Abstract

Pulmonary tuberculosis (TB) is a contagious infectious disease that remains a global and national health problem. It is caused by Mycobacterium tuberculosis and is transmitted through droplets. Based on data from the World Health Organization (WHO) in 2024,the number of TB cases reached approximately 10.7 million worldwide.In Indonesia, there were around 889,000 cases,while in West Sumatra Province there were approximately 25,000 tuberculosis cases. At RSUD Prof. H. Muhammad Yamin, S.H. Pariaman,the number of cases in the last three years was 56 cases in 2023, 69 cases in 2024, and 45 cases in 2025.The purpose of this case study is to provide nursing care for a pulmonary TB patient in the pulmonary inpatient ward at RSUD Prof. H.Muhammad Yamin, S.H. The method used is a case study with a duration of 5 days,conducted from February 5 to February 9, 2026, in the pulmonary ward of RSUD Prof. H.Muhammad Yamin,S.H.Pariaman.The approach used is the nursing process,which includes assessment,diagnosis, intervention,implementation, and evaluation stages,with one patient diagnosed with pulmonary TB as the subject.The results identified six nursing diagnoses: ineffective airway clearance,hyperthermia,disturbed sleep pattern,risk of nutritional deficit,risk of infection,and knowledge deficit.The nursing interventions provided included airway management,hyperthermia management,sleep support,nutritional management,infection prevention,and health education.The evaluation over five days showed that all nursing problems were resolved,and the patient was allowed to be discharged.Patients and their families are expected to adhere to the full course of treatment,adopt a healthy lifestyle, use masks, and increase the intake of nutritious food to support recovery and prevent transmission.

Abstract

According to data from the World Health Organization (WHO), the global prevalence of diabetes reached 11.11%, or approximately 589 million adults, in 2024 and is projected to increase to 853 million (12.96%) by 2050. In Indonesia, the prevalence of diabetes has risen from 5.7% in 2007 to 11.7% in 2023, with the number of cases reaching 20.4 million and expected to continue increasing. In West Sumatra, the prevalence based on physician diagnosis is 1.6%, with cases increasing from 43,464 in 2021 to more than 52,000 in 2023. The city of Pariaman has the highest prevalence in West Sumatra at 2.23%, with the number of cases continuing to rise annually. The purpose of this case study is to enable the researcher to provide nursing care for a patient with Type II Diabetes Mellitus. The research method used is a case study design, in which one patient was selected as the sample and managed through a nursing care approach. The assessment was conducted on February 9, 2026. Based on the assessment, the nursing diagnoses identified were: unstable blood glucose levels, impaired skin integrity, risk of infection, and risk of nutritional deficit. The interventions implemented included hyperglycemia management, wound care, medication administration, and nutritional management, all carried out in accordance with the established care plan. After five days of implementation, the outcomes showed a reduction in tingling in the right foot, increased comfort after dressing changes, a decrease in fever, and improved appetite. It can be concluded that appropriate nursing care can improve the patient’s health status. It is expected that the patient and family will be able to provide proper home care, anticipate potential complications, and offer support during the recovery process, maintain a healthy lifestyle—particularly in terms of diet—and utilize nearby healthcare facilities. These measures are important to ensure that the patient’s blood glucose levels remain controlled and to help prevent disease within the family.

Abstract

Acute gastroenteritis is an infectious disease that remains a global and national health problem caused by pathogenic microorganisms such as bacteria, viruses, or parasites that enter the digestive tract. Based on data from the World Health Organization (WHO) in 2024, the number of acute gastroenteritis cases reaches 1.7 billion each year. According to the Indonesian Ministry of Health, gastroenteritis is the second most common disease in Indonesia with around 2.2 million cases, while in West Sumatra there were 43,706 recorded cases. In Pariaman City, 1,325 cases were treated out of a target of 2,544. Medical record data at RSUD Prof. H. Muhammad Yamin, S.H. Pariaman showed that the number of patients with gastroenteritis in 2025 reached 219 cases. The purpose of this case study is to describe the application of nursing care for patients with acute gastroenteritis. The method used was a case study through observation and interviews conducted for 5 days, from February 11 to 15, 2026, in the Internal Medicine Ward of Prof. H. Muhammad Yamin Regional Public Hospital, S.H Pariaman, involving one patient with acute gastroenteritis. The results of the study identified six nursing problems: diarrhea, hypovolemia, acute pain, risk for ineffective peripheral perfusion, risk of infection, and risk of nutritional deficit, with diarrhea as the primary priority. Evaluation over five days of care showed that all nursing problems gradually improved and were resolved, allowing the patient to be discharged in a stable condition. Patients is expected to maintain a proper diet, food and environmental hygiene, as well as a healthy lifestyle.

Abstract

Lung cancer is a malignancy of the lung tissue or bronchi caused by uncontrolled cell growth and is one of the cancers with the highest mortality rates. According to Globocan 2022 data, there were approximately 2.3 million new cases and 1.8 million deaths from lung cancer. In Indonesia, 408,661 cases were recorded, resulting in 242,099 deaths. This indicates that lung cancer remains a serious health issue requiring attention in prevention and management efforts. An increase in cases has also been observed in West Sumatra, with 1,142 cases at Dr. M. Djamil General Hospital in Padang during the 2021–2025 period. Therefore, this study aims to describe nursing care for lung cancer patients admitted to the pulmonary inpatient ward in 2026. The method used was a case study conducted over 5 days, from January 30 to February 3, 2026, in the Pulmonary Ward of Dr. M. Djamil General Hospital in Padang, involving one patient. The research findings, obtained through interviews, observations, and physical examinations, identified six nursing diagnoses: ineffective airway clearance, ineffective breathing pattern, acute pain, nutritional deficit, sleep disturbance, and risk of infection. The evaluation showed that several issues had been partially resolved, such as airway clearance, ventilatory support, pain, and improved intravenous medication administration, while nutritional deficit and sleep pattern disturbance had been resolved. Following the provision of nursing care, it can be concluded that the patient and family are advised to understand the condition of lung cancer, adhere to treatment and undergo regular follow-ups, adopt a healthy lifestyle, and provide optimal support to improve the patient’s quality of life.

Abstract

Appendicitis is an inflammatory condition of the appendix caused by irritation and infection of the appendiceal wall as a result of obstruction in the lumen. The Indonesian Health Survey (SKI) reported that in 2023 there were 621,435 cases of appendicitis in Indonesia. Medical record data at Prof. H. M Yamin, S.H Pariaman Regional Hospital showed that the number of patients with appendicitis in 2025 was 88 cases. The purpose of this case study was to obtain an overview of the implementation of nursing care for patients with appendicitis and to improve students’ ability to provide nursing care for patients with appendicitis in the Surgical Inpatient Ward of Prof. H. M Yamin, S.H Pariaman Regional Hospital in 2026. The method used was a case study including interviews and observations conducted in the Surgical Inpatient Ward of Prof. H. M Yamin, S.H Pariaman Regional Hospital for 5 days, from February 8 to February 12, 2026, with the sample being Ms. C diagnosed with appendicitis. The results of the case study identified five nursing problems: acute pain, hyperthermia, anxiety, impaired physical mobility, and risk of infection. Nursing interventions carried out included pain management, hyperthermia management, anxiety reduction, mobilization support, and infection prevention. Evaluation obtained during the 5 days showed that all nursing problems, namely acute pain, hyperthermia, anxiety, impaired physical mobility, and risk of infection, were resolved so that the patient was allowed to go home. Patients are advised to maintain a balanced, high-fiber nutritious diet and comply with treatment recommendations to accelerate recovery and prevent complications.

Abstract

Chronic Kidney Disease (CKD) is a global health problem with an increase in the burden of disease of around 29%–41% in the last three decades. The prevalence of CKD in all stages (1–5) reached 13.4%, while advanced stages (3–5) were around 10.6%. Globally, around 788 million people live with CKD (±14.2%) and it is the 9th leading cause of death in the world (WHO, 2023; ISN, 2023). In Indonesia, the prevalence of CKD was 3.8% (Riskesdas), while in West Sumatra it was around 0.2%. Prof. H. Muhammad Yamin Regional General Hospital in 2025 recorded 335 cases of CKD with 16 deaths in CKD Stage V, indicating that CKD is still a serious problem, especially in advanced stages with complications of hypervolemia due to decreased kidney function. This study aims to describe medical-surgical nursing care for patients with CKD Stage V during 4 days of treatment. The method used was a descriptive case study on Mrs. S in the Internal Room of Prof. H. Muhammad Yamin Regional Hospital through interviews, observations, physical examinations, and documentation. The results established seven nursing diagnoses, namely, ineffective breathing patterns, hypervolemia, ineffective peripheral perfusion, activity intolerance, nausea, acute pain, and the risk of impaired skin/tissue integrity. Interventions focused on fluid management, respiratory optimization, activity intolerance, pain management, and prevention of impaired skin integrity. Evaluation showed temporary improvement in nausea and pain., ineffective breathing patterns, ineffective peripheral perfusion, and activity intolerance improved for a while, but decreased on the fourth day. The risk of impaired skin integrity was not found until the end of treatment, but the patient's condition worsened until he died on the fourth day due to the progression of terminal illness. The conclusion emphasized the need for close, comprehensive, and continuous monitoring in Stage V CKD patients and improvement of nursing service facilities.
