Abstract

Coronary heart disease was ranked as the second cause of death in Indonesia in 2014. The purpose of this study was to determine the factors associated with the incidence of CHD in patients with DM in Kardinah Hospital Tegal. This type of research is analytic observational with case control design. Samples were taken as many as 55 cases and 58 controls by purposive sampling technique. Data were analyzed by chi square test for bivariate test and logistic regression for multivariate test. The results of the study were the relationship between age (p value=0.034,OR=2,873), family history of CHD (p value = 0.032, OR = 2.463), duration of DM (p value=0.046,OR=2.31), history of phisically activity (p value=0.043,OR=3,077), consumption of fruits and vegetables (p value=0.047,OR=2,448), consumption of fatty (p value=0,049,OR=2,277), history of obesity (p value=0,000,OR=5,391), history of dyslipidemia(p value=0.004,OR=3,338), history of hypertension(p value=0.026,OR=2,566) DM diet (p value=0.021,OR=2,531). While there is no relationship between sex, education, occupation, smoking history, duration of smoking, consumption of salty foods, coffee consumption, history of stress, and health insurance.

Abstract

The total of participants beneficiaries National Health Insurance in Puskesmas Kalongan as many as 10.715 people. In 2017 from January to October the total of beneficiaries  participants visits decreased with the lowest number of visits by 580 people and the coverage of  patient visits was less than target (90 ‰ of 150 ‰). The purpose of this research is to know the correlation between perception  about sick, knowledge and satisfaction of beneficiaries of NHI program with health service utilization. This type of research is an analytical survey with cross sectional design. Population and samples from this study were participants of beneficiaries National Health Insurance registered in Puskesmas Kalongan with systematic random sampling technique and determined by using 105 respondents. Instrument used in the research is a questionnaire. Data analysis was done by using Chi-Square test. The result of statistical test showed that there was correlation between perception about sickness (p= 0,007), knowledge (p=0,000) and satisfaction (p=0,024) with utilization of health service at Puskesmas Kalongan, East Ungaran Subdistrict with weak correlation strength (CC= 0,256, CC=0,335, CC=0,258) The research  conclude that the perception about sickness, knowledge and satisfaction is related to the utilization of health services in Puskesmas Kalongan.   Keywords: Perception about Sickness, Knowledge, Satisfaction, Utilization of Health Service

Abstract

Demam tifoid pada tahun 2014-2016 merupakan penyakit peringkat 1 dari 10 besar penyakit di rumah sakit  Kota Semarang dan di wilayah kerja Puskesmas Tlogosari Kulon terjadi peningkatan kasus dari 211 kasus, 570 kasus, dan 829 kasus. Tujuan penelitian ini adalah untuk mengetahui faktor risiko yang berhubungan dengan kejadian demam tifoid pada usia 15-44 tahun di wilayah kerja Puskesmas Tlogosari Kulon. Jenis penelitian ini adalah analitik observasional dengan rancangan case control, dengan 26 sampel kasus dan 26 sampel kontrol. Instrumen yang digunakan adalah kuesioner. Analisis data dilakukan secara univariat dan bivariat dengan menggunakan uji chi square dan uji fisher. Hasil penelitian menunjukkan bahwa ada hubungan antara cuci tangan sebelum makan (p=0,026), kebiasaan makan (p=0,002), tempat makan (p=0,035), dan tidak ada hubungan antara konsumsi es (p=0,249), konsumsi sayuran mentah (p=0,703), jenis air minum (p=0,554), dan jenis makanan (p=0,140) dengan kejadian demam tifoid.

Abstract

ABSTRACT Tuberculosis and HIV/AIDS infection has a very strong relationship, and with the presence of HIV infection the insidence  of TB disease is increasing. The purpose of this study is to determine the incidence of TB in people with HIV/AIDS health services in Semarang. The type of this research is  analytic observational with case-control research design to know the determinant of tuberculosis in people with HIV/AIDS. The research  use secondary and primary data. The sample is determined by 30 cases and 30 controls. The instrument use a questionnaire. Data analyzed by univariate analysis to describe caharacteristics and bivariate analysis.  The results obtained from factors correlation with incidence of tuberculosis in people with HIV/AIDS are smoking history (p-value = 0,002; OR= 19,33; 95% CI: 2.313-161.565), clinical stage HIV (p-value =0,000; OR=0,067; 95% CI: 0,017-0,254), history of contact with TB patients ( p-value = 0,001; OR=8; 95% CI:2,475-25,860 ). Suggestions obtained from this study are to reduce smoking actvities, always monitor the condition of the body and routine follow the treatmen and wear maskd to reduce the risk of contact with TB patients. Abstract

Puskesmas Ngesrep performance ratings have always declined in the last five years, in 2012 ranking 3, 2013 ranked 6th, 2014 ranked 20th, 2015 ranked 27th, 2016 ranked 32. due to the leadership role was not optimal. The purpose of this study was to determine the role of Puskesmas Ngesrep leaders towards achieving their performance. The type of this research is qualitative descriptive. The research informants were 11 people who were selected by purposive sampling technique. Data collection techniques with in-depth interviews. Data were analyzed qualitatively. The results of the study showed that the role of leadership that had not been maximally affected the decline in performance at the Puskesmas Ngesrep. Motivation, guidance, and coordination  have not been optimal. The leadership is still not brave and firm enough to innovate and face employees. The lack of targets before the deadline still occurs due to leadership supervision that is not yet optimal. The conclusions from this study are the implementation of the role of Leaders in Puskesmas Ngesrep is not yet optimal.

Abstract

Kabupaten Boyolali merupakan salah satu kabupaten endemik antraks di Jawa Tengah. Kasus antraks di Boyolali terus berulang. Tujuan dari penelitian ini adalah untuk mengetahui korelasi antara faktor lingkungan fisik dengan kejadian antraks di Boyolali. Desain penelitian ini adalah studi ekologi dengan pendekatan spasial dan dengan populasi subyek penelitian 49 (21 antraks manusia dan 28 antraks hewan). Penelitian dilakukan 1 Agustus menggunakan lembar observasi, citra Landsat & DEM, dan aplikasi GPS. Analisa data menggunakan univariat dan bivariat.Variabel yang signifikan adalah variabel ketinggian terhadap kejadian antraks manusia (p=0,001 dan r=-0,864) dan variabel kerapatan vegetasi terhadap kejadian antraks hewan (p=0,039 dan r=0,6). Ada korelasi antara ketinggian terhadap kejadian antraks manusia dan kerapatan vegetasi terhadap kejadian antraks hewan.

Abstract

The prevalence of periodontal disease in Klaten Regency incresed in 2017 from 2016. Jogonalan I Public Health Center has the second highest prevalence of periodontal disease cases, the number. The purpose of this study was to determine what risk factors were associated with periodontal disease status in adult male smokers. This type of research is observational analytic with aapproach case control. The sample set was 42 cases and 42 control samples with purposive sampling technique. The results showed that smoking status (p = 0.017), length of smoking (p = 0.037), number of cigarettes (p = 0.040), brushing behavior (p = 0.030), consumption of fruit and vegetables (p = 0.008) were associated with periodontal disease status in adult male smokers. The conclusion of this study is that there is a relationship between smoking status, length of smoking, number of cigarettes, tooth brushing behavior and consumption of vegetable fruit with periodontal disease status, and there is no relationship between types of cigarettes and coffee consumption with periodontal disease status. Abstract

ABSTRAK DOTS merupakan strategi untuk menanggulangi penyakit tuberkulosis. Angka kesembuhan TB di Kabupaten Semarang mengalami penurunan. Tahun 2015 sebesar 80,67% mengalami penurunan pada tahun 2016 sebesar 65,15%. Di Puskesmas Bergas angka keberhasilan pengobatan mengalami penurunan yaitu pada tahun 2016 sebesar 84,6%, tahun 2017 sebesar 79,4%. Tujuan penelitian ini adalah untuk menganalisis program penanggulangan TB Paru dengan strategi DOTS di Puskesmas Bergas. Jenis peneitian ini adalah kualitatif dengan pendekatan deskriptif. Informan utama yaitu kepala puskesmas, petugas TB, dan petugas laboratorium. Informan triangulasi yaitu Dinas Kesehatan, pasien TB, dan pengawas menelan obat (PMO). Instrumen penelitian berupa pedoman wawancara dan alat perekam. Pemeriksaan keabsahan data menggunakan triangulasi dan anaisis data menggunakan analisis isi (conten analysis) serta narasi sebagai bentuk penyajian data. Hasil penelitian menunjukkan bahwa dari segi ketenagaan, puskesmas telah sesuai dengan pedoman, namun puskesmas masih mengalami kekurangan dana. Perencanaan dan pengorganisasian sudah sesuai dengan pedoman. Penemuan, diagnosis, pengobatan dan pematauan kemajuan dah hasil pengobatan sudah sesuai dengan pedoman, namun penjaringan aktif dan kunjungan rumah kurang. Pemantauan dan Evaluasi sudah sesuai dengan pedoman. Pelaksanaan DOTS di Puskesmas Bergas sudah sesuai dengan Pedoman Nasional Tuberkulosis tahun 2014. Kata Kunci: DOTS, Analisis, Tuberkulosis ABSTRACT DOTS is a strategy for tackling tuberculosis. TB cure rates in Semarang Regency decreased. In 2015, 80.67% experienced a decline in 2016 of 65.15%. In the Bergas Health Center the treatment success rate has decreased, namely in 2016 amounting to 84.6%, in 2017 at 79.4%. The purpose of this study was to analyze the Pulmonary TB control program with the DOTS strategy at Bergas Health Center. This type of research is qualitative with a descriptive approach. The main informants were the head of the puskesmas, TB officers, and laboratory staff. Triangulation informants are the Health Office, TB patients, and drug swallowing supervisors (PMO). Research instruments in the form of interview guidelines and recording devices. Examination of the validity of the data using triangulation and data analysis using content analysis (conten analysis) and narration as a form of data presentation. The results showed that in terms of labor, the puskesmas was in accordance with the guidelines, but the puskesmas was still experiencing a lack of funds. Planning and organizing are in accordance with the guidelines. The discovery, diagnosis, treatment and monitoring of progress and results of treatment are in accordance with the guidelines, but active screening and lack of home visits. Monitoring and Evaluation are in accordance with the guidelines. DOTS implementation at Bergas Health Center is in accordance with the 2014 National Tuberculosis Guidelines. Abstract

Abstrak Angka putus berobat dari tahun 2014 hingga 2016 masing-masing angka sebesar 13,9%; 3,40% (13 kasus); dan 6,54% (14 kasus). Tujuan penelitian ini adalah untuk mengetahui determinan kejadian putus berobat penderita TB Paru di Balkesmas Wilayah Semarang. Jenis penelitian ini adalah penelitian observasional analitik dengan rancangan penelitian case control. Kasus didefinisikan penderita putus berobat Tuberkulosis yang berumur 15 tahun keatas dan tercatat pernah menjadi pasien Balkesmas Wilayah Semarang. Kontrol didefinisikan penderita Tuberkulosis yang sembuh atau pengobatan lengkap, berusia 15 tahun keatas dan tercatat pernah menjadi pasien Balkesmas Wilayah Semarang. Sampel sebesar 23 kasus dan 23 kontrol teknik purposive sampling. Penelitian ini menggunakan instrumen yakni kuesioner. Data dianalisis dengan menggunakan uji chi square. Hasil penelitian menunjukkan bahwa motivasi penderita rendah (p value 0,046) dan sedang (p-value 0,004), motivasi keluarga rendah (p-value 0,03), serta efek samping OAT berat (p-value 0,01) dan ringan (p-value 0,04) berhubungan terhadap kejadian putus berobat penderita TB Paru. Sedangkan biaya kesehatan, waktu tempuh, peran PMO , dan sikap petugas kesehatan tidak berhubungan terhadap kejadian putus berobat penderita TB Paru. Simpulan penelitian ini terdapat hubungan antara motivasi penderita, motivasi keluarga yang rendah, dan efek samping OAT terhadap kejadian putus berobat penderita TB Paru di Balkesmas wilayah Semarang. Maka perlu disarankan adanya monitoring efek samping pelayanan kesehatan kepada penderita selama masa pengobatan. Abstract The drop out rate for treatment from 2014 to 2016 was 13.9%; 3.40% (13 cases); and 6.54% (14 cases). The purpose of this study was to determine the occurrence of treatment of patients with pulmonary TB at the Balkesmas Semarang. This type of research is an analytic observational research by arranging research case controls. Cases defined by treatment for tuberculosis were carried out 15 years and above and were recorded as patients of Balkesmas Semarang Region. Control of Tuberculosis patients who have recovered or fully recovered, namely 15 years and above and pregnant patients Balkesmas Semarang Region. Samples were 23 cases and 23 controls using purposive sampling technique. This study uses an instrument namely a questionnaire. Data were analyzed using the chi square test. The results showed that patient motivation was low (p value 0.046) and moderate (p-value 0.004), family motivation was low (p-value 0.03), and side effects were severe (p-value 0.01) and mild (p -value of 0.04) Relation to treatment withdrawal in patients with pulmonary TB. While health costs, travel time , role of PMO, and attitudes of health workers (p-value) were not related to the drop out of treatment for patients with pulmonary TB. The conclusions of the study are the relationship between motivation, low family, and side effects on the drop out of treatment for pulmonary TB patients in the Balkesmas Semarang area. Then it is necessary to read monitoring the side effects of health services until the treatment period.

Abstract

Abstract. Mental disorders in children and adolescents are the focus of global health because of their relationship to suffering, functional impairment, stigmatized exposure, discrimination, and the potential for death. Based on global epidemiological data 12-13% of children and adolescents suffer from mental disorders. This study aims to determine the factors that affect emotional mental disorders in elementary school students. The cross-sectional method was used with 222 respondents from six different elementary schools. Chi-Square test analysis is used for data analysis.  121 (53.3%) subjects experienced symptoms of emotional mental disorders. The results show that the education level of the father (PR= 1,785 ;p <0.05), the level of family income per month (PR=2,345 ;p <0.05), sleep disorders (PR= 1,79 ;p <0.05), and vegetable consumption affects the incidence of emotional mental disorders (PR= 2,023; p <0.05)  Keywords: children, elemtary school, mental health disorder   Abstrak. Gangguan mental pada anak dan remaja menjadi fokus kesehatan global karena hubungannya dengan penderitaan, fungtional impairment, paparan stigma, diskriminasi, hingga potensi kematian. Berdsasarkan data epidemiologi global 12-13% anak dan remaja menderita gangguan mental. Penelitian ini bertujuan untuk mengetahui faktor yang memepengaruhi gangguan mental pada siswa sekolah dasar. Metode cross-sectional digunakan dengan jumlah responden 222 dari enam sekolah dasar yang berbeda. Analisis uji Chi-Square digunakan untuk analisa data. Hasil menunjukan 121 (53,3%) subjek terdeteksi mengalami gejala gangguan mental emosional, selanjutnya bahwa tingkat pendidikan ayah (PR=1,785; p<0,05), tingkat penghasilan keluarga per bulan (PR=2,345; p<0,05), gangguan tidur (PR= 1,79; p<0,05), dan konsumsi sayur mempengaruhi kejadian gangguan mental emosional (PR= 2,023; p<0,05).  Kata kunci: anak-anak , gangguan mental emosional, sekolah dasar
