Abstract

The increasing use of herbal products among patients with diabetes mellitus highlights the need for a clear understanding of their potential interactions with oral antidiabetic drugs (OADs). Such interactions may be synergistic or may pose risks, particularly hypoglycemia. To identify the mechanisms underlying interactions between herbal products and OADs and to assess their relevance in the management of type 2 diabetes mellitus. A literature search was conducted through Google Scholar, PubMed, and ScienceDirect using keywords related to herbal products, oral antidiabetic drugs, and drug interactions. Articles were selected based on inclusion criteria (2015–2025, original research, human subjects with type 2 diabetes mellitus or animal models, English or Indonesian language, and involving OADs) and relevant exclusion criteria. The most frequently reported interactions were pharmacodynamic, particularly the combination of Cinnamomum verum, Curcuma longa, and Momordica charantia with metformin or sulfonylureas, which enhanced insulin sensitivity, activated AMPK, or stimulated insulin secretion, potentially increasing the risk of hypoglycemia. Pharmacokinetic interactions were also identified, such as ginseng-induced modulation of the OCT1 transporter, which may influence metformin accumulation. Several other herbal products, including Aloe vera, demonstrated glucose-lowering activity; however, evidence of direct interactions with OADs remains limited, suggesting a primarily supportive role. Interactions between herbal products and oral antidiabetic drugs have important clinical implications, as they may alter both therapeutic efficacy and safety. Close monitoring and patient education are essential, alongside further research to clarify interactions that remain insufficiently documented.

Abstract

Telemedicine and telepharmacy play an important role in expanding access to pharmaceutical services, particularly in rural areas that face shortages of pharmacy personnel and healthcare facilities. However, comparative syntheses focusing on the differences in telemedicine implementation between rural and urban communities in Indonesia remain limited. This literature review aims to analyze the role of telemedicine in improving pharmaceutical service accessibility and to compare its implementation between rural and urban communities. The review was conducted systematically using PubMed and Google Scholar databases with the keywords “telemedicine, rural and urban telepharmacy, and telemedicine accessibility.” From 540 identified articles, 11 articles were selected based on inclusion and exclusion criteria. The findings indicate that telemedicine facilitates medication consultations, e-prescriptions, patient education, and chronic disease monitoring through various digital platforms. Several services, such as WhatsApp, Halodoc, and other digital health applications, have supported the improvement of pharmaceutical service accessibility. A significant gap was identified between telemedicine implementation in urban and rural areas, where urban communities demonstrated more optimal implementation due to better technological infrastructure and digital literacy, while rural communities continued to face limitations related to internet connectivity, healthcare facilities, and technology adoption. This literature review concludes that telemedicine significantly contributes to improving efficiency and promoting equitable access to pharmaceutical services between rural and urban communities.

Abstract

Drug interactions between antibiotics and antacids represent an important clinical issue that can affect therapeutic efficacy and increase the risk of treatment failure. The concurrent use of these drug classes frequently occurs in clinical practice, particularly in patients receiving antibiotic therapy who also present with gastrointestinal complaints. This systematic literature review aims to evaluate the clinical trial evidence regarding antibiotic–antacid interactions, focusing on pharmacokinetic and pharmacodynamic mechanisms, clinical implications, and current research limitations. Relevant articles were identified through PubMed, Google Scholar, and other scientific databases using standardized keywords. Literature selection followed the PRISMA guidelines to ensure the quality and relevance of included studies. The analysis revealed that antacids and proton pump inhibitors (PPIs) can significantly reduce the bioavailability of antibiotics, especially fluoroquinolones and tetracyclines, through chelation with metal ions and by increasing gastric pH, thereby impairing drug absorption. Concomitant use of antibiotics and PPIs was also associated with a higher risk of Clostridioides difficile infection (CDI), particularly in elderly or comorbid patients. Conversely, newer acid-suppressing agents such as vonoprazan demonstrated good efficacy in Helicobacter pylori eradication regimens without compromising antibiotic activity. Despite these findings, most studies were retrospective, with limited sample sizes and specific populations. Therefore, large-scale prospective clinical trials are needed to strengthen the evidence base. A comprehensive understanding of antibiotic–antacid interactions is essential to support rational, safe, and effective prescribing practices in clinical settings.
