Abstract

Mutu layanan primer di Indonesia masih menjadi tantangan meskipun Jaminan Kesehatan Nasional (JKN) telah memperluas akses masyarakat terhadap pelayanan kesehatan. Penelitian ini bertujuan untuk menganalisis peran BPJS Kesehatan dalam penguatan layanan primer melalui mekanisme pembiayaan dan tata kelola sistem. Metode penelitian yang digunakan adalah tinjauan literatur sistematis terhadap artikel dan jurnal ilmiah yang diterbitkan pada periode 2018-2024. Penelusuran sumber dilakukan melalui Google Scholar dan perangkat Publish or Persih. menggunakan kata kunci BPJS Kesehatan, layanan kesehatan primer, Jaminan Kesehatan Nasional, dan strategic purchasing. Hasil kajian menunjukkan bahwa BPJS Kesehatan berperan strategis sebagai pembeli layanan kesehatan (strategic purchaser) dengan menerapkan mekanisme kapitasi dan insentif berbasis kinerja di fasilitas kesehatan tingkat pertama (FKTP). Mekanisme ini berkontribusi pada peningkatan efisiensi pembiayaan hingga 10–15% pada FKTP yang mencapai indikator mutu dan keberhasilan program Prolanis. Namun, pelaksanaannya belum optimal karena keterbatasan kapasitas manajerial, sumber daya manusia, serta infrastruktur teknologi informasi, terutama di wilayah terpencil. Dalam perspektif Health System Strengthening Framework WHO, aspek pembiayaan yang dikelola BPJS merupakan salah satu pilar penting, namun keberhasilan penguatan layanan primer menuntut dukungan pada pilar lain seperti tenaga kesehatan, sistem informasi, dan tata kelola. Penelitian ini menyimpulkan bahwa integrasi kebijakan melalui program Integrasi Layanan Primer (ILP) menjadi solusi strategis untuk memperkuat koordinasi antarlembaga, meningkatkan mutu layanan, dan mempercepat pencapaian Universal Health Coverage di Indonesia.

Abstract

Cervical cancer is the uncontrolled growth of cells (cancer) in the cervix. All women are at risk of developing this cancer. However, sexually active women are more likely to be affected. In its early stages, cervical cancer usually does not cause easily recognizable symptoms. The most common symptom is abnormal bleeding, for example, bleeding after sex, outside the menstrual cycle, or after menopause. The benefits and objectives of this outreach program are expected to improve women's knowledge and quality of life regarding cervical cancer prevention. The method used in this program is to provide health education through counseling about cervical cancer and cervical cancer prevention practices such as proper vaginal hygiene. Based on a self-awareness survey, the most common problem identified was a lack of knowledge about cervical cancer and a lack of awareness of early cervical cancer prevention methods. The priority problem was the lack of knowledge among the Sawangan Village community about early cervical cancer detection in women of childbearing age (WUS). To address this, we implemented one effort by conducting outreach on cervical cancer for women of childbearing age (WUS) and providing guidance on proper vaginal hygiene to prevent cervical cancer. After the outreach and practice of cervical cancer prevention, participants had a better understanding of cervical cancer and how to prevent it.
