Abstract

ABSTRACT Background: Laryngeal cancer is one of the upper respiratory tract malignancies strongly associated with smoking habits. Adolescents are considered a high-risk group because they are easily influenced by peers and generally have low levels of health literacy. Limited understanding of the risks of laryngeal cancer may affect adolescents’ attitudes and smoking behaviors. Objective: This study aimed to determine the relationship between the level of knowledge about laryngeal cancer and attitudes as well as smoking behavior among students of SMA Negeri 1 Palmatak. Methods: This study used a descriptive analytic design with a cross-sectional approach. The study population consisted of all students of SMA Negeri 1 Palmatak, with 140 respondents selected using a total sampling technique. Data were collected using a structured questionnaire. Data analysis was performed using Spearman’s rho test to examine the relationship between knowledge and attitude, and the Lambda (λ) test to analyze the relationship between knowledge and smoking behavior. Results: Most students had good knowledge (53.6%), showed a positive attitude toward smoking prevention (65.7%), and were non-smokers (62.1%). The analysis showed a significant relationship between knowledge and attitude toward smoking (r = 0.730; p < 0.001). In addition, the Lambda test indicated a strong association between knowledge and smoking behavior (λ = 0.698; p < 0.001). Conclusion: The results revealed a strong and statistically significant association between knowledge of laryngeal cancer and smoking-related attitudes and behaviors among students at SMA Negeri 1 Palmatak. Abstract

ABSTRACT Background: Noise Induced Hearing Loss (NIHL) is a hearing impairment due to continuous or certain duration of noise exposure. This hearing impairment usually develops slowly over several years, this hearing impairment can occur due to several factors such as noise intensity that exceeds the threshold value, age and also length of service. This study aims to analyze the relationship between noise intensity and noise-induced hearing impairment (NIHL) in production workers at PT X. Methods: This study is an observational analytical study with a cross-sectional approach. The study sample consisted of 67 workers in the production department of PT X. Data were obtained through workers' medical records and analyzed using the Chi-square test with a significance level of 95% (p ≤ 0.05). Results: Of the total 67 respondents, 35.8% were exposed to noise with an intensity of <85 dB and 64.2% were exposed to noise with an intensity of >85 dB. As many as 46.3% of workers did not experience noise-induced hearing loss (NIHL), and 53.7% of workers experienced noise-induced hearing loss (NIHL). Bivariate analysis showed a significant relationship between noise intensity and noise-induced hearing loss (NIHL) (p = 0.001). Conclusion: There is a significant relationship between noise intensity and noise-induced hearing loss (NIHL). Therefore, close monitoring and education regarding NIHL risk factors need to be improved to reduce the incidence and complications caused. Keywords: Noise Intensity, Hearing Loss, NIHL

Abstract

Background: Acute appendicitis is the most common abdominal surgical emergency, yet its diagnosis remains a clinical dilemma owing to frequently atypical presentations. Clinical scoring systems were developed to expedite and improve diagnostic accuracy. This study aimed to compare the diagnostic accuracy of the RIPASA and Alvarado scores against histopathology as the gold standard. Methods: A retrospective cross-sectional analytic study of 125 patients with suspected acute appendicitis who underwent appendectomy at Hj. Bunda Halimah Hospital, Batam, between 1 January 2022 and 31 December 2024. The Alvarado (cut-off ≥7) and RIPASA (cut-off ≥7.5) scores were derived from medical records and compared with histopathology. Sensitivity, specificity, positive (PPV) and negative (NPV) predictive values, accuracy, and the area under the curve (AUC) were analysed. Results: Histopathology confirmed appendicitis in 114 of 125 patients (91.2%; negative appendectomy rate 8.8%). The Alvarado score yielded a sensitivity of 87.7%, specificity 63.6%, PPV 96.1%, NPV 33.3%, and accuracy 85.6% (AUC 0.797; 95%CI 0.715–0.863). The RIPASA score yielded a sensitivity of 91.2%, specificity 54.5%, PPV 95.4%, NPV 37.5%, and accuracy 88.0% (AUC 0.847; 95%CI 0.772–0.905). Conclusion: Both scores demonstrated good and comparable discriminative capacity; RIPASA tended to be more sensitive and marginally more accurate, whereas Alvarado was more specific. The overlapping AUC confidence intervals preclude a conclusion of statistically significant superiority of RIPASA. Both instruments are suitable screening tools in resource-limited, imaging-scarce settings.
