detection of osteoporosis in postmenopausal women. The value of N-MID Oc in serum was determined by reagent Elecsys N-MID Osteocalcin kit and the value of β-CTx in serum was determined by reagent Elecsys β-Crosslaps kit which works with the principle of Electrochemiluminescence Immunoassay (ECLIA). The data obtained were examined using T-test. The sample collected from normal, osteopenia, and osteoporotic in menopausal women. There were a significant difference N-MID Oc between normal, osteopenia, and osteoporotic in menopausal women (α=0,05). There were significant differences of β-CTx between normal and osteopenia, normal and osteoporosis but between osteopenia and osteoporotic menopausal women not showed any difference (α=0,05). N-MID Osteocalcin (N-MID Oc) and β-CTx can be used as parameter for early detection of osteoporosis.Key words: N-MID Osteocalcin (N-MID Oc), Osteoporosis, β-CrossLaps (β -CTx)
