Abstract

The hospital played a crucial role in treating health conditions and had the potential to cause negative impacts through generated medical waste. Based on the preliminary survey conducted, it was found that the management of infectious solid medical waste at Rumkital Dr. Oepomo Surabaya did not meet the requirements for containment, transportation, temporary storage, human resources, and infrastructure. Based on these issues, this study aimed to describe the management of infectious solid medical waste at Rumkital Dr. Oepomo in 2024. The method used was descriptive with a cross sectional approach. Data was collected through direct observation of the management of infectious solid medical waste. Information was also obtained through interviews with infectious solid medical waste management officers. In addition, the volume of infectious solid medical waste was measured directly in the field. The data obtained was compared with Minister of Health Regulation Number 2 of 2023. The results of this study indicated that the volume of infectious solid medical waste generated at Rumkital Dr. Oepomo Surabaya over 7 days was 5.34 kg/day. The assessment of the management process for infectious solid medical waste found that the containment, transportation, and temporary storage processes did not meet the requirements. The evaluation of supporting factors for managing infectious solid medical waste including human resources and facilities also did not meet the criteria. The conclusion was that the management process of infectious solid medical waste at Rumkital Dr. Oepomo Surabaya did not fully meet the requirements. The recommendations that can be given were as follows in the containment process medical waste should not exceed ¾ of its maximum volume and should be tied into a single braid, transportation should use trolleys, and temporary storage should involve the use of freezers. Supporting factors in waste management include the need for personnel to undergo training in medical waste handling and the procurement of complete PPE facilities for waste management personnel.

Abstract

Background: Wastewater treatment at Jombang Regional General Hospital (RSUD Jombang) has not consistently met effluent quality standards for parameters such as pH, Biochemical Oxygen Demand (BOD), and ammonia (NH₃). In addition, environmentally friendly approaches, such as eco-enzymes, have not yet been optimally implemented. This study aimed to analyze the effectiveness of the eco-enzyme in reducing pH, BOD, and NH₃ levels in hospital wastewater at Jombang Regional General Hospital. Methods: This study employed a laboratory experimental design using a post-test-only control group approach. Phytochemical analysis was conducted to identify the bioactive compounds present in the eco-enzyme solution. Wastewater samples were collected from the Wastewater Treatment Plant (WWTP) of Jombang Regional General Hospital and treated with eco-enzyme concentrations of 5%, 10%, and 15% for an incubation period of 5 days. The measured parameters included pH, BOD, and ammonia (NH₃). Statistical analysis was performed using one-way analysis of variance (ANOVA) at a significance level of α = 0.05, followed by Tukey’s Honestly Significant Difference (HSD) post hoc test to determine differences among treatment groups. Results: Phytochemical analysis identified five main bioactive compounds in the eco-enzyme solustion, namely flavonoids (14.246 mg/mL), polyphenols (3.648 mg/mL), citric acid (0.6954 g/L), lactic acid (0.05 mg/mL), and bromelain (0.714 U/mL). Eco-enzyme treatment significantly reduced all measured wastewater parameters. The mean pH decreased from 8.18 to 6.00 (26.59%), BOD decreased from 73.63 mg/L to 38.57 mg/L (47.62%), and NH₃ decreased from 9.45 mg/L to 2.78 mg/L (70.52%) at the 15% eco-enzyme concentration. One-way ANOVA indicated statistically significant differences among eco-enzyme treatment groups for pH, BOD, and NH₃ parameters (p < 0.05), with the greatest reductions observed at the 15% eco-enzyme concentration. Conclusions: Eco-enzymes demonstrated potential effectiveness in reducing pH, BOD, and NH₃ levels in hospital wastewater, particularly at a concentration of 15%. These findings suggest that eco-enzymes may serve as a sustainable and cost-effective approach for hospital wastewater treatment. Further studies are required to optimize fermentation conditions, extend treatment duration, and evaluate large-scale applications to assess long-term stability and economic feasibility.

Abstract

The 4th pillar of the Community-Based Total Sanitation (STBM) program emphasizes household waste management, which is crucial due to the significant contribution of household waste to environmental pollution. This study aimed to analyze the influence of knowledge, education, and triggering on the implementation of the 4th pillar of STBM in Mojosulur Village, Mojosari District, Mojokerto Regency in 2025. An analytic observational design with a cross-sectional approach was applied, involving 92 housewives selected through systematic random sampling. Data were collected using questionnaires to measure knowledge, education, and triggering experience, and observation sheets to evaluate household waste management practices. The results showed that the overall implementation of the 4th pillar of STBM was still low, with only 15.2% of households practicing proper waste management. The Chi-Square test indicated that knowledge had no significant association with STBM 4th pillar implementation (p = 0.109), while education (p = 0.004) and triggering (p = 0.000) were significantly associated. These findings highlight that higher education and structured triggering activities play a key role in strengthening household waste management. Local governments and environmental health officers are recommended to intensify triggering interventions and strengthen community education programs, focusing on the three main components of STBM enabling environment, increasing demand, and increasing supply to promote sustainable waste management behavior at the household level.

Abstract

Dengue Hemorrhagic Fever cases are one of the public health problems that are endemic in the Sememi Health Center working area. This study analyzes the relationship between environmental factors that influence dengue fever cases, including the larval index, rainfall, temperature, and humidity in the working area of the Sememi Community Health Center, which recorded 183 cases from 2020 to 2024.This study utilized a cross-sectional analytical design, with analysis conducted on 60 monthly secondary data collected over five years. The correlation test results show that only the number of free larvae has a significant relationship with cases of dengue fever (Sig.= 0.000), while other climate factors do not. These findings confirm the crucial role of the number of free larvae as a determining indicator of dengue fever cases. The main limitation of this study is the aggregate and secondary nature of the data, which does not allow for confirmation of cause and effect, but only correlation. Based on these findings, it is recommended that the Mosquito Nest Eradication program be strengthened as a top priority in efforts to control dengue fever.
