Abstract

Hypertension is a major risk factor for chronic kidney disease (CKD) with a complex bidirectional relationship. Appropriate selection of antihypertensive drugs in patients with hypertension and end-stage CKD is crucial to prevent disease progression and improve clinical outcomes. This study aimed to compare the effectiveness of Calcium Channel Blockers (CCB) and Angiotensin Receptor Blockers (ARB) on changes in blood pressure and kidney function in patients with hypertension and stage 5 CKD at RSI Ibnu Sina Bukittinggi. A descriptive-analytic design with a retrospective approach was employed, involving 30 patients who met the inclusion criteria from a total of 78 patients during the period February 2023–December 2024. Parameters observed included systolic/diastolic blood pressure, serum creatinine, glomerular filtration rate (GFR), and urea levels before and after therapy. The results showed that both therapy groups experienced reductions in blood pressure and improvements in kidney function. The decrease in systolic/diastolic blood pressure for CCB was 14.7/11.4 mmHg, while for ARB it was 14.2/11.2 mmHg. Increases in GFR, reductions in creatinine, and decreases in urea occurred in both groups with no statistically significant differences (p>0.05). In conclusion, CCB and ARB have comparable clinical effectiveness in controlling blood pressure and improving kidney function parameters in patients with hypertension and end-stage CKD.

Abstract

The use of antibiotics without prescription remains a community health issue that increases the risk of bacterial resistance and reduces treatment effectiveness. This study aimed to determine the relationship between the level of knowledge and attitudes of the community regarding the use of antibiotics without prescription in two pharmacies located in Pangkalan Koto Baru District. The research employed an analytic design with a cross-sectional approach. The study involved 100 respondents selected using purposive sampling who met the inclusion criteria. Data were collected using a structured questionnaire covering sociodemographic characteristics, knowledge, and attitudes related to antibiotic use. Data were analyzed using univariate, bivariate chi-square tests, and multivariate analysis to determine the strength of associations between variables. The results showed that most respondents had a moderate level of knowledge and a positive attitude toward the use of antibiotics without prescription. Chi-square analysis indicated a significant relationship between knowledge level and community attitudes (p < 0.05). The study concluded that better knowledge is associated with more positive attitudes toward rational antibiotic use. Continuous education through pharmacies and healthcare providers is necessary to improve community awareness of appropriate antibiotic use and prevent the emergence of resistance.

Abstract

Bronchopneumonia is an inflammatory lung condition affecting one or more lobes, characterized by infiltrative patches caused by bacteria, viruses, fungi, and foreign substances. The purpose of this study was to obtain an overview and analysis of the appropriateness of antibiotic use. This was a descriptive observational study with a retrospective approach using secondary medical record data. The results for the description of antibiotic use were Seftriakson (53.4%), Sefotaksim (27.6%), and meropenem (19.0%). The results of the quality assessment using the Gyssens Method revealed that 1.7% of antibiotic prescriptions were in category III B, 19% in category IV A, and 79.3% in category 0. These data indicate that there is still irrational use of antibiotics. Assessed using the ATC/DDD methodology, resulting in a total of 25.57 DDD per 100 days. The calculation findings show the highest Defined Daily Dose (DDD) value for the antibiotic Seftriakson at 18.06 DDD per 100 days of hospitalization, while the lowest value was seen for Meropenem at 2.09 DDD per 100 days and Sefotaksim at 5.42 DDD per 100 days of hospitalization. Clinical parameters showed a P value > 0.05, with p = 0.88, indicating no statistically significant difference between antibiotic administration and leukocyte levels in pediatric bronchopneumonia patients in the Melati ward at Mukomuko Hospital in 2024.
