Abstract

ABSTRAK Hipertensi merupakan suatu kondisi dimana tekanan darah sistolik sama atau lebih dari 140 mmHg dan tekanan darah diastolik lebih dari atau sama dengan 90 mmHg. Health Anxiety atau kecemasan kesehatan adalah kondisi dimana seseorang merasa khawatir akan adanya ancaman terhadap kesehatannya. Natural scenery diketahui dapat mengurangi emosi negatif seperti rasa takut, marah, agresif, ketegangan dan kecemasan serta meningkatkan emosi positif. Green breathing therapy adalah sebuah terapi relaksasi yang dapat membantu seseorang dalam mengurangi stress dan tingkat kecemasan. Penelitian ini bertujuan untuk mengetahui pengaruh natural scenery and green breathing therapy terhadap perubahan tekanan darah dan tingkat kecemasan kesehatan pada pasien hipertensi. Rancangan penelitian ini menggunakan jenis penelitian kuantitatif pre experiment dengan pre-post test. Jumlah sampel dalam penelitian ini 80 responden, namun hanya dianalisis 48 responden yang sesuai kriteria inklusi dan eksklusi, instrument yang digunakan dalam penelitian ini menggunakan tensimeter untuk mengukur tekanan darah dan kuesioner Health Anxiety Inventory (HAI).  Teknik pengambilan sampel dalam penelitian ini menggunakan metode total sampling. Analisa data yang digunakan adalah uji wilcoxon. Hasil penelitian menunjukkan bahwa natural scenery and green breathing therapy berpengaruh secara signifikan terhadap penurunan tekanan darah sistolik dari rata-rata 153,50 mmHg menjadi 141,90 mmHg, serta tekanan darah diastolik dari 89,02 mmHg menjadi 82,19 mmHg. Tingkat kecemasan kesehatan juga menurun, ditunjukkan dengan peningkatan jumlah responden yang tidak mengalami tingkat kecemasan dari 50,0% menjadi 97,9%. Hasil uji statistik menunjukkan pengaruh signifikan terhadap tekanan darah sistolik sebesar -5,714 (p-value< 0,001), tekanan darah diastolik sebesar -4,685 (p-value< 0,001), dan tingkat kecemasan sebesar -4,421 (p-value < 0,001). Saran dari penelitian ini adalah agar natural scenery and green breathing therapy dapat dijadikan terapi untuk menurunkan tekanan darah dan mengurangi tingkat kecemasan pada lansia hipertensi.   Kata Kunci: Natural Scenery, Green Breathing Therapy, Tekanan Darah, Health Anxiety, Hipertensi

Abstract

Diabetes Mellitus (DM) is a chronic metabolic disease characterized by persistently high blood sugar levels, which can lead to complications such as diabetic foot issues. A crucial aspect for healthcare professionals providing care to DM patients is assessing the patient's level of adherence and understanding regarding self-care and disease management. Data from the 2023 SKI (Indonesian Health Survey) indicates there were 118,184 cases of Diabetes Mellitus in Central Java. Foot care education delivered via remote services assists patients in performing proper foot care to prevent complications associated with the condition. This study aimed to determine the effectiveness of an animated video based on the Theory of Planned Behavior in improving the knowledge and adherence of patients with diabetic ulcers. A quasi-experimental design was employed, utilizing a one-group pre-test and post-test approach with a control group. Incidental sampling was used to select 60 respondents. The Mann-Whitney test yielded a p-value of < 0.05 (0.017), demonstrating that the animated video based on the Theory of Planned Behavior was effective in enhancing both adherence and knowledge among patients with diabetic ulcers. The findings suggest that this animated video, leveraging audiovisual media, can be implemented in educational settings to help prevent foot complications in patients with diabetes.

Abstract

A megathrust disaster is a geological event caused by the movement of tectonic plates, leading to potentially severe damage to infrastructure due to immense pressure accumulated in the subduction zone from the friction between two plates. This disaster caused 3 people to die, 29 were injured, 151 people were displaced, out of 101,500 people who suffered. The impacts of megathrust earthquakes affect various aspects of life and the environment, including individuals’ knowledge and preparedness. Coastal traders, whose locations are often in close proximity to the coastline, are at high risk of losing their livelihoods and assets, which may increase their anxiety levels. This study aims to examine the relationship between knowledge and preparedness with the anxiety levels of coastal traders in the southern coastal region in the context of megathrust disasters. A quantitative research method with a cross-sectional design was employed. Data were collected using questionnaires measuring knowledge, preparedness, and anxiety levels. The study involved 71 respondents selected through purposive sampling. The results showed no significant relationship between knowledge and anxiety levels (p = 0.653; r = 0,054). However, a significant relationship was found between preparedness and anxiety levels (p = 0.049, r = 0,234). It is recommended that future studies explore the effectiveness of disaster education or training interventions in reducing anxiety levels among traders in high-risk coastal areas.

Abstract

Immobilization in mechanically ventilated ICU patients frequently alters cardiorespiratory stability. While regular repositioning is standard nursing care to prevent complications, the optimal duration tolerated before hemodynamic shifts occur remains poorly characterized. Objective to analyze the effect of repositioning and its duration on hemodynamic and respiratory status among mechanically ventilated patients in the Intensive Care Unit. A pre-experimental study with a one-group pretest-posttest design was conducted in the ICU of Telogorejo Hospital, Semarang. A total of 20 mechanically ventilated patients were recruited via accidental sampling. Physiological parameters heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP), oxygen saturation (SpO2), respiratory rate (RR), and tidal volume (TV) were measured before and immediately after repositioning until physiological changes occurred (maximum duration 2 hours). Data were analyzed using paired comparative tests and correlation analyses. Repositioning significantly altered all measured cardiorespiratory parameters (p<0.05): HR increased from 89.00 ± 15.45 to 103.00 ± 18.79 bpm (p = 0.003), SBP from 119.00 ± 26.65 to 129.00 ± 23.49 mmHg (p = 0.014), DBP from 63.00 ± 15.68 to 67.00 ± 16.10 mmHg (p = 0.006), SpO2 from 99.00 ± 0.92% to 100.00 ± 0.59% (p = 0.035), RR from 16.00 ± 4.55 to 27.00 ± 9.01 breaths/min (p = 0.000), and TV from 441.00 ± 64.97 to 476.00 ± 76.77 mL (p = 0.002). The mean duration before parameter changes was 46.75 ± 10.42 minutes. Repositioning duration showed a significant moderate positive correlation only with changes in diastolic blood pressure (r = 0.602, p = 0.005), while changes in other parameters were not significantly correlated with duration (p > 0.05). Repositioning induces significant physiological responses in mechanically ventilated patients, with hemodynamic shifts emerging at an average of 47 minutes. Critical care nurses should adopt individualized repositioning intervals tailored to patient stability rather than relying solely on rigid two-hour protocols.
