Abstract

Hallucinations are a sensory perception disorder characterized by the emergence of false sensations, including auditory, visual, olfactory, tactile, and gustatory stimuli that do not actually exist or are unreal. Data from Prof. Hb. Saanin Psychiatric Hospital in Padang indicate that hallucinations have been the most common nursing diagnosis over the past three years (2023–2025). with a total of 17,591 patients. The purpose of this study is to describe the implementation of psychiatric nursing care for Mrs. L with auditory hallucinations in Melati Ward. The method used is a case study with a nursing process approach through five stages: assessment, diagnosis, planning, implementation, and evaluation, conducted over six meetings. The assessment results showed that Mrs. L experienced signs and symptoms of hallucinations, such as hearing whispers and an inability to perform self-care. Case analysis indicates that implementation strategy interventions (SP), such as rebuking hallucinations, medication adherence, engaging in conversations, and scheduled activities, are effective in helping patients control hallucinations. The discussion demonstrates an improvement in the patient’s ability to manage hallucination symptoms and increased independence in self-care. It is expected that the results of this research will increase public understanding of psychiatric nursing care. This understanding will serve as a basis for building awareness, maintaining mental health, and adopting behaviors that support optimal mental well-being.

Abstract

Hallucinations are a form of perceptual disorder, a condition where a person experiences sensory stimulation without any real stimulus from the surrounding environment. Based on medical record data from RSJ Based on medical record data from RSJ Prof. HB. Saanin Padang, hallucinations have become the most common nursing diagnosis in the last 3 year (2023-2025) with a total of 17,591 clients Prof. HB. Saanin Padang. When hallucinations occur, individuals often feel very anxious, panicked, and have difficulty distinguishing between what they imagine and the reality they are experiencing. This study aims to describe the application of psychiatric nursing care to patients with auditory and visual hallucinations at Wisma Flamboyan. The method used was a case study with a nursing process approach that includes assessment, diagnosis, planning, implementation, and evaluation. The results of the assessment showed that the client experienced signs and symptoms of hallucinations in the form of hearing whispers and seeing shadows that were not there. The main nursing diagnosis was hallucinations with a co-diagnosis of self-care deficit. After being given nursing care, the client was able to control his hallucinations by implementing hallucination implementation strategies, namely SP 1-4. These results indicate that SP implementation is effective in helping clients control hallucinations. Self Care Deficit diagnoses can also be addressed with implementation strategies (SP), namely 1-4. This research is expected to increase public understanding and awareness of the importance of maintaining mental health to achieve optimal mental well-being.

Abstract

Hypertension is a chronic disease characterized by an increase in blood pressure ≥140/90 mmHg and commonly occurs in the elderly. Hypertension can cause serious complications such as stroke, heart failure, and kidney failure if not properly managed. This study aimed to provide gerontic nursing care for hypertensive patients at the Sabai Nan Aluih Sicincin Social Nursing Home, Padang Pariaman Regency. The method used was a case study with a nursing process approach conducted on one hypertensive patient from February 26  to March 3, 2026. The assessment results showed that the patient experienced chronic pain, ineffective health management, and risk of falls. Nursing interventions included pain management using deep breathing relaxation techniques, progressive muscle exercises, blood pressure monitoring, health education, a low-salt diet, and fall risk prevention. Nursing evaluation showed an increase in the patient’s knowledge regarding hypertension and the importance of adherence to treatment. After five days of nursing interventions, there was a decrease in pain intensity, improved therapy adherence, and improvement in the patient’s general condition. The recommendations given were to maintain a healthy lifestyle, improve medication adherence, and routinely monitor blood pressure. The conclusion of this study showed that the implementation of comprehensive and systematic gerontic nursing care was highly effective in overcoming nursing problems in hypertensive patients.

Abstract

Risk of violent behavior is one of the psychiatric nursing problems in patients with severe mental disorders caused by the inability to control emotions and aggressive impulses, which may potentially harm themselves, others, or the environment. Based on medical record data at RSJ Prof. HB. Saanin Padang, cases of risk for violent behavior remain relatively high, with more than 100 patients treated each year. This condition indicates that the risk of violent behavior continues to be a serious issue in psychiatric nursing services and requires optimal and continuous management. The purpose of this case study was to obtain an overview of the implementation of psychiatric nursing care for patients with the risk of violent behavior. The method used was a case study approach through the nursing process, including assessment, diagnosis, intervention, implementation, and evaluation. Nursing interventions were provided through deep breathing exercises, hitting pillows, education on medication adherence, effective verbal communication, and spiritual activities. The results showed that the patient became calmer, more cooperative, able to control anger, did not use harsh words, and adhered to medication. It can be concluded that psychiatric nursing care is effective in reducing the signs and symptoms of the risk of violent behavior. Nurses and families are expected to play an active role in supporting treatment and preventing patient relapse.

Abstract

Hallucinations are false perceptual distortions that occur in maladaptive neurobiological responses, patients actually experience sensory distortions, but respond to them as real. WHO data in 2024 shows that mental disorders have increased to exceed 1.1 billion people. Based on preliminary data obtained from Prof. HB Saanin Mental Hospital, Padang in 2026, the most common nursing problem in the last three years was hallucinations with a total of 17,446 patients. This study aims to apply psychiatric nursing care to patients with hallucinations at Prof. HB Saanin Mental Hospital, Padang. The method used in this scientific paper is a case study with a nursing process approach that includes assessment, diagnosis, planning, implementation, and evaluation. The implementation of nursing care to patients was carried out from February 14 to 18, 2026, with 5 visits. The results of the assessment showed that the patient experienced signs and symptoms of hallucinations in the form of hearing whispers and seeing shadows. The main nursing diagnosis was hallucinations with a companion diagnosis of Self-Care Deficit (DPD). The results of the implementation of Mental Health Nursing Care showed that the patient was able to control his hallucinations independently through implementation strategies (SP) 1-4. After the hallucination management nursing intervention was carried out, these results showed that the implementation of implementation strategies was effective in helping patients control hallucinations and improving self-care skills. Through this study, it is hoped that everyone can understand the importance of mental health nursing care, thereby increasing awareness in maintaining and preserving mental health, and implementing behaviors that support the achievement of mental well-being in everyday life.
